Geometric, Biomechanic and Haemodynamic Aortic Abnormalities Assessed by 4D Flow Cardiovascular Magnetic Resonance in Patients Treated by TEVAR Following Blunt Traumatic Thoracic Aortic Injury.

OBJECTIVE: Thoracic endovascular aortic repair (TEVAR) is widely used for the treatment of patients with blunt traumatic thoracic aortic injury (BTAI). However, aortic haemodynamic and biomechanical implications of this intervention are poorly investigated. This study aimed to assess whether patients treated by TEVAR following BTAI have thoracic aortic abnormalities in geometry, stiffness, and haemodynamics. METHODS: Patients with BTAI treated by TEVAR at Vall d'Hebron Hospital between 1999 and 2019 were compared with propensity score matched healthy volunteers (HVs). All subjects underwent c

Risk factors of acute kidney injury in patients with Stanford type B aortic dissection involving the renal artery who underwent thoracic endovascular aortic repair.

BACKGROUND: Acute kidney injury (AKI) is one of the most common and serious complications in patients with type B aortic dissection (TBAD). This study aimed at investigating the incidence and risk factors of in-hospital AKI in TBAD patients involving the renal artery who underwent thoracic endovascular aortic repair (TEVAR) only. METHODS: A total of 256 patients who were diagnosed as TBAD combined with renal artery involvement were included in this retrospective study. All patients were divided into the AKI group and the non-AKI group according to the KDIGO criteria. The risk factors for AKI

Genetic variants in inducible nitric oxide synthase gene are associated with the risk of radiation-induced lung injury in lung cancer patients receiving definitive thoracic radiation.

BACKGROUND AND PURPOSE: Nitric oxide (NO), mainly synthesized by inducible nitric oxide synthase (NOS2) in pathological conditions, plays an important role in cytotoxicity, inflammation and fibrosis. Elevations in exhaled NO after thoracic radiation have been reported to predict radiation-induced lung injury (RILI). This study examined whether genetic variations in NOS2 gene is associated with the risk of RILI. MATERIAL AND METHODS: A cohort of 301 patients between 2009 and 2011 were genotyped for 21 single nucleotide polymorphisms (SNPs) in the NOS2 gene by the Sequenom MassArray system. Kap

Sonic Hedgehog Signaling Promotes Survival and Lineage Specific Differentiation of Transplanted Neural Precursor Cells and Improves Motor Recovery After Thoracic Spinal Cord Injury in Rats.

Transplanted neural precursor cells (NPCs) can support repair after spinal cord injury (SCI) but often face poor survival and limited differentiation within the hostile post-injury microenvironment. We tested whether subacute activation of Sonic hedgehog (Shh) signaling improves NPC engraftment and functional recovery over the duration of 6 weeks. Female Wistar rats underwent T9/T10 clip-contusion SCI and were assigned to vehicle, Shh-only, NPC-only, Shh + NPC, or sham groups (n = 8 per SCI arm, n = 5 sham, total n = 37). GFP + NPCs (4 × 105) were grafted at 7 days post injury. Recombinant Shh

Acute intermittent hypoxia elicits sympathetic neuroplasticity independent of peripheral chemoreflex activation and spinal cord tissue hypoxia in a rodent model of high-thoracic spinal cord injury.

The loss of medullary control of spinal circuits controlling the heart and blood vessels is a unifying mechanism linking both hemodynamic instability and the risk for cardiovascular diseases (CVD) following spinal cord injury (SCI). As such, new avenues to regulate sympathetic activity are essential to mitigate CVD in this population. Acute intermittent hypoxia (AIH) induces a type of neuroplasticity known as long-term facilitation (LTF), a persistent increase in nerve activity post-AIH in spinal motor circuits. Whether LTF occurs within the sympathetic circuit following SCI is largely unknown

Effect of colchicine on perioperative atrial fibrillation and myocardial injury after non-cardiac surgery in patients undergoing major thoracic surgery (COP-AF): an international randomised trial.

BACKGROUND: Higher levels of inflammatory biomarkers are associated with an increased risk of perioperative atrial fibrillation and myocardial injury after non-cardiac surgery (MINS). Colchicine is an anti-inflammatory drug that might reduce the incidence of these complications. METHODS: COP-AF was a randomised trial conducted at 45 sites in 11 countries. Patients aged 55 years or older and undergoing major non-cardiac thoracic surgery were randomly assigned (1:1) to receive oral colchicine 0·5 mg twice daily or matching placebo, starting within 4 h before surgery and continuing for 10 days.

A comparative study of the effects of spontaneous ventilation versus one-lung mechanical ventilation on ipsilateral lung injury during thoracic surgery: an empirical analysis based on a rat model.

BACKGROUND: One-lung ventilation (OLV) is prone to adverse reactions, particularly ventilator-induced lung injury (VILI). Spontaneous ventilation (SV) is supposed to reduce this risk. To investigate the effects of the ventilated lung during surgery, we established a rat model for research. METHODS: Forty-two rats were randomly divided into 7 groups: control, OLV, and SV [OLV/SV group was subdivided into 3 time points (10, 20, 30 min)]. Blood gas analysis, bronchoalveolar lavage fluid (BALF) test, and lung tissue wet-to-dry ratio were performed. Hematoxylin and eosin (HE), Masson and immunohis

Hemothorax caused by zone I penetrating neck injury effectively treated with pleural dome laceration covering performed using video-assisted thoracic surgery: a case report.

BACKGROUND: Penetrating neck injuries in the area between the clavicles and the cricoid cartilage sometimes cause intrathoracic problems such as pneumothorax or hemothorax. Injuries of subclavian vessels were reported as a cause of hemothorax caused by penetrating neck injuries. Coil embolization and hemostasis with electrocautery were reported as treatment. Herein we present a case of hemothorax caused by penetrating neck injury, which was treated with pleural dome laceration covering performed using video-assisted thoracic surgery. CASE PRESENTATION: A 68-year-old Japanese woman presented t
