Hospice Clinicians' Approaches to Terminal Restlessness: A Qualitative Analysis.

CONTEXT: Terminal restlessness is frequently observed in hospice, yet it lacks a consistent definition, diagnostic framework, and treatment approach. This ambiguity complicates care by increasing caregiver distress, delaying diagnosis, and creating uncertainty around appropriate treatment. OBJECTIVES: This qualitative study explored how hospice clinicians define, diagnose, and pharmacologically manage terminal restlessness in practice. METHODS: We conducted semi-structured interviews with hospice clinicians from diverse settings across Michigan. Participants were asked how they define termin

Continuous sedation in adolescent and young adult cancer patients under home-based end-of-life care.

PURPOSE: This study examined the implementation status of continuous sedation (CS) and its clinical and social background factors in adolescent and young adult (AYA) patients with cancer who received end-of-life care at home. METHODS: We conducted a single-center retrospective observational study with 62 AYA patients with cancer. We compared the CS (S) and non-sedation (NS) groups, analyzing factors contributing to sedation implementation. RESULTS: CS implementation rate was 37.1% (23/62 cases). Participants' median age was 36 (S group) and 33 (NS group) years (p = 0.08). Both groups had sim

Palliative and end of life care in older major trauma - A point prevalence evaluation in England, Wales and Scotland.

INTRODUCTION: Traumatic injury in older people is a significant health burden with higher mortality rates than younger cohorts. Survival following older trauma may be complicated by the patients pre-injury state and clinical uncertainty. Timely identification of palliative and end-of-life care needs may be challenging for acute clinical teams, and treatment escalation planning is not routinely embedded in trauma care. This point prevalence snap-shot aimed to evaluate treatment escalation discussions and palliative/end of life care (EoLC) practice in older major trauma patients at a national le

High-intensity treatment is common at the end of life among children and young people who died of cancer: a retrospective cohort study.

BACKGROUND: Annually, approximately 500 children and young people (aged <25 years) with cancer die in England. Little is known about the intensity of their end-of-life care. AIM: The primary aim of this study was to establish the prevalence of high-intensity treatment at the end of life in England and assess the relationship between high-intensity treatment and palliative care integration in main cancer treatment centres. The secondary aim was to assess the relationship between palliative care integration and place of death. DESIGN: Retrospective, national full-population cohort study. SETT

Associations Between End-Stage ALS Care and Specialty Palliative Care: A Hypothesis-Generating Study.

INTRODUCTION/AIMS: Amyotrophic lateral sclerosis (ALS) care is typically delivered via a multidisciplinary approach that may include specialty palliative care (SPC). Opportunities for SPC to enhance ALS care have been identified; however, investigation of these proposed benefits is scant. In this exploratory study, investigators examined associations between receipt of SPC and variables particularly relevant to end-stage ALS. METHODS: Researchers reviewed electronic health records for all patients with ALS who received standard ALS care from one Midwestern US academic medical center and died

[What really matters can't be planned ahead : Impediments for advance care planning from a qualitative study on very old people living alone].

BACKGROUND: Despite there is evidence for the need for advance care planning for older people living alone (OPLA), especially if they want to stay at home until the end, utilization of measures for ACP remains rather scarce. The causes for this discrepancy have not yet been researched in this target group. OBJECTIVES: Aim of this study was to get insight into the perspectives of OPLA in the community on planning ahead for the end of life. MATERIALS AND METHODS: (Secondary) Analysis of 26 qualitative interviews. 17 were from the Austrian Survey on the very old (ÖIHS), and 9 additional intervi

Lived experiences of family caregivers of those with advanced illnesses: A secondary qualitative data analysis.

BACKGROUND: Family caregivers play a crucial role in providing home palliative care for non-cancer patients with advanced illnesses. Given the complexity and heterogeneity of end-of-life caregiving experiences, there is a need to understand factors influencing caregiver outcomes for different profiles of caregivers. It is also crucial to consider the influence of cultural values on caregiving experiences, particularly in Asian settings. AIM: To explore the lived experiences of caregivers of non-cancer patients with advanced illnesses in Singapore. DESIGN: A secondary qualitative analysis of

Promoting Continuity of Care During End-of-Life in Bangkok: An Action Research Study.

