Surgical management of paediatric stroke.

Childhood cerebrovascular disease is characterised by a wide range of relatively rare conditions. The management of a selection of some of the more frequently encountered, complex conditions is reviewed. The key to achieving the optimal therapeutic strategy for the individual child is multidisciplinary team management within a specialist neurovascular team. Access to rehabilitation is crucial.

[Secondary prevention after ischemic stroke].

This paper reviews secondary prevention of stroke by the therapy of vascular risk factors, anticoagulation, surgical and endovascular procedures. Two recently published studies, the PROtection aGainst REcurrent Stroke Study (PROGRESS) and the Heart Protection Study (HPS) demonstrated for the first time the efficacy of antihypertensive and lipid lowering by statins in stroke secondary prevention. PROGRESS has shown that the combination of perindoprile and indapamide reduced the occurrence of ischemic and hemorrhagic strokes in hyper- and normotensive patients by 40%, whereas HPS demonstrated a …

Stroke in systemic lupus erythematosus.

With improved control of the immunological phenomena of systemic lupus erythematosus, the epidemiology of this disease is changing. While both cardiac and neurological manifestations of systemic lupus erythematosus have been consistently described, there is increasing awareness of the need for general cardiovascular prevention in systemic lupus erythematosus. An excess of stroke is seen in systemic lupus erythematosus especially, in those cases with anticardiolipin antibodies. There is increasing evidence for the contributory role of other cardiovascular risk-factors, including hypertension, h…

[Emergency CAT in stroke-why?].

INTRODUCTION: A stroke is considered to be a neurological emergency, and it is recommended that an immediate CAT scan should be done in all patients. However, its usefulness may be controversial in the early stages. OBJECTIVE: To show the diagnostic usefulness of early CAT scans following strokes. PATIENTS AND METHODS: We made a retrospective study of a random sample of the requests for cranial CAT scans in suspected cerebrovascular disease between October 1998 and April 2000. We analyzed the diagnostic and therapeutic variations before and after CAT scans (pre CAT and post CAT moments). RE…

Complications following acute ischemic stroke.

The objective of this study was to assess typical early-onset complications following ischemic stroke in a large, hospital-based cohort to provide clinical data for future randomized trials and quality standards in clinical routine. 3,866 patients with acute ischemic stroke were prospectively documented in 14 Neurology Departments with an acute stroke unit. Within the first week after admission, increased intracranial pressure (7.6%) and recurrent cerebral ischemia (5.1%) were the most frequent neurological complications. Fever >38 degrees C (13.2%), severe arterial hypertension (7.5%) and pne…

Sleep-disordered breathing and stroke.

Sleep-disordered breathing (obstructive and central sleep apnea) is common in persons who have had a cerebrovascular accident (CVA). This article describes both sleep-disordered breathing and CVAs and reviews the related risk factors that link them together. In addition, the article discusses sleep-disordered breathing after CVA. The article concludes by presenting the clinical implications of this topic for nurses.

Ischemic stroke and active migraine.

OBJECTIVE: To determine the characteristics of acute ischemic stroke (IS) in patients with active migraine in a prospective stroke registry. METHODS: The authors studied the characteristics of patients admitted to a population-based primary-care center with IS and active migraine. The diagnosis of previous migraine was made based on replies to International Headache Society-based questions. Patients with migraine were divided into two age groups, one below 45 years of age and one 45 years or older, and compared by univariate and multivariate analysis with age-matched control subjects with IS,…

[Stroke. Options for primary prevention].

Age, gender, race, and genetic factors count among the nonmodifiable risk factors for stroke. But primary prevention is possible through modification of other vascular risk factors. The treatment of arterial hypertension, with optimal values around 135/85 mmHg, reduces the risk of stroke by 50%. A significant risk reduction for ischemic stroke is possible with at least 30 minutes of physical activity twice a week, cessation of cigarette smoking, and treatment of hypercholesterolemia with statins. Dietary measures should include a reduction of animal proteins, normalization of body weight and a…

Hydrodynamic model of heat stroke.

This work presents an hydrodynamical model of heat stroke, which is a physiopathological state of stress, due to an exposure of animals to an ambient temperature of approximatively 40 degrees C during two hours. The evolution of body temperature during this stress process is characterised by three phases. A first phase of increase is followed by a plateau which occurs before a second phase of increase which can be lethal. The model is based on the analogy of a boat progressively caught in a whirlpool. The evolution of the degree of freedom lost by the boat is mathematically analysed and this s…

Sinusitis and carotid artery stroke.

The relationship between sinusitis and ischemic stroke is unexplored. The anatomic proximity between the paranasal sinuses and the internal carotid artery suggests that inflammation of the sinuses could easily extend to the intracranial vasculature. We report 4 patients with acute ischemic stroke and extensive disease of the paranasal sinuses. All patients had large vessel stroke involving the internal carotid artery territory. All patients had extensive disease of the sphenoid and other sinuses. The sinus disease was demonstrated by magnetic resonance imaging. These case report observations s…

Coexisting causes of ischemic stroke.

