The lateral ligament complex of the ankle is the most frequently injured structure in the body. Although most simple ankle sprains do not result in long-term disability, a significant number do not completely resolve, leading to residual symptoms that may persist for years. The most commonly reported symptoms, particularly among athletes, include instability, re-injury, and tendinitis. Ballet dancers are a combination of artist and high-performance athlete; consequently, they are subjected to the same types of injuries as other athletes, including lateral ankle sprains and their sequelae. Children commonly present with acute inversion injuries of the ankle. Radiographs occasionally show small fragments under the tip of the lateral malleolus. The purpose of our study was to clarify their origin. A prospective standardized clinical examination and a magnetic resonance imaging were carried out. Among the 116 included patients, seven of them showed an os subfibulare. They presented more pains on the lateral ankle area than the other children. We found also more injuries on the anterior talofibular ligament. The os subfibulare could be secondary to an accessory ossification but the

Physician awareness of ankle syndesmosis injuries is improving. The anatomy involved and the mechanism of injury are extremely relevant for the understanding and treatment of this type of injury. Examination under anesthesia may confirm the syndesmosis instability. Based on those findings, stabilization is the recommended approach.

Prevention of ankle sprain, the most common sporting injury, is only possible once risk factors have been identified. Voluntary strength, proprioception, postural sway, and range of motion are possible risk factors. A systematic review was carried out to investigate these possibilities. Eligible studies were those with longitudinal design investigating ankle sprain in subjects aged > or = 15 years. The studies had to have measured range of motion, voluntary strength, proprioception, or postural sway before monitoring incidence of lateral ankle sprain. Dorsiflexion range strongly predicted risk

This case report describes a novel occurrence of a Grade III ankle sprain complicated by cellulitis. The patient ultimately required aspiration of an extra-articular fluid collection and treatment with parenteral antibiotics due to worsening of the infection despite treatment with second-generation cephalosporins. This individual's infection was consistent with MRSA-cellulitis. Cellulitis, in the context of a preceding ankle sprain, has not been reported in the literature; the resultant edema from the injury may have served as the nidus for infection. MRSA comprises a significant proportion of

Biomechanical models generally assume that muscle fascicles shorten uniformly. However, dynamic magnetic resonance (MR) images of the biceps brachii have recently shown nonuniform shortening along some muscle fascicles during low-load elbow flexion (J. Appl. Physiol. 92 (2002) 2381). The purpose of this study was to uncover the features of the biceps brachii architecture and material properties that could lead to nonuniform shortening. We created a three-dimensional finite-element model of the biceps brachii and compared the tissue strains predicted by the model with experimentally measured ti

Foot and ankle pain is common in ballet dancers. Although clinical examination often points to the underlying cause, imaging is often necessary to confirm the diagnosis and thus ensure appropriate future management. Factors predisposing to the increased incidence of injuries in this population include the classical position in which ballet dancers stand, which is on the tips of the toes in the en pointe position or on the balls of the feet in the demi-pointe position. Furthermore, the repetitious nature of ballet and the long hours spent rehearsing cause over-use injuries. The causes of foot a

OBJECTIVE: To determine an ankle inversion-eversion movement range part where evertor/invertor muscles torque ratio is altered after recurrent ankle lateral ligament sprains.
DESIGN: The ankle evertor/invertor muscles torque ratios were determined in the different range parts of angular positions by the muscle isokinetic movements.
BACKGROUND: It is important to determine the movement range part where the evertor muscle weakness after the ankle lateral ligament sprains is more expressed and a repeated trauma of the lateral ligaments is more probable.
METHODS: Twenty-eight male handball play

Injuries of the toes and sesamoids of the first metatarsophalangeal joint comprise a wide spectrum of traumatic entities. Despite the fact that a majority of lesions may well respond to nonsurgical treatment and exhibit an excellent prognosis, appropriate clinical and imaging analysis is mandatory to select those injuries that require specific therapeutic and surgical measures to avoid long-term sequelae of functional disability.

