Duodenal Strongyloidiasis Mimicking Inflammatory Bowel Disease: A Case Report of a 52-Year-Old Immunocompetent Woman.

BACKGROUND: Strongyloides stercoralis is an intestinal nematode with a unique autoinfective life cycle that enables decades-long persistence … w-up. CONCLUSION: This case emphasizes the clinical and histopathological overlap between duodenal strongyloidiasis and IBD, particularly when stool parasitology is unrevealing. The novelty of this report lies primarily in its didactic value for clinicians in endemic regions: Strongyloides should remain in the differential diagnosis of IBD-like presentations with …

Spotlighting zoonotic strongyloidiasis: a semi-systematic review of threadworms within baboons highlights opportunities for human infections.

BACKGROUND: Both humans and non-human primates are susceptible to Strongyloides fuelleborni, a grossly underappreciated parasitic zoonotic threadworm across the world, in addition to the more widely reported Strongyloides stercoralis. This semi-systematic review sought to assess the global evidence on zoonotic strongyloidiasis in baboons, Africa's most prolific non-human primate, to better understand the zoonotic threat these animals may pose to control strategies and public health goals. METHODS: Using appropriate keyword terminology, PubMed, Scopus and Web of Science databases were searched…

A Rare Case of Disseminated Strongyloidiasis Leading to Multisystem Organ Failure Following Deceased Donor Kidney Transplantation.

Strongyloidiasis is a rare systemic parasitic infection caused by the nematode, Strongyloides stercoralis, that is often insidious and may remain dormant for many years before progressing to fulminant hyperinfection in an immunocompromised host. In this report, we present a case of disseminated strongyloidiasis in a patient who underwent a deceased donor kidney transplantation 2 months prior to presentation. Our patient developed multisystem organ failure as well as secondary hemophagocytic lymphohistiocytosis (HLH) and ultimately died despite extensive resuscitative efforts and antiparasitic …

Regulatory T cell expansion resolves after effective strongyloidiasis treatment in subjects with HTLV-1 co-infection.

BACKGROUND: Regulatory T-cells (Tregs) are increased in patients with HTLV-1/Strongyloides stercoralis co-infection, and they may modify oth … oduction remained blunted in co-infected subjects. CONCLUSION: These results suggest that treating strongyloidiasis infection decreases circulating Tregs, but antigen-specific cytokine remains altered. This may reflect blunting of sensitization by Tregs.

Symptomatic chronic strongyloidiasis in children following treatment for solid organ malignancies: case reports and literature review.

Strongyloidiasis is an infection caused by the intestinal nematode Strongyloides stercoralis. Infected healthy individuals are usually asymptomatic, however it is potentially fatal in immunocompromised hosts due to its capacity to cause an overwhelming hyperinfection. Strongyloidiasis could be missed during routine screening because of low and intermittent larval output in stool and variable manifestations of the symptoms. We present two cases of strongyloidiasis occurring in children with solid organ malignancies suspected to have the infection based on their clinical conditions and treatment…

Th2/1 Hybrid Cells Occurring in Murine and Human Strongyloidiasis Share Effector Functions of Th1 Cells.

Infections by the soil-transmitted threadworm Strongyloides stercoralis affect 30-100 million people worldwide, predominantly in tropic and sub-tropic regions. Here we assessed the T helper cell phenotypes in threadworm-infected patients and experimental murine infections with focus on CD4+ T cells co-expressing markers of Th2 and Th1 differentiation. We show that mice infected with the close relative S. ratti generate strong Th2 responses characterized by the expansion of CD4+ GATA-3+ cells expressing IL-4/-5/-13 in blood, spleen, gut-draining lymph nodes, lung and gut tissue. In addition to

Prevalence of Chagas disease and strongyloidiasis among HIV-infected Latin American immigrants in Italy - The CHILI study.

INTRODUCTION: Screening HIV-positive migrants for neglected tropical diseases having potential for life-threatening reactivation, such as Chagas disease and strongyloidiasis is not widely implemented. We evaluated the prevalence of these infections among a large cohort of HIV-infected migrants from Latin America living in Italy. METHOD: Cross-sectional study evaluating the prevalence of Trypanosoma cruzi and Strongyloides stercoralis infections in HIV-infected migrants from Latin America enrolled in the Italian Cohort of Antiretroviral-Naïve patients (ICONA) between 1997 and 2018, based on se…

Transplant-related strongyloidiasis in solid organ transplant recipients in Saudi Arabia and the Gulf Cooperation Council countries.

BACKGROUND: Strongyloidiasis is a devastating disease with a mortality rate exceeding 50% in immunocompromised patients. The disease usually results from reactivation of a latent infection in a transplant patient. Alternatively, donor-derived transmission of Strongyloides may occur. METHODS: In this review, we report a case of Strongyloides hyperinfection syndrome in a liver transplant recipient to illustrate the severity of this infection. Following this, PubMed was searched for cases of transplant-related strongyloidiasis in the Gulf Cooperation Council (GCC) countries. Demographic data, th…

The severely under-recognized public health risk of strongyloidiasis in North American cities-A One Health approach.

Strongyloides and other soil-transmitted helminths represent a severely under-recognized zoonotic public health risk, especially in North American cities. They are present throughout North America, including in urban areas, causing morbidity and mortality in human and non-human animals. Epidemiological "masking" of strongyloidiasis due to overlapping symptoms with other systemic diseases, including allergies, and diagnostic limitations complicate our understanding of the epidemiological extent of this disease, and auto-infection allows long-term persistence of individual infections. Zoonotic t…

Gelatin particle indirect agglutination and enzyme-linked immunosorbent assay for diagnosis of strongyloidiasis using Strongyloides venezuelensis antigen.

Routine microscopical examination of stool specimens for diagnosis of strongyloidiasis is insensitive and serological methods using Strongyloides stercoralis antigen are at present not available for field studies. We evaluated 2 techniques, enzyme-linked immunosorbent assay (ELISA) and gelatin particle indirect agglutination (GPIA), using an antigen obtained from the rodent parasite, S. venezuelensis. Fifty-four Peruvian patients with different clinical forms of strongyloidiasis were studied: 12 asymptomatic, 31 symptomatic, and 11 hyperinfection cases. Our results demonstrate that both ELISA …

Influence of human T-cell lymphocytotropic virus type 1 infection on serologic and skin tests for strongyloidiasis.

The aim of this study was to determine whether human T-cell lymphocytotropic virus type 1 (HTLV-1) infection may affect the levels of parasite-specific immunoglobulin (Ig) G and IgE and the positivity of the skin test for strongyloidiasis. Participants included 67 patients with strongyloidiasis (40 without HTLV-1 infection and 27 coinfected with HTLV-1). We determined IgG and IgE levels by enzyme-linked immunosorbent assay, and the immediate hypersensitivity skin test was performed with the metabolic Strongyloides stercoralis antigen. Specific IgE levels and the size of skin reactions in patie…

First evidence of significant autochthonous transmission of strongyloidiasis in northern Spain: A retrospective study from Asturias, Spain.

BACKGROUND: Autochthonous strongyloidiasis in Spain has traditionally been considered a recognized but underdiagnosed condition, primarily affecting individuals in agricultural environments along the Mediterranean coast. We describe the first series of autochthonous strongyloidiasis cases diagnosed in Asturias, a region in northern Spain with no previously documented endemic transmission. METHODS: This is a retrospective review of all diagnosed cases of autochthonous strongyloidiasis at the Central University Hospital of Asturias, from 2016 to 2024. Inclusion required no travel history to end…
