Spontaneous epidural hematoma of the spine in pregnancy.

Spontaneous spinal epidural hematomas are uncommon. Progressive neurologic deficits that are associated with epidural hematomas can develop rapidly and require prompt treatment. We present a case of spontaneous epidural hematoma of the thoracic spine that complicated a term pregnancy.

Ligamentum flavum hematoma in the lumbar spine.

A previously healthy 45-year-old woman presented with ligamentum flavum hematoma manifesting as radicular leg pain. Neurological examination findings were consistent with L-5 root compression. An old hematoma inside the degenerated ligamentum flavum was drained. The cyst showed no connection to the facet joint. Complete resection of the mass was performed, resulting in excellent pain relief. The most common cystic lesion in the lumbar spine is synovial cyst associated with the facet joints, but ligamentum flavum hematoma should be included in the differential diagnosis. The preoperative diagno…

Embryology of the spine and associated congenital abnormalities.

BACKGROUND CONTEXT: The spine is a complex and vital structure. Its function includes not only structural support of the body as a whole, but it also serves as a conduit for safe passage of the neural elements while allowing proper interaction with the brain. Anatomically, a variety of tissue types are represented in the spine. Embryologically, a detailed cascade of events must occur to result in the proper formation of both the musculoskeletal and neural elements of the spine. Alterations in these embryologic steps can result in one or more congenital abnormalities of the spine. Other body sy…

PURPOSE OF THE STUDY: Several reports have examined the pathophysiology of degenerative spondylolisthesis. Very little work has however been … T9 sagittal tilt and S1-S2 angle. These variables were measured on digitalized lateral views of the spine using a dedicated software (SpineView). Univariate analysis of the values obtained was performed to identify the variable distributions. Multivariate analysis was applied to study the relationships between these variables and to better define pertur …

Medical devices of the head, neck, and spine.

There are many medical devices used for head, neck, and spinal diseases and injuries, and new devices are constantly being introduced. Many of the newest devices are variations on a previous theme. Knowing the specific name of a device is not important. It is important to recognize the presence of a device and to have an understanding of its function as well as to be able to recognize the complications associated with its use. The article discusses the most common and important devices of the head, neck, and spine, including cerebrospinal fluid shunts and the Codman Hakim programmable valve; s…

Imaging in spine and spinal cord malformations.

Spinal and spinal cord malformations are collectively named spinal dysraphisms. They arise from defects occurring in the early embryological stages of gastrulation (weeks 2-3), primary neurulation (weeks 3-4), and secondary neurulation (weeks 5-6). Spinal dysraphisms are categorized into open spinal dysraphisms (OSDs), in which there is exposure of abnormal nervous tissues through a skin defect, and closed spinal dysraphisms (CSD), in which there is a continuous skin coverage to the underlying malformation. Open spinal dysraphisms basically include myelomeningocele and other rare abnormalities

Vertebroplasty and kyphoplasty in the fractured osteoporotic spine.

Percutaneous vertebroplasty and kyphoplasty have during the last decade gained an increasing popularity in the treatment of osteoporotic fractures of the spine. The techniques, clinical results and complications are described. There is however a disturbing lack of scientific evidence regarding the efficacy of the procedures, no controlled studies are published. An ongoing prospective randomised study is presented.

[MRT diagnosis for degenerative changes in the spine].

MRT is very well suited to the diagnosis of degenerative alterations in the spine. The option of imaging in multiple planes, the excellent soft-tissue contrast offering tissue differentiation, the absence of hardening artefacts and the avoidance of exposure to radiation have led to a shift in favour of MRT for diagnosis. In the present paper the MRT characteristics of the most important degenerative alterations that affect the spine are discussed.

Gluteal abscess: a manifestation of Pott's spine.

A 3-year-old girl presented with features suggestive of compression of cauda equina. In addition, she had soft, fluctuant gluteal swelling, which on aspiration yielded pus, positive for acid-fast bacilli on staining and culture. She showed significant clinicoradiological improvement following aspiration of pus and antitubercular treatment. Rare manifestations of cold abscess in the spine are discussed.

Magnetic resonance imaging of the pediatric spine.

Magnetic resonance is an excellent modality for imaging the pediatric spine. Its successful use requires understanding both the basic physics and the sedation protocols necessary for acquiring high-resolution images. Interpreting the images accurately depends on appreciating the differences between the normal anatomy of the pediatric and the adult spine. Evaluating the images requires familiarity with the differential diagnosis of pediatric spine disease, including the most common processes (infections, neoplasms, and trauma) as well as spinal dysraphism. Despite the acknowledged usefulness of…

Posterior decompressive procedures for the cervical spine.

Cervical degenerative disease encompasses a complex array of pathologies. Before considering surgical intervention, the surgeon should have a complete understanding of the indications for surgery and should have conducted a thorough physical examination and evaluation of radiologic studies. In general, unless myelopathy is present, surgery should be the treatment of last resort, used only after conservative measures have failed. The indications for posterior cervical decompression include both radiculopathy and myelopathy. However, these conditions alone are not sufficient to indicate that a p

Treatment of tuberculosis of the spine with neurologic complications.

Neurologic complications are the most dreaded complication of spinal tuberculosis. The patients who have paraplegia develop in the active stage of tuberculosis of the spine require active treatment for spinal tuberculosis and have a better prognosis than the patients who have paraplegia develop many years after the initial disease has healed. Neurologic dysfunctions in association with active tuberculosis of the spine can be prevented by early diagnosis and prompt treatment. Prompt treatment can reverse paralysis and minimize the potential disability resulting from Pott's paraplegia. When need…

Evaluation of spine injury in blunt trauma.

Although prehospital trauma guidelines call for spine immobilization for many trauma victims, there is a lack of clarity in medical institutions as to how trauma or emergency medicine physicians should proceed to remove cervical immobilization devices (CIDs) and "clear" the spine. Despite wide variations in physicians' approaches to such matters, however, certain specific aspects of vertebral assessment in such circumstances are well documented. The authors describe and explore several of these issues with respect to initial approach to the immobilized patient, clinical clearance of the spine,…