BACKGROUND: Coexistence of multiple potential causes of cerebral infarct (MPCI) has been poorly studied. OBJECTIVE: To determine the risk factors, clinical findings, and topographical patterns of patients with at least 2 potential causes of cerebral infarct. DESIGN: Data analysis from a prospective acute stroke registry (the Lausanne Stroke Registry, Lausanne, Switzerland) in a community-based primary care center. RESULTS: Among 3525 patients with first-ever ischemic stroke consecutively admitted to a primary care stroke center, 250 patients (7%) had at least 2 MPCIs, with the following sub…

Stroke risk, cholesterol and statins.

The natural statins should be used as first line agents in the prevention of stroke. The effects of the synthetic statins on the prevention of coronary events and stroke have not been reported at this time. The National Stroke Association's Stroke Prevention Advisory Board has prepared a consensus statement on risk reducing intervention. The Board identified hypertension, MI, atrial fibrillation, hyperlipidemia and asymptomatic carotid artery stenosis (60% to 99% occlusion) as proven stroke risk factors. The Board's recommendations for the prevention of a first stroke are: 1. Hypertension shou…

Oxidants, antioxidants, alcohol and stroke.

Free radicals are involved in the formation of both atherosclerosis and thrombosis. Therefore, considerable interest has recently been aroused by their role in the development of ischemic cerebral injury. Experimental observations suggest that antioxidants could reduce cerebral arterial vasospasm, reduce infarct size and prevent the development of both atherosclerosis and thrombosis. However, clinical evidence for these beneficial effects is still lacking. Alcohol can act as an antioxidant and an oxidant, and its intake seems to exert both beneficial and untoward effects on stroke, depending o…

[Obstetric problems followed by stroke].

Obstetrical problems sometimes portend manifestations of atherosclerosis, as illustrated by two case reports. The first patient had the combination of hyperhomocysteinaemia due to chronic vitamin deficiencies in the diet, and smoking. The second was also a smoker and had a genetically determined mild hyperhomocysteinaemia, aggravated by chronic vitamin deficiencies resulting from poor dietary habits; she also had an increased folic acid requirement because of use of anti-epileptic drugs in combination with a familial predisposition for premature atherosclerotic manifestations. The first patien

[Stages in coping after stroke].

People who suffer a stroke most often experience a major crisis in their lives. This leads to psychological problems which are likely to influence the patient's life satisfaction, long-term rehabilitation outcome, and quality of life as much as the physical consequences caused by the stroke, The patient's ability to accept the new reality created by a disability as a part of their future life is therefore essential. This necessitates an individual coping strategy in the center of which is a mourning process, which unfolds in the same way as other experiences of vital threat, e.g. confrontation…

Alcohol and stroke: pathophysiologic mechanisms.

Epidemiological evidence indicates that recent heavy alcohol consumption increases the risk for all major types of stroke, whereas light-to-moderate alcohol intake is associated with a decreased risk of ischemic stroke. Although heavy drinking elevates blood pressure, there is no firm evidence to indicate that alcohol consumption causes the formation of aneurysms, microaneurysms or other lesions in human arteries. Alcohol has been reported to precipitate vasoconstriction and rupture of small cerebral arteries in experimental animals. Alcohol-induced neck trauma has been shown to precipitate tr…

[Acute therapy of ischemic stroke].

Ischemic stroke is a neurological emergency as transition form reversible to irreversible lesions of the brain may be prevented by starting the therapy within six hours after the onset of the neurological deficits. Intravenous thrombolysis may be administered within the first three hours and intraarterial thrombolysis within the first six hours after considering inclusion and exclusion criteria. Neuroprotective drugs are under investigation and may enlarge the spectrum of acute therapy in the near future. Ischemic strokes that do not qualify for thrombolysis may be treated with unfractionated …

Antiplatelet agents and stroke prevention.

Aspirin (acetylsalicylic acid [ASA]) from 75 to 1,300 mg/day is an effective agent in the secondary prevention of stroke. Although no properly designed study has been performed, circumstantial evidence suggests that the favorable effect of ASA for stroke may be enhanced by higher doses. Ticlopidine is more effective than 1,300 mg/day of ASA, and clopidogrel is at least as effective as 325 mg of ASA a day; but, for various reasons, ASA at higher doses is still first choice by many. Three studies have shown no benefit of adding dipyridamole to ASA. A fourth did, but, in addition to other problem…

Optic ataxia following unilateral stroke.

Five patients with optic ataxia together with tactile apraxia are described. They also demonstrated difficulty in finger movements under visual guidance. The CT scan showed lesions in the posterior parietal region in three and in two they were located anteriorly. It is well recognised that lesions in the posterior parietal area may cause disruption of visual guided movements. On the basis of the two cases it is postulated that the same phenomenon could occur with lesions located more anteriorly. The assumption is that optic ataxia may occur either following damage to the parieto-occipital regi
