A personal narrative intervention combined with self-monitoring strategies: Outcomes for Mandarin-speaking adolescents with Down syndrome.

BACKGROUND: Personal narratives play an essential role in children's social and academic development. However, children with Down syndrome have ongoing challenges with constructing and communicating personal narratives. METHODS: Using a single-case multiple-probe across participants design, we examined whether a targeted intervention could improve both micro- and macro-structural aspects of personal narratives from Chinese adolescents with Down syndrome. RESULTS: All three participants demonstrated high treatment effects in two macrostructural narrative outcomes (i.e., narrative element comp

The impact of aversive personality traits on the psychotic-spectrum of disorders.

OBJECTIVES: Though relatively unexplored in clinical populations, aversive personality traits have been shown to impact the expression, the management, and the outcomes of psychotic disorders. This paper seeks to gather and organize existing evidence of the complex interplay linking social ethics, personality and experiences of psychosis through the lens of the so-called Dark Triad personality model, comprising the three multidimensional constructs Machiavellianism, narcissism and psychopathy. METHODS: A semi-systematic review of major literature databases was conducted; search terms aimed at

How are traits related to problem behavior in preschoolers? Similarities and contrasts between temperament and personality.

The lack of empirical research relating temperament models and personality hinders conceptual integration and holds back research linking childhood traits to problem behavior or maladjustment. This study evaluates, within a sample of 443 preschoolers, the relationships between children's maladaptation and traits measured by three temperament models (Thomas and Chess, Buss and Plomin, and Rothbart), and a Five-Factor based personality model. Adequate reliabilities and expected factor structures are demonstrated for most scales. A joint principal component analysis combining 28 temperament and 1

People With Haemophilia as Data Coordinators: An Analysis of the Ethics and Feasibility of Self-Management With Personal Health Records.

BACKGROUND: People with haemophilia perform various self-management tasks, supported by multiple health apps. Personal health records will enable individuals to access and add health information from different institutions in a single digital tool, providing an integrated overview of data. Later, individuals will also be able to share their data with health care providers and relatives. This creates a new role for users: Coordinator of data exchange. OBJECTIVE: To analyze if and how personal health records contribute to self-management, with a particular emphasis on the role of coordinating d

Exploring loneliness in individuals with diabetes - The role of childhood abuse and neglect, and personality functioning: Findings from a German population-based sample.

BACKGROUND: This study examines 1) loneliness frequency in individuals with diabetes, 2) associated factors, and 3) personality functioning and the epistemic stance as underlying mechanisms linking childhood abuse and neglect, and loneliness. METHODS: Using representative population data (N = 2428), loneliness (UCLA-LS) was assessed in individuals with self-reported diabetes (n = 206) and compared to those without. Regression analyses tested age, sex, relationship status, childhood abuse and neglect (ICAST-R), depression and anxiety (PHQ-4), personality functioning (OPD-SQS), and the epistemi

Policy Recommendations to Address Housing Shortages for People With Severe Mental Illness.

Affordable housing for persons with severe mental illness has never developed on the scale needed. Nearly one million individuals with mental illness are incarcerated, homeless, couch surfing, or cycling among institutions and unstable housing options across the United States. The three legs of the stool of housing development for persons with disabilities-capital to acquire or build, operations to support rent payments, and services to support tenants-exist to a small degree in a single federal housing program-the 811 program-which has fallen short in its impact on persons with mental illness

Frailty and exercise interventions : Evidence and barriers for exercise programs.

BACKGROUND: Due to the worldwide demographic transition healthcare systems are facing new demands and are increasingly confronted with an older population with specific medical needs related to multiple chronic disorders. The majority of older persons have an increased risk of frailty. In addition to pharmaceutical interventions another beneficial approach for counteracting frailty might be exercise or physical activity intervention. OBJECTIVE: The first goal was to narratively investigate the state of the art effective exercise interventions for frail older persons and briefly discuss the im

Risk estimates for complex disorders: comparing personal genome testing and family history.

PURPOSE: Personal genome testing allows the identification of single-nucleotide polymorphisms associated with an increased risk for common complex disorders. An area of concern in the use of personal genome testing is how risk estimates generated differ from traditional measures of risk (e.g., family history analysis). We sought to analyze the concordance of risk estimates generated by family history analysis and by personal genome testing. METHODS: Risk categorizations for 20 complex conditions included in Navigenics personal genome testing were compared with risk categorization estimates de

Efficacy of diaphragmatic breathing in persons with chronic obstructive pulmonary disease: a review of the literature.

The evidence base for diaphragmatic breathing (DB) as an adjunctive treatment modality for persons with COPD is questionable. This article reviews the literature regarding the efficacy of DB in persons with chronic obstructive pulmonary disease (COPD), and reports on the beneficial and detrimental effects of DB in persons with COPD. Diaphragmatic breathing has been described as breathing predominantly with the diaphragm while minimizing the action of accessory muscles that may assist with inspiration. No single or combined patient characteristic has been identified consistently to help predict

Pathological gambling: understanding relapses and dropouts.

