Mild traumatic brain injury does not produce post-traumatic stress disorder.

It has been widely assumed that patients who sustain mild traumatic brain injury (MTBI) or post-concussive syndrome develop post-traumatic stress disorder (PTSD) in response to their cognitive difficulties, diminished coping skills, or other losses. This study examined 70 patients who had previously been diagnosed as having either PTSD or MTBI. Each patient was asked to provide a highly detailed chronological history of the events which preceded, followed, and occurred during the traumatic event, to indicate whether they were rendered unconscious or had amnesia for the event, and to describe t

Psychosocial and emotional sequelae of individuals with traumatic brain injury: a literature review and recommendations.

The psychosocial problems of decreased social contact, depression, and loneliness that occur for many with traumatic brain injury (TBI) create a major challenge for enhancing efforts at community re-entry. Despite dramatic physical progress within the first six months after injury, these psychosocial problems remain a persistent long-term problem for the majority of individuals with severe TBI. Researchers have consistently suggested that the psychosocial problems associated with TBI may actually be the major challenge facing rehabilitation. The majority of individuals who sustain TBI are youn

Psychosexual problems in people with HIV infection: controlled study of gay men and men with haemophilia.

HIV infection can be associated with major psychological and social disturbance. Psychosexual problems would be expected to arise in the context of the infection, in view of the contribution that sexual behaviour can make to the acquisition and spread of HIV infection. Here the results of a study of the psychosexual consequences of HIV infection in gay men and men with haemophilia are presented, with the inclusion of data from control groups. Sixteen HIV-positive and 23 HIV-negative gay men, and 20 HIV-positive and 24 HIV-negative men with haemophilia with sexual partners were studied. HIV inf

[Cognitive and behaviorally relevant aspects of compliance with prescribed drugs by geriatric patients].

Studies of compliance with drug treatment in the elderly have mainly focused on treatment variables, and patient knowledge or comprehension. However, information on behavioral aspects is sparse. Therefore, interviews concerning medication were performed in 31 geriatric patients, mean age 79.6 years. The aim was to evaluate whether there were relations between coping with health problems and attitudes towards/taking of medication prescribed. It was concluded from the analysis of these tape-recorded interviews that different forms of coping strategies were reflected by patients' attitudes toward

[Screening for psychogenic factors in functional dysphonia].

Psychosocial influences are now increasingly accepted to be important factors in the etiology of many diseases. The application of these findings is often not possible in clinical routine, especially because there is a lack of practical instruments for the diagnoses of psychogenic disorders. Based on previous investigations determining relevant psychogenic factors causing functional dysphonia, the goal of this study was to develop an easy-to-manage screening sheet for the estimation of psychogenic factors in the etiology of functional dysphonia. With this approach, physicians not trained in ps

The social consequences of surgical complications for patients with proximal femoral fractures.

A case-matched study was performed to assess the social effects of surgical complications of operative fixation of fractures of the proximal femur. Fifty-two patients aged over sixty-five, half with deep wound infection, the remainder with serious early mechanical problems requiring revision surgery to correct these, had their hospital fatality, length of stay, mobility and social dependency at admission and on discharge compared with patients free from such complications. Deep wound infection was associated with a high fatality and in the survivors a prolonged period in hospital, a marked dec

Psychological functioning before and after treatment of torticollis with botulinum toxin.

The impact of botulinum toxin injection on psychological function was assessed in 26 patients with idiopathic torticollis. Eighty five per cent of the patients and 88% of the relatives considered torticollis to be better following the injections. Symptomatic improvement with the injections was associated with significant reduction of depression and disability, but non-significant improvement in body concept, and self-esteem. This concordant pattern of change in symptoms and psychological function with the injections supports the proposal that in torticollis depression and disability are conseq

The angry liver, the anxious heart and the melancholy spleen. The phenomenology of perceptions in Chinese culture.

The purpose of this article is to advance a different approach to the phenomenology of the "lived-body." To understand the role of the body in generating culture, traditional constraints influencing bodily perceptions are considered. The alternative is found in the phenomenological method of bodily perceptions. Numerous examples from traditional Chinese medicine, based on research in China, illustrate a wealth of symptoms, sensations, and their relation to the world of emotions. These examples provide arguments for collapsing the strict distinction between somatic changes and emotions as based

The role of support in influencing outcome of end-stage renal disease.

An important part of health behavior variability is a supportive environment. This paper examines the concept and discusses its application to a general and geriatric home dialysis population. Social environment is analyzed along three interrelated dimensions: social support, social integration, and social network. The authors conclude that a multimethod approach for assessing environmental support is preferred over a single-item index. Furthermore, a supportive interpersonal environment is important for patients and family, both as a preventive agent and as a protective buffer against the imp

Psychiatric complications of surgery in the male.

Four male patients with severe personality disturbance are described in whom surgery on their genital or perineal areas is followed or preceded by increased psychiatric symptomatology. In one case where adequate historical detail was available, the psychodynamics of the case, including the unconscious meaning of surgery to the patient, are formulated. Suggestions are made on how to identify and manage such patients.

Psychosocial problems of patients on the renal unit and their relation to treatment outcome.

Eighty patients on a renal unit were interviewed to assess their psychiatric status and social functioning, and 68 survivors were re-interviewed one year later. Psychiatric disorders, which were mainly mood disorders, were inversely related to duration of treatment, and had reduced considerably at follow-up. Social disability was more stable and was related to abnormal personal relationships and to treatment status. Psychiatric disorder was only weakly associated with social disability or physical outcome at follow-up. Despite psychiatric and social problems, 11 patients commented positively o

[Mental health in the aged].

The life expectancy of the world population has increased considerably and the absolute number of 65-year olds will approximately triple in the next 40 years. Since the prevalence of chronic illness and disability increases steeply with advancing age, it is to be expected that the utilization of health services and services provided for the elderly will also increase. In the developed countries about one quarter of those aged 65 and over suffer from mental disorders. An important risk factor for mental disorders of old age seems to be the subjective awareness of loneliness, combined with a qua

Functional incapacity and physical and psychological symptoms: how they interconnect in chronic fatigue syndrome.

BACKGROUND: It has been argued that perceived functional incapacity might be a primary characteristic of chronic fatigue syndrome (CFS) and could be explained by physical symptoms. If so, it could be expected to be closely associated with physical, but not psychological symptoms. The study tests this hypothesis. SAMPLING AND METHODS: The sample consisted of 73 patients, with a diagnosis of CFS according to the Oxford criteria, randomly selected from clinics in the Departments of Immunology and Psychiatry at St. Bartholomew's Hospital, London. The degree of fatigue experienced by patients was

Intentions to work during terminal illness: an exploratory study of antecedent conditions.

Facing a terminal illness is an unimaginably difficult experience, yet many individuals intend to work despite their prognosis. However, research has not systematically examined the potential antecedents underlying such intentions. Using behavioral intention theory as an underlying framework, this study hypothesized that reasons for working (intrinsic and extrinsic), the will to live, disability severity, accessibility of travel, and age would predict intentions to work during terminal illness. A representative sample of medically diagnosed amyotrophic lateral sclerosis (a.k.a. Lou Gehrig's di
