[Vasovagal syncope in pediatric patients: a medium-term follow-up analysis].

INTRODUCTION AND OBJECTIVES: Little information is available on the evolution of pediatric patients with vasovagal syncope. We therefore aimed to assess the medium-term clinical outcome of children evaluated by tilt testing for syncope of unknown origin. PATIENTS AND METHOD: Fifty-one children under 17 years of age who had undergone tilt testing were identified from a data base and studied prospectively. Kaplan-Meier and Cox regression analyses were performed to estimate syncope-free survival, its predictors, and the relative risks of several patient subgroups. RESULTS: Forty-seven (92%) of …

Chiari type I presenting as left glossopharyngeal neuralgia with cardiac syncope.

Glossopharyngeal neuralgia is an uncommon craniofacial pain syndrome that is occasionally associated with cardiac syncope. However, we relate Chiari I syndrome as a cause of this clinical picture for the first time in the literature. The authors analyze the relevant literature and discuss the pathogenesis and treatment of associated syndromes. We describe the case of a 45-year-old female patient who presented with a 3-year history of left glossopharyngeal neuralgia with occasional dysphagia and episodes of syncope when eating or swallowing. The pain was not disseminated to the right side and w…

Autonomic seizures versus syncope in 18q- deletion syndrome: a case report.

PURPOSE: The 18q- deletion syndrome (18qDS) is frequently associated with cardiac anomalies. Patients with this syndrome may also have epile … seizure detection software can be helpful in diagnostically differentiating autonomic seizures from syncope. Dysmyelination due to loss of the myelin basic protein gene on 18q and cortical dysgenesis may be of pathogenic relevance.

Methods other than tilt testing for diagnosing neurocardiogenic (neurally mediated) syncope.

The recording of spontaneous episodes of bradycardiac neurocardiogenic syncope (NCS) has shown that: a prolonged ventricular asystole seems necessary to cause syncope; asystole is preceded by other bradyarrhythmias in the vast majority of cases; some warning symptoms precede the loss of consciousness in most cases; conventional dual-chamber pacing is efficacious both in patients with a positive response to carotid sinus massage (CSM) and eyeball compression test (EBC) and in those with a positive response to tilt-testing (TT). CSM, EBC, and TT are established tools for diagnosing NCS, when the…

Differences between syncope resulting from rapid onset acceleration and orthostatic stress.

BACKGROUND AND HYPOTHESIS: Orthostatically-induced syncope is accompanied by venous pooling and vasodilatation. Loss of consciousness during head-to-foot acceleration (G-LOC) in aviators may be caused by a different mechanism, as venous pooling should be prevented through the use of an anti-G suit. This research was conducted to test the hypothesis that in individuals wearing a well-fitted anti-G garment, no important changes occur in the volume of dependent regions during loss of consciousness resulting from rapid onset acceleration stress. Further, this work compares venous pooling patterns …

Overdrive pacing in quinidine syncope and other long QT-interval syndromes.

Prolongation of QT interval is associated with repetitive paroxysm of a particular ventricular tachycardia. It is a typical complication of quinidine therapy but may occur in various other conditions. We used endocardial pacing in nine patients with prolongation of the QT interval who suffered from bouts of ventricular tachycardia and fibrillation. In six patients, the syndrome was due to quinidine and in three, to prenylamine. Acceleration of heart rate resulted in immediate suppression of all arrhythmias. Pacing was continued until the condition producing the QT prolongation disappeared. In

An Adrenal Adenoma as the Culprit Behind 30 Years of Recurrent Syncope.

BACKGROUND: Primary aldosteronism (PA) is a treatable endocrine cause of secondary hypertension and, rarely, ventricular tachycardia (VT), syncope, or sudden cardiac death. CASE SUMMARY: A 59-year-old woman with hypertension presented with recurrent syncope for 30 years, worsening over 2 weeks. Electrocardiogram showed multifocal premature ventricular complexes (PVCs), with one dominant morphology triggering predominantly monomorphic VT, plus corrected QT interval prolongation. Hypokalemia, renal potassium wasting, left ventricular enlargement, and mild systolic dysfunction were also present.…

Syncope and 2:1 Atrioventricular Block in a Middle-Aged Man.