Continuity of care prioritizes patient-centered approaches. Effective continuity of care is essential for ensuring high-quality service delivery and benefits for patients in the end-of-life phase. Although continuity of care has been well-established, few studies have focused on promoting continuity of care in big cities. The purpose of this action research was to develop a model for continuity of care during the end-of-life phase in Bangkok. Key participants were 8 nurses involved in continuity of care services. The data collection consisted of in-depth interviews, focus group discussions, ob

[Psychiatric symptoms at the end of life, part 2: Depressive disorder].

The prevalence of depression, including adjustment disorders, in palliative care ranges from 20% to 40%. Depressive symptoms are often not reported spontaneously and must be classified using differential diagnosis for therapy planning. The generally negative view caused by the illness often makes therapeutic access to patients difficult. This article presents and explains the diagnosis, aetiology and various non-pharmacological and pharmacological treatment options for the different depressive syndromes that are relevant to general practice.

Monitoring the clinical practice of palliative sedation (PALSED) in patients with advanced cancer: an international, multicentre, non-experimental prospective observational study protocol.

BACKGROUND: Palliative sedation involves the intentional lowering of consciousness at the end of life. It can be initiated to relieve a patient's burden caused by refractory symptoms at the end of life. The impact of palliative sedation needs to be clinically monitored to adjust the proper dose and regimen of sedative medication to ensure that patients are at ease and comfortable at the end of their lives. Although there is consensus among health care professionals and within guidelines that efficacy of palliative sedation needs to be closely monitored, there is no agreement about how, when, a

The meaning of living with uncertainty for people with motor neurone disease.

AIMS AND OBJECTIVES: To explore the meaning of living with uncertainty for people diagnosed with motor neurone disease (MND). BACKGROUND: Motor neurone disease is a progressive neurodegenerative condition resulting in multiple needs, arising from the complex nature of the disease trajectory. People with MND are often required to make decisions for symptom management and end-of-life care. Research into the lived experience of MND has previously highlighted the following: the shock of receiving such a diagnosis and prognosis; subsequent concerns relating to the future and loss; and the existent

Self-management support at the end of life: Patients', carers' and professionals' perspectives on managing medicines.

BACKGROUND: Pain is a frequently reported symptom by patients approaching the end of life and well-established that patients and carers hold fears relating to opioids, and experience side effects related to their use. The management of medicines is intrinsic to achieving effective pain relief. The concept of self-management support whilst well characterised in the context of chronic illness has not been elaborated with respect to end of life care. AIM: To identify patient, carer and professional views on the concept of self-management support at end of life, specifically in relation to analge

Nutritional support at the end of life.

For patients with cancer at the end of life the goal of nutritional care is to optimize quality of life and comfort. Food and drink should be served as requested by the patient but without exerting pressure. For patients who have developed cachexia and are potentially candidates to receive artificial nutrition, discussions between the patient, family and health care team are needed to set the goals of nutritional care, considering both the risk of adverse effects of the treatment and ethical issues. The premise for a benefit from parenteral nutrition is that survival of the tumor spread exceed

Palliative use of non-invasive ventilation in end-of-life patients with solid tumours: a randomised feasibility trial.

BACKGROUND: Despite best-possible medical management, many patients with end-stage cancer experience breathlessness, especially towards the end of their lives. We assessed the acceptability and effectiveness of non-invasive mechanical ventilation (NIV) versus oxygen therapy in decreasing dyspnoea and the amount of opiates needed. METHODS: In this randomised feasibility study, we recruited patients from seven centres in Italy, Spain, and Taiwan, who had solid tumours and acute respiratory failure and had a life expectancy of less than 6 months. We randomly allocated patients to receive either

Managing Use of Propofol for Refractory Symptoms in End-of-Life Care: Creating an Institutional Guideline.

BACKGROUND: End of life (EOL) care in hospitals is increasingly complex as patients are often critically ill and medically unstable. Propofol can be an important tool in treating refractory symptoms during EOL care to ensure comfort and prevent suffering during the dying process. INTERVENTION: We aimed to define and standardize institutional practices for using propofol to manage refractory symptoms during EOL care. Using expert opinions, consensus building, and a microsystems team approach, we defined parameters for propofol use in managing refractory symptoms. We developed a flow diagram to