BACKGROUND: Lateral ankle sprains are the most common sports related injury. Unfortunately a number of other lateral ankle injuries are often misdiagnosed as a sprain, resulting in misdirected therapy, delayed functional return and unresolved ankle pain.
OBJECTIVE: This review focuses on the differential diagnosis and management of unresolved lateral ankle pain, with an emphasis on methods to correctly identify the injury.
DISCUSSION: A surprisingly long list of conditions can mimic a lateral ankle sprain, including syndesmosis sprains, subtalar sprains with or without instability, impingeme

The lower extremity is the most commonly injured anatomic area in sports. An appropriate history and physical examination is sufficient to diagnose most injuries. By knowing the position where a joint has the greatest and least bony stability, one can predict the likelihood of bony and ligament injury. Such information is helpful in determining which structures are most susceptible to injury as well as identifying when radiographs or other diagnostic studies are indicated. An accurate diagnosis is crucial for planning effective therapy and in determining the need for surgical referral. An unde

The variation of practice with respect to the treatment of the acutely sprained ankle suggests a lack of evidence-based management strategies for this problem. The objective of this review was to assess the effectiveness of the various methods of immobilisation for acute ankle sprain. An electronic database search was conducted using MEDLINE, EMBASE, BIOSIS, CINAHL, Cochrane Controlled Trial Register and Current Contents. Randomised and quasi-randomised clinical trials describing skeletally mature individuals with an acute ankle sprain and comparing immobilisation for the treatment of injuries

Improvements of the shoe design to increase ankle stability have been discussed for a number of years. This accounts also for floorball where the incidence of ankle injuries is very high. The purpose of the present study was to describe the influence of the shoe upper on a typical floorball sideward cutting movement. A film analysis was conducted (200 Hz) where the achilles tendon angle beta was measured on the shoe as well as within the shoe. Subjects were twelve active floorball players who performed a defined sideward cutting movement with a low-cut and high-cut shoe as well as in the baref

BACKGROUND: Some sports, for example basketball and soccer, have a very high incidence of ankle injuries, mainly sprains. Consequently, ankle sprains are one of the most commonly treated injuries in acute care.
OBJECTIVES: To assess the effects of interventions used for the prevention of ankle ligament injuries or sprains in physically active individuals from adolescence to middle age.
SEARCH STRATEGY: We searched the Cochrane Musculoskeletal Injuries Group's specialised register, MEDLINE, PubMed, EMBASE, CINAHL, the National Research Register and bibliographies of study reports. We also con

The purpose of this study was to analyse objective modalities of ankle joint function after an acute ankle sprain and to see whether treatment with an air-cushioned ankle brace could enhance the restoration of function compared with a traditionally used compression bandage. The study included 73 consecutive patients between 15 and 55 years of age with an acute grade II or III ankle sprain, who sought medical care within 24 h of the time of injury. Patients with recurrent sprain were excluded. The patients were allocated at random to treatment with compression bandage or an air-cushioned ankle

The diagnosis and management of lateral ankle injuries require the physician to obtain an accurate history, complete a thorough physical examination, and institute appropriate treatment protocol. Labeling all acute lateral ankle injuries as ankle sprains can lead to long-term mechanical and functional instability and chronic pain around the ankle. Appropriate and aggressive functional rehabilitation of the acute ankle limits the postinjury convalescence and need for surgical reconstruction. If surgical repair of the chronic or acute ankle is warranted, the Brostrom-Gould procedure serves as a

Children at summer camp are at risk of injury and illness. The American Academy of Pediatrics and the American Camping Association have specific guidelines for the medical care of children in summer residential camps. This report describes the types of illnesses and injuries seen in 730 children attending camp for 2 weeks, use of the infirmary, and difference in the onset of infectious diseases compared with injuries during the camp session. During the month studied, there were 921 infirmary visits. The most common diagnoses were injuries and infectious disease. Girls and boys were equally lik

An alternative to traditional lateral ankle stabilization procedures is presented using freeze-dried fascia lata graft tissue. This procedure is minimally invasive and obviates the complications typically associated with tenodesis procedures. Furthermore, the fascia lata graft is more anatomically similar to ligament tissue, and studies suggest that the architecture is maintained through a neovascularization process.

This study presents the results of a three-year (1989-1991) retrospective survey of sports injuries to children and adolescents (5-17 years of age) treated at a sports injuries clinic (Crystal Palace, London). Of the total of 394 injuries, 178 (45.2%) were to females and 216 (54.8%) were to males. In both females and males the frequency of injuries showed a unimodal relationship with increasing age; the peak frequency in females occurred at age 13-14 years and in males at age 15-16 years. Fifty per cent of the injuries (197) were acute, 49.5% (195) were chronic and 0.5% (2) were inconclusive i
