Chronic GM1 gangliosidosis presenting as dystonia: clinical and biochemical studies in a new case.

Clinical and biochemical findings in a patient affected by chronic GM1 gangliosidosis, presenting as progressive dystonia and mental deterioration, are reported. The patient, a 13-year-old male, showed, at the age of 3 years, an impairment of gait with frequent falls, dysarthria and stuttering. At the age of 6, writing dystonia appeared and subsequently mental deterioration and dystonic postures of arms and legs became evident. The clinical features presented by this patient are similar to those shown by the cases of adult/chronic GM1 gangliosidosis previously reported, except for the early on

Changing Polish university students' attitudes toward cluttering.

PURPOSE: This quasi-experimental design study in Poland explored the extent to which attitudes toward cluttering of university students could be changed or improved after a series of activities dedicated to attaining deeper recognition of problems associated with fluency disorders. METHOD: University students were assigned to either an Experimental or a Control group, with 39 in each (total = 78). They all completed the Polish version of the Public Opinion Survey of Human Attributes-Cluttering (POSHA-Cl) on two occasions up to eight weeks apart. Participants in the Experimental group attended

Multiple voices in Charles Van Riper's desensitization therapy.

BACKGROUND: Charles Van Riper (1905-94) author, researcher and practitioner, was one of the major contributors to the field of stuttering in the 20th century. His series of Action Therapy videotapes provide a useful model of how therapy was implemented by a master clinician. AIMS: Van Riper's session with a client is analysed to reveal how voices are used by these two participants during desensitization therapy, and to demonstrate how Van Riper's expertise manifests itself during the session. METHODS & PROCEDURES: Interaction between Van Riper and a client during the desensitization stage of

Priapism in sickle cell disease.

Priapism, an unwanted painful erection of the penis, is a little discussed but common complication of sickle cell disease. What is known about the prevalence of priapism, efficacy of management approaches, and outcome is drawn primarily from retrospective and single-center reports. Priapism occurs in two patterns: prolonged and stuttering (ie, recurrent brief episodes that resolve spontaneously). If priapism persists for 4 hours or more without detumescence, the patient is at risk for irreversible ischemic penile injury, which may terminate in fibrosis and impotence. Large multicenter studies

Treatment of Refractory Idiopathic Ischemic Priapism: A Case Report and Literature Review.

Priapism is a medical condition characterized by a prolonged period of penile rigidity in the absence of external sexual stimulation. Three broad categories exist for this condition: ischemic (low venous flow), nonischemic (high arterial flow), and recurrent (stuttering). Ischemic priapism is a urological emergency necessitating immediate medical attention. This literature aims to highlight the importance of prompt workup and treatment of ischemic priapism in order to prevent irreversible damage to the penis, such as erectile dysfunction and impotence. This case report presents a 35-year-old p

Further study of spastic dysphonia.

Seven women and two men with spastic dysphonia, ranging in age from 20 to 59 years, were studied to determine the cause and the means of development of the condition. The patients were examined by a laryngologist and, when feasible, by a psychiatrist or other medical specialist for coordination and corroboration of the diagnosis. Psychiatric observations were significant. A background of conflict with a parent or spouse was frequent and typical. In almost every case there were deep guilt feelings associated with something the patient had said to the person with whom he was in conflict. Similar

Functional Speech and Voice Disorders: Case Series and Literature Review.

BACKGROUND: Functional disorders of speech and voice, subtypes of functional movement disorders, represent abnormalities in speech and voice that are thought to have an underlying psychological cause. These disorders exhibit several positive and negative features that distinguish them from organic disorders. METHODS AND RESULTS: We describe the clinical manifestations of functional disorders of speech and voice, and illustrate these features using six clinical cases. CONCLUSIONS: Functional disorders of speech and voice may manifest in a variety of ways, including dysphonia, stuttering, or p

Cerebral hyperperfusion syndrome after intracranial stenting of the middle cerebral artery.

Cerebral hyperperfusion syndrome (CHS) is a rare complication following cerebral revascularization. It presents with ipsilateral headache, seizures, and intracerebral hemorrhage. It has mostly been described following extracranial carotid endarterectomy and stenting and it is very unusual after intracranial stenting. A 71-year-old man with a stuttering stroke was taken up for a cerebral angiogram (digital subtraction angiography), which showed a dissection of the distal left middle cerebral artery. This was recanalized with a solitaire AB stent. After 12 h, the patient developed a right hemipl

Atypical Hallervorden-Spatz disease with preserved cognition and obtrusive obsessions and compulsions.

We describe the case of an adult female with Hallervorden-Spatz disease (HSD), "eye-of-the-tiger" sign on cranial magnetic resonance imaging scan, and two mutations in the pantothenate kinase 2 (PANK2) gene. Symptomatic presentation included stuttering dysarthria, dystonic posturing, increased limb and axial muscle tone, choreoathetosis, stereotyped motor behaviors, and obsessive-compulsive symptomatology since adolescence. Extensive neuropsychological testing at 40 and 44 years of age revealed a relatively normal IQ and stable cognitive pattern overall. This case demonstrates that HSD patient

[Elucidation on a dystonic emperor].

Tiberius Claudius Caesar Augustus (10 BC.-54 DC.) governed the Roman empire for 14 years, being popularly known as Claudium, the <<stuttering>> and <<lame>> emperor. Most of the works published on his health coincide that the cause of his sufferings was an infant Athetoid cerebral palsy. However, the reading of the classics (Suetonius, Dion Cassius, Seneca and Tacitus) manifests the existence of some symptoms such as hypoacusis, recurrent abdominal pain, sleep disorders and probable myopathy that could not only be explained by this clinical picture. The analysis of all the vegetative symptoms,

There is more to cluttering than meets the eye: The prevalence of cluttering and association with psychological well-being indices in an undergraduate sample.

BACKGROUND: Cluttering is a fluency disorder characterized by an abnormally fast or irregular speech delivery rate along with disfluencies that are frequent but are not judged to be stuttering. Data on cluttering prevalence in the general population are scarce, as well as its association with psychological well-being indices, such as anxiety, and depressive symptoms. AIMS: To estimate cluttering prevalence among undergraduates, as well as its relationship with psychological and well-being indicators. METHODS & PROCEDURES: To address these issues, a large sample (n = 1582) of undergraduates c

[Attention deficit hyperactivity disorder: pharmacotherapy in special situations].

Attention deficit hyperactivity disorder (ADHD) is a common neurodevelopmental disorder. In addition to the core symptoms, patients with ADHD often have manifestations of neurocognitive dysfunction (executive functioning deficiency, speech disorders, sluggish cognitive tempo), and about two thirds of children with established ADHD have another comorbid disorder. When prescribing pharmacotherapy, drugs with a positive effect on both the symptoms of ADHD and comorbid disorders are preferred. These include atomoxetine hydrochloride. The possibilities of using atomoxetine in the comorbidity of ADH

Immune disorders and handedness in dyslexic boys and their relatives.

Thirty dyslexic and 30 control boys aged 7-11 years were compared for frequency of immune disorders and handedness as well as for family history of immune disorders and learning disabilities (dyslexia and stuttering). They were also compared for neurological status and for history of speech and language difficulties. There were no significant differences between the two groups in the frequency of immune disorders and in handedness. The results showed significantly more dyslexic boys with soft neurological signs and signs of speech and language disorders. The frequency of dyslexia was significa