There is little available information on the factors that influence relapses and dropouts during therapy for pathological gambling (PG). The aim of this study was to determine socio-demographic, clinical, personality, and psychopathological predictors of relapse and dropout in a sample of pathological gamblers seeking treatment. A total of 566 consecutive outpatients diagnosed with PG according to DSM-IV-TR criteria were included. All patients underwent an individualized cognitive-behavioral treatment program. We analyzed predictors of relapse during 6months of treatment and during the subsequ

Measuring Performance on the ABCDEF Bundle During Interprofessional Rounds via a Nurse-Based Assessment Tool.

BACKGROUND: Nurse-led rounding checklists are a common strategy for facilitating evidence-based practice in the intensive care unit (ICU). To streamline checklist workflow, some ICUs have the nurse or another individual listen to the conversation and customize the checklist for each patient. Such customizations assume that individuals can reliably assess whether checklist items have been addressed. OBJECTIVE: To evaluate whether 1 critical care nurse can reliably assess checklist items on rounds. METHODS: Two nurses performed in-person observation of multidisciplinary ICU rounds. Using a sta

Analysis of the BDNF Gene rs6265 Polymorphism in a Group of Women with Alcohol Use Disorder, Taking into Account Personality Traits.

It seems that BDNF has a direct influence on the brain pathways and is typically engaged during the processing of rewards. A surge in BDNF levels in the ventral tegmental area (the region from which the dopaminergic neurons of the mesocorticolimbic dopamine system originate and extend to the dorsolateral and ventromedial striatum) triggers a state of reward similar to that produced by opiates in animal studies. The aims of the study were (1) to analyze the association of the BDNF gene rs6265 polymorphism with AUD (alcohol use disorder) in women, (2) analyze personality and anxiety in alcohol-d

The effects of the CACNA1C rs1006737 A/G on affective startle modulation in healthy males.

BACKGROUND: The CACNA1C rs1006737 risk A allele has been associated with affective psychoses and functional studies indicate that it is associated with increased hippocampal/amygdala activity during emotional face-processing. Here we studied the impact of the risk A allele on affective startle modulation. METHODS: Hundred and ninety-four healthy males stratified for their CACNA1C rs1006737 genotype (GG:111, GA:67, AA:16) were presented with 18 pleasant, 18 unpleasant and 18 neutral pictures with acoustic probes (104 dB) occurring during 12 pictures in each affective category. OHVIRA syndrome: rare cause of chronic vaginal discharge in an unmarried female.

Chronic vaginal discharge in adolescent and young females, not responding to antibiotics, can pose a diagnostic dilemma for many gynecologists and general practitioners. Uterus didelphys with obstructed hemivagina and ipsilateral renal agenesis (OHVIRA syndrome) is a rare congenital anomaly. We present a case of a 22-year-old unmarried female with this syndrome presenting with chronic purulent vaginal discharge. The uniqueness about the case is its much delayed presentation.

Predictors of loneliness in U.S. adults over age sixty-five.

The purpose of this study was to examine sociodemographic and health-related risks for loneliness among older adults using Health and Retirement Study Data. Overall prevalence of loneliness was 19.3%. Marital status, self-report of health, number of chronic illnesses, gross motor impairment, fine motor impairment, and living alone were predictors of loneliness. Age, female gender, use of home care, and frequency of healthcare visits were not predictive. Loneliness is a prevalent problem for older adults in the United States with its own health-related risks. Future research of interventions ta

Widowhood and other demographic associations of pain in independent older people.

OBJECTIVES: To determine if psychosocial factors, as suggested by the demographic variables of widowhood and living alone, are associated with pain, particularly severe pain, in a representative sample of independent older people. DESIGN: One thousand older people (65+) randomly selected from independent residents living in a major city were surveyed about their health status (Health Status of Older People Study). Demographic characteristics, including age, gender, education, income, living alone, widowhood, and childlessness, were analyzed by logistic regression for their association with pa

A severe frontal-parietal lobe syndrome following cerebellar damage.

We report a case study of a frontal and parietal lobe syndrome with memory loss after unilateral left-sided cerebellar damage caused by a stroke in a patient with right cerebellar unusual developmental agenesis. The syndrome consisted of severe deficits in planning an organized sequence of events, in visuo-constructive abilities and inappropriate jocularity. These changes are ascribed in part to cerebellar-pontine lesions with resulting frontal lobe diaschisis as documented by single-photon emission computed tomography in the absence of morphological damage to the neocortex.

Prior living arrangements and nursing home resident admission ADL characteristics: a study of two states.

OBJECTIVES: This study examines the relationship between prior living arrangements and average activities of daily living (ADL) function upon nursing home admission across two states. METHODS: Minimum Data Set Plus records from 1993 and 1994 on 4,837 Medicaid reimbursed nursing home residents aged 65 years and older from two states were used. Medicaid reimbursed residents were chosen because Medicaid reimbursement policies differ at the state level, and such differences might affect admission characteristics across states. Ordinary least squares models were used to examine the correlates of t
