Hand and Wrist Injuries: Common Problems and Solutions.

Injuries to the hands and wrist are common in athletes. Injuries include acute fractures, dislocations, ligamentous, and tendon injuries, as well as more chronic injuries such as sprains and strains. Complications in the treatment of sports injuries of the hand and wrist may be divided into 2 categories: incorrect or delayed diagnosis and iatrogenic injury related to treatment. This article highlights common sports injuries of the hand and wrist and their complications, and includes tips for successful management.

ACFAS Clinical Consensus Statement: Acute Achilles Tendon Pathology.

Injuries to the Achilles tendon are a challenge to the foot and ankle surgeon. In recent years, research has led to a relative change in the way that many surgeons view acute Achilles tendon rupture. In an effort to fully evaluate these trends, as well as to evaluate all aspects of care for acute Achilles tendon rupture, the American College of Foot and Ankle Surgeons convened a panel of experts to create a clinical consensus statement to address selected aspects of care of the acute Achilles tendon injury.

Scientific bases and clinical utilisation of the calf-raise test.

BACKGROUND: Athletes commonly sustain injuries to the triceps surae muscle-tendon unit. The calf-raise test (CRT) is frequently employed in sports medicine for the detection and monitoring of such injuries. However, despite being widely-used, a recent systematic review found no universal consensus relating to the test's purpose, parameters, and standard protocols. OBJECTIVES: The purpose of this paper is to provide a clinical perspective on the anatomo-physiological bases underpinning the CRT and to discuss the utilisation of the test in relation to the structure and function of the triceps s

Lesions of the subscapular tendon regarding two cases in arthroscopic surgery.

Subscapularis tendon lesions in the shoulder are usually difficult to diagnose through clinical examination alone. Isolated subscapularis tendon lesions are infrequent, and most of these injuries are associated with biceps tendon pathology, rotator cuff tears, fractures to the tuberosities, or as a part of an instability complex of the shoulder. All of these other diagnoses over-shadow and complicate the diagnosis of the subscapularis tendon. Although magnetic resonance imaging is frequently effective in evaluating subscapularis tendon tears, arthroscopy is certainly more efficient and affords

Five Operations That Give the Best Results after Brachial Plexus Injury.

Treatment of brachial plexus injuries has improved slowly over the past 45 years. Changes in strategy, techniques, microsurgical equipment, and technology have expanded the surgical options for reconstructing these life-altering, highly complex injuries. The surgical techniques available include neurolysis, nerve repair, nerve grafting, nerve transfers, tendon transfer, muscle transfer, and other soft- and bony-tissue procedures. In this article, the authors have selected five surgical procedures (i.e., Oberlin procedure, Leechavengvongs procedure, free functional muscle transfer, radial nerve

Neglected ruptures of the Achilles tendon.

Neglected Achilles tendon ruptures, although uncommon, are debilitating injuries. The optimal management is surgical. Many different techniques can be used to repair or reconstruct the rupture, including tendo-Achilles advancement or flap reconstruction; local tendon transfer; and autologous, autograft, or synthetic implantation. Comparison of different techniques is difficult.

Combined Anterior Cruciate Ligament, Medial Collateral Ligament, and Posterior Oblique Ligament Reconstruction Through Single Tibial Tunnel Using Hamstring Tendon Autografts.

Combined anterior cruciate ligament (ACL) and medial collateral ligament (MCL) injuries are the most common type of combined ligamentous injury of the knee. The optimal treatment for these combined injuries is controversial. Combined ACL and MCL-posterior oblique ligament (POL) reconstruction avoids late anteromedial rotatory instability and chronic valgus instability of the knee and decreases the increased stress on the ACL graft. Graft choice (hamstring tendon autograft, quadriceps bone-patellar tendon-bone autograft, or Achilles tendon allograft) and anatomic restoration of the medial and p

Management of a 38-Year-Old Man With Complex Elbow Fracture-Dislocations and Triceps Tendon Avulsion: A Case Report.

Terrible triad injuries of the elbow, which combine posterior dislocation, radial head fracture, and coronoid process fracture, present significant management challenges due to the high potential for instability and stiffness. These injuries require prompt diagnosis and a carefully planned surgical approach to restore joint stability and function. This report describes a complex case involving a 38-year-old male who sustained a terrible triad injury with an uncommon complication of triceps tendon avulsion after a motor vehicle accident. The injury consisted of a comminuted radial head fracture

The 4-Strand Hamstring Overlap Link for All-Inside Anterior Cruciate Ligament Reconstruction: Graft Preparation Technique.

Anterior cruciate ligament (ACL) injury is among the most prevalent sports-related knee injuries. Arthroscopic ACL reconstruction (ACLR) is the standard minimally invasive surgical treatment for ACL injuries. The all-inside technique, as a significant recent innovation in ACLR, has become an important clinical option due to its advantages of reduced invasiveness, stronger graft constructs, and less early postoperative pain. However, a consensus on the optimal tendon preparation technique for all-inside ACLR has yet to be established. This article details an overlapping suture graft preparation

Knee Posterolateral Corner Reconstruction with a Single Tendon.

Knee posterolateral corner (PLC) injuries are troublesome conditions and are always involved in complicated knee-ligament injuries. Various surgical techniques have been reported to address these conditions, in either an open or an arthroscopic manner. However, a simple and effective method is still being pursued. We introduce a mini-invasive PLC reconstruction technique in which a single tendon is used to reconstruct the lateral collateral ligament, the popliteofibular ligament and the popliteal tendon simultaneously. The critical points of this technique are proper location and creation of t

Acute hand injuries in athletes.

Hand and wrist injuries in athletes are common, representing between 3 and 25% of all sports injuries. As many as a quarter of all sports injuries involve the hand or wrist. We review the recent literature regarding acute hand injuries in athletes based on the structures involved - bone, muscle/tendon, ligament, and neurovascular - including diagnosis and pathophysiology of these injuries, focusing on athlete-specific facets of treatment, and when available, opinions on return to play.

Peroneal tendon pathology: Pre- and post-operative high resolution US and MR imaging.

Peroneal tendon pathology is an important cause of lateral ankle pain and instability. Typical peroneal tendon disorders include tendinitis, tenosynovitis, partial and full thickness tendon tears, peroneal retinacular injuries, and tendon subluxations and dislocations. Surgery is usually indicated when conservative treatment fails. Familiarity with the peroneal tendon surgeries and expected postoperative imaging findings is essential for accurate assessment and to avoid diagnostic pitfalls. Cross-sectional imaging, especially ultrasound and MRI provide accurate pre-operative and post-operative

Validity of injury self-reports by novice runners: comparison with reports by sports medicine physicians.

This study examined the criterion validity of self-reported running-related injuries (RRI) by novice runners. Fifty-eight participants (41 females; age 46 ± 11 yrs) of the "Start-to-Run" program provided self-reports on their RRIs using an online questionnaire. Subsequently, they attended injury consultations with sports medicine physicians who provided physician-reports (blinded for the self-reports) as a reference standard. Self-reports and physician-reports included information on injury location (i.e., hip/groin, upper leg, knee, lower leg, and ankle/foot) and injury type (i.e., muscle-ten