CASE SUMMARY: A man in his early 50s with no cardiovascular risk factors presented after a brief syncopal episode and was found to have bradycardia with 2:1 atrioventricular block on electrocardiogram. Careful inspection of the inferior leads showed hyperacute T waves with subtle ST-segment elevation and reciprocal ST-segment depression, suggesting evolving inferior occlusion myocardial infarction. He underwent urgent coronary angiography, which revealed proximal right coronary artery occlusion, followed by successful percutaneous revascularization. This case highlights that syncope with new a…

Deprescribing of blood pressure-lowering medications in patients with hypotensive syncope.

AIM: To assess the efficacy of deprescribing blood pressure-lowering medications in hypertensive patients suffering from severe hypotensive syncope. METHOD: Multicentre, prospective, pragmatic, non-randomized uncontrolled, study. Deprescribing (withdrawal or dose reduction of antihypertensive and psychoactive medications with blood pressure lowering effects) was guided by clinical experience and supported by two 24-hour ambulatory blood pressure monitoring (ABPM) assessments-one before (ABPM 1) and one after (ABPM 2) drug optimization. Primary endpoint was the reduction of the burden of synco…

[Cardioneuroablation for vasovagal syncope: rationale, evidence, and limitations of an emerging therapy].

Cardioneuroablation (CNA) is an emerging catheter-based neuromodulation strategy aimed at reducing vagal influence on the sinoatrial and atrioventricular nodes through ablation of atrial ganglionated plexi. Its rationale is particularly strong in forms with a predominant cardioinhibitory component, especially asystolic forms, whereas it appears weak in purely vasodepressor forms. Available evidence suggests a reduction in syncope recurrence and disease burden in selected patients, but derives largely from observational, often single-center studies, with heterogeneous selection criteria, proced…

Diazepam-Associated Hypotension Contributing to Recurrent Syncope in an Older Adult.

Syncope in older adults is often multifactorial, with polypharmacy being a common and potentially modifiable factor. Benzodiazepines are prescribed for anxiety and insomnia in this population despite being associated with adverse effects, including falls, cognitive impairment, and hypotension. We present the case of a 68-year-old female patient with multimorbidity and recurrent unexplained syncope despite extensive cardiologic and neurologic evaluations. She was on diazepam therapy for anxiety as part of a complex medication regimen. A temporal relationship was observed between diazepam admini…

Spontaneous Atraumatic Rupture of a Liver Hemangioma as a Rare Cause of Syncope.

BACKGROUND: Syncope is common in emergency medicine, but only a minority of syncopes is caused by hemorrhage. Liver hemangioma is the most frequent benign liver tumor, and they rarely lead to symptoms or complications. Case Presentation. We describe the case of an 81-year-old man with hemorrhagic shock due to an atraumatic rupture of a hepatic hemangioma while on oral anticoagulation. The patient presented to the emergency department after three episodes of syncope before admission, nausea, vomiting, mild epigastric abdominal pain, but with clinical signs of peritonitis. On admission, the pati…

Use of a leadless pacemaker in the management of swallow syncope: A case report.

A 41-year-old male presented with syncope whilst eating and was subsequently demonstrated to have recurrent symptomatic sinus pauses whilst swallowing. Following the exclusion of structural heart disease, he was diagnosed with swallow syncope, an uncommon variant of neurocardiogenic syncope. To avoid long-term complications of a transvenous pacemaker, the case was managed with a leadless pacemaker which resulted in complete resolution of symptoms.

Unconventional diagnosis of bradyarrhythmic syncope in Brugada syndrome: a case report.

BACKGROUND: The Brugada syndrome (BrS) is an inherited disorder associated with the risk of ventricular fibrillation and sudden cardiac death (SCD). The current main therapy is an implantable cardioverter-defibrillator (ICD). However, the risk stratification and management of patients remain challenging. Here, we present a case of BrS representative of the pitfalls that clinicians may encounter in the management of Brugada patients in routine clinical practice. CASE SUMMARY: A 39-year-old man with BrS and recurring syncope was implanted with a subcutaneous ICD (S-ICD) (EMBLEM MRI S-ICD, Bosto…
