OBJECTIVE: To determine the long-term results and possible complications of the posterior tibialis transfer in correction of the foot-drop in leprosy patients, and to compare the results by the circum-tibial and interosseous routes.
METHODS: From January to October 2001, 37 cases (treated from October 1989 to October 1999) were followed up. Walking gait, active dorsiflexion and plantar flexion of the ankle joint, deformities of the feet, and patients' satisfaction were recorded.
RESULTS: Of 37 patients, 22 were treated by circum-tibial transfer, 15 by interosseous transfer. All patients' Ach

Pectoralis major transfer (partial or complete) is the most common transfer to the scapula for winging as a result of serratus anterior paralysis or injury. The most commonly described technique is to transfer the pectoralis major prolonged with tendon allograft or autograft to the scapula. We present a technique that entails direct transfer of the sternal head of the pectoralis major with its bony insertion to the lower scapula.

When faced with revision surgery for recurrent congenital vertical talus deformity, the tibialis anterior tendon may be deficient or absent as a result of the initial procedure. This tendon cannot then be used as a dynamic sling to dorsiflex the talar neck. We present two cases in which the peroneus longus tendon was transferred dorsally into the talar neck as an alternative to an absent tibialis anterior tendon. Excellent corrections were achieved and maintained. The use of this tendon has not been described previously in surgery for this condition.

Surgical restoration of hand grasp in the patient with spinal cord injury at the sixth cervical level often involves use of one of the two radial wrist extensors. Because the loss of the remaining wrist function would be devastating, it is important to establish techniques for quantitatively predicting postoperative function before tendon transection. An in situ method has been developed for determining muscle strength during tendon transfer surgery. Buckle transducers are placed on the tendons of the extensor carpi radialis brevis and longus for simultaneous measurement of strength of each in…

Although spastic conditions often involve the shoulder, it is rare for surgical intervention to be required. In cases in which chemodenervation and therapy are insufficient to optimize the patient's function or minimize their care requirements, surgical options, such as tendon and joint releases, can be considered. Tendon transfers are rarely indicated. Nerve transfers, particularly contralateral C7, may play a larger role in the future as we gain further understanding into the risks, indications, and contraindications of this exciting technique.

Percutaneous Achilles tendon lengthening is an effective surgical procedure to treat and prevent forefoot and midfoot ulcerations in patients with diabetes. Patients with diabetes are prone to plantar ulcerations due to a combination of factors, such as peripheral neuropathy, decreased tendon elasticity, peripheral vascular disease, and hyperglycemia. Complications such as re-ulceration and transfer lesion to the heel, associated with a calcaneal gait secondary to over-lengthening, are possible with percutaneous Achilles tendon lengthening. Although percutaneous Achilles tendon lengthening is

BACKGROUND: The transition to volar locking plates for the treatment of distal radius fractures has improved patients' range of motion, reduced extensor tendon issues, and hastened return to activity compared to older fixation methods or nonoperative treatment. One drawback to volar plating is the potential for flexor tendon ruptures due to aberrant plate or screw placement. This study aims to describe the reconstruction options and patient outcomes after flexor tendon rupture due to volar plating.
METHODS: This is a single-institution, retrospective review of patients who presented with flex

The distal attachments of the extensor hallucis longus (EHL) tendons in 47 amputated legs and in eight cadavers were examined. The EHL had t … to obtain data for using these tendons in tendon repair and hallux varus corrections by autogenous tendon transfer surgery. Additionally, on the right foot of one of the cadavers, it was observed that the extensor hallucis brevis tendon united with the lateral tendon of the EHL. We recommend that foot-ankle surgeons be aware of the various accessory EHL tendons …

Peroneal tendon pathology is often an overlooked and underdiagnosed condition. It is often confused with chronic ankle instability. It is important when surgically managed to assess the condition of the tendons, muscle viability and strength, and associated cavovarus deformity. Complex reconstruction may be needed, including 2-stage procedures with a silicone rod and tendon transfer.

An improved method for reconstructing injured tendons of the dorsum of the hand is presented. We transferred a two-layered temporal fascial flap to the hand and inserted the rolled, deep temporal fascia between the stump of the extensor tendons. This procedure can improve extensor tendon function with minimal scarring in spite of the limitation of available tissue.

Cerebrotendinous xanthomatosis is a rare, autosomal recessive, lipid storage disease with accumulation of cholestanol in most tissues, particularly in the Achilles tendons. We described a 23-year-old female patient who had progressive painfull swelling of both Achilles tendons due to cerebrotendinous xanthomatosis. We performed surgery on both-side Achilles tendon tumors. Wide degenerative areas of the tendons were resected, and the flexor hallucis longus tendon was harvested and transferred to reconstruct motion function.

BackgroundPeroneal tendon disorders, encompassing tendinopathy, tears, subluxation, and instability, represent a significant source of lateral ankle pain and functional impairment, often exacerbated by underlying conditions such as cavovarus foot deformities or chronic lateral ankle instability. In cases of severe peroneus brevis degeneration or irreparable tears, peroneal longus to brevis tendon transfer is an effective technique to restore eversion strength and dynamic stability, though challenges persist in patients with poor tissue quality necessitating augmentation.PurposeTo present a nov…

We report on a 74-year-old man with extensor indicis proprius (EIP) tendon rupture secondary to avascular necrosis and collapse of the scaphoid. The collapsed scaphoid was removed, followed by 4-corner fusion and tendon transfer of the extensor digiti minimi to the EIP. At the 3-year follow-up, the patient was painfree and had full extension of his left index finger, with proper alignment of the carpal bones and union of the 4 bones.

The calcaneonavicular (spring) ligament complex is the soft tissue most often seen to fail in flatfoot pathology and is associated with deformity of the talonavicular joint. The spring ligament complex supports the talar head, preventing it from displacing into excessive plantar flexion/adduction. An anatomical reconstruction of the spring ligament should replicate this function. A new method of spring ligament reconstruction using autogenous flexor hallucis longus tendon transfer is reported.

The majority of patients with Achilles tendinopathy respond to nonoperative treatment. In patients with refractory symptoms, surgery can be considered. If paratenonitis is present, the paratenon is partially excised, and adhesions are released. Areas of symptomatic tendinosis are excised with repair of the residual defect in the Achilles tendon. An alternative for patients with tendinosis who are at increased risk for wound problems or who do not want a large open incision is percutaneous or endoscopic tenotomy. A symptomatic Haglund's deformity or inflamed retrocalcaneal bursa is excised. Aug

Stage II PTT dysfunction is a complex problem that has multiple treatment options. Whether it is treated nonoperatively or operatively, multiple factors affect the success of treatment. The orthopedic literature reports many factors that can lead to failure. Although it is virtually impossible to account for every possible scenario, adhering to sound principles and understanding what can go wrong and why, can help to avoid many of the pitfalls.

Repair of late avulsion of the flexor pollicis longus tendon at insertion is a rare but difficult problem. A 52-year-old female labourer presented with a 3-month history of inability to flex her right thumb at the interphalangeal joint. Avulsion of the flexor pollicis longus tendon at its insertion was noted on exploration. Because the tendon had contracted and could not be reinserted directly, a technique of step-wise lengthening was used for reinsertion. Two months after operation, the patient resumed her daily activities and went back to work with satisfactory use of her right thumb. This r
