Cerebrospinal fluid formula in patients with central nervous system infection.

Knowledge of the CSF formula is clinically useful and at times critical in the analysis of a number of diagnostic problems presented by patients with CNS infections. The decision to withhold antimicrobial therapy in the patient with an acute meningitis syndrome, when viral etiology is suspected, is justified in the presence of a polymorphonuclear cellular reaction provided the CSF glucose is greater than 45 mg per dl and the protein less than 130 mg per dl. Patients who fail to improve clinically should be retapped in 8 to 12 hours. A persisting PMN reaction or the combination of a PMN pleocyt

Case Report: Improvement from no light perception after radiotherapy and surgical debulking for orbital Rosai-Dorfman disease.

Rosai-Dorfman disease (RDD) is a rare proliferative histiocytic disorder with ophthalmic manifestations occurring in 11% of cases. This report details the case of a 75-year-old woman presenting with orbital RDD characterized by right-sided proptosis and progressive vision loss, culminating in no light perception (NLP) for 8 months. Imaging studies revealed tumor involvement of the right ethmoid, maxillary, and sphenoid sinuses as well as the right orbit. Biopsy confirmed extranodal RDD. The patient underwent radiotherapy, which resulted in an improvement in the visual acuity to hand motion 3 m

Pembrolizumab-Induced Chronic Rhinosinusitis in a Patient With Non-Small Cell Lung Cancer.

A 63-year-old man was administered pembrolizumab for stage IV squamous cell lung cancer. After 54 cycles of pembrolizumab, the patient maintained complete remission of lung cancer but developed nasal obstruction, olfactory disturbance, conjunctivitis, stomatitis, glossitis, pharyngitis, and weight loss. Laboratory data showed increased levels of neutrophils, eosinophils, C-reactive protein, squamous cell carcinoma antigen, and cytokeratin 19 fragments. Sinus computed tomography revealed obstruction of the maxillary, ethmoid, and sphenoidal sinuses. Histological examination of nasal septal tiss

Skull Base Meningiomas.

Skull base meningiomas are among the most challenging meningiomas to treat clinically due to their deep location, involvement or encasement of adjacent essential neurovascular structures (such as key arteries, cranial nerves, veins, and venous sinuses), and their often-large size prior to diagnosis. Although multimodal treatment strategies continue to evolve with advances in stereotactic and fractionated radiotherapy, surgical resection remains the mainstay of treatment for these tumors. Resection of these tumors however is challenging from a technical standpoint, and requires expertise in sev

[A case of sinonasal inverted papilloma with malignant transformation and distant metastasis: a 10-year follow-up report].

Sinonasal inverted papilloma(SNIP) is a kind of benign tumor originating from the nasal cavity and paranasal sinuses, accounting for 70% of papillomas. The incidence of the disease is more common in males, with an average age of 50-60 years. It is most likely to occur in unilateral maxillary sinus and ethmoid sinus, followed by sphenoid sinus and frontal sinus. It has the characteristics of local invasion, high recurrence rate and malignant tendency, and most malignant transformation into squamous cell carcinoma. Endoscopic nasal resection and appropriate adjuvant therapy can help to reduce the

Dental and craniofacial characteristics caused by the p.Ser40Leu mutation in IFITM5.

Severe forms of osteogenesis imperfecta (OI) are usually caused by mutations in genes that code for collagen Type I and frequently are associated with craniofacial abnormalities. However, the dental and craniofacial characteristics of OI caused by the p.Ser40Leu mutation in the IFITM5 gene have not been reported. We investigated a 15-year-old girl with severe OI caused by this mutation. She had marked deformations of extremity long bones. There were no clinical or radiological signs of dentinogenesis imperfecta, but one tooth was missing and several teeth were impacted. Cone beam computed tomo

Ochroconis humicola coexisting with esthesioneuroblastoma: an incidental coloniser or allergen?

Ochroconis humicola, a fish pathogen, is rarely reported to cause disease in human. We report its first isolation from nasal tissue of a human immunodeficiency virus-positive young female patient. Histopathologically, the nasal mass was diagnosed as esthesioneuroblastoma. She presented with right-sided nasal obstruction and bleeding for two and half months. Computed tomography scan showed the nasal mass filling the whole right nasal cavity, maxillary, ethmoid and sphenoid sinuses. The direct microscopy of the nasal tissue and mucin demonstrated the presence of septate hyphae. On culture, O. hu

A case of intranasal schwannoma with bilateral nasal polyposis.

Schwannoma is a benign neoplasm originating from schwann cells of the peripheral nerve sheath, and its occurrence in the nasal cavity and paranasal sinuses is rare. We present a case of nasal schwannoma originating from the left nasal cavity, accompanied by bilateral nasal polyposis. A 66-year-old man presented with complaints of progressive left nasal obstruction, hyposmia, and headache. Anterior rhinoscopic and endoscopic examinations revealed a mass that almost completely filled the left nasal cavity. A polypoid mass was present in the right nasal cavity, as well. On computed tomography, th

Abscess of the clivus in an adolescent with complicated acute rhinosinusitis: a case report.

BACKGROUND: Complications of acute sinusitis affecting multiple sites are very uncommon, and generally develop for a delayed diagnosis of the primary infection, with possible severe and life-threatening evolution. Patients can have variable presentations according to the site and extent of the infection. Multiple forms generally include the coexistence of orbital manifestations and intracranial infections. We here present a case with unusual multiple sites locations (i.e.: intraorbital intraconic abscess, sigmoid sinus thrombosis, preclival abscess, multiple splanchnocranium osteomyelitic proc

Septic thrombosis of the dural venous sinuses.

From 1940 to 1984, 19 cases of septic dural-sinus thrombosis have been diagnosed at the Massachusetts General Hospital, and some 136 cases have been reported from other institutions. Septic thrombosis most frequently involves the cavernous sinuses (96 cases). Facial or sphenoid air sinus infection often precede cavernous-sinus disease. In addition to the classical signs of proptosis, chemosis, and oculomotor paralysis, isolated sixth-nerve palsy and hypo- or hyperesthesia of the fifth nerve may be found. The major pathogens associated with cavernous-sinus infection include Staphylococcus aureu

The inverted Schneiderian papilloma: a clinical and pathological study.

Inverted papillomas which arise from the lining membranes of the nose and paranasal sinuses are relatively unfamiliar lesions which have been reported in the literature under a variety of titles. The uncertainly surrounding their etiology, their relationship to nasal polyps and their malignant potential have resulted in an ill-defined clinical approach to their management. The designation Inverted Schneiderian Papilloma is suggested as an appropriate title that best conveys the qualities of inversion, location and distinctiveness of character. The characteristic microscopic feature is the incr

Coronal CT scan abnormalities in children with chronic sinusitis.

Coronal computed tomography (CT) scans are currently the optimal study to display the normal and abnormal anatomy in children with chronic and recurrent acute sinusitis after failure of medical therapy. To assess the extent and distribution of disease as well as associated anatomic abnormalities in this pediatric population, 74 coronal CT scans of children with continued symptoms of sinusitis after failure of extensive medical therapy were reviewed retrospectively. Twelve children with cystic fibrosis showed the characteristic features of medial displacement of the lateral nasal wall in the mi

Update on sinonasal imaging: anatomy and inflammatory disease.

Functional endoscopic sinus surgery, now the standard of care for surgical treatment of chronic and recurrent sinusitis, has changed current concepts of sinus health and disease. Interpretation of the CT scan must reflect this focus on anatomy and function. This article seeks to provide a clear understanding of sinonasal drainage. The normal frontal recess, ostiomeatal unit, and sphenoethmoid recess are considered along with the anatomic variations that distort their appearance and predispose the patient to developing sinus disease. The spectrum of uncomplicated and complicated sinonasal infla
