Thyroid disease in children.

Among thyroid diseases occurring in children are congenital hypothyroidism, acquired hypothyroidism, Graves' disease, subacute thyroiditis and thyroid nodules. Evaluation for suspected hypothyroidism or hyperthyroidism can be accomplished efficiently with a serum free thyroxine level and the new, highly sensitive thyroid-stimulating hormone assays. Rarely, measuring various thyroid antibody titers may be required to better define a particular thyroid disease. Solitary thyroid nodules in children occasionally represent malignancy. Evaluation of such lesions with radioisotope scanning and ultras

Granulomatous Thyroiditis: A Case Report and Literature Review.

BACKGROUND: Granulomatous disease in the thyroid gland has been linked to viral, bacterial and autoimmune etiologies. The most common granulomatous disease of the thyroid is subacute granulomatous thyroiditis, which is presumed to have a viral or post-viral inflammatory cause. Bacterial etiologies include tuberculosis, actinomycosis, and nocardiosis, but are extremely rare. Disseminated actinomycosis and nocardiosis more commonly affect organ-transplant patients with the highest susceptibility within the first year after transplant surgery. CASE: A 45-year-old African American male, who recei

[Thyroiditis: What's new in 2019?].

Thyroiditis is a frequent and mostly benign disease that can sometimes disrupt the thyroid balance. Their diagnosis, as well as their aetiology, is a necessary step in the management of the patients. Painful thyroiditis includes acute thyroiditis of infectious origin and subacute thyroiditis. The first one can be treated by antibiotics or antifungals depending on the germ found. The second one will be treated with non-steroidal anti-inflammatory drugs or corticosteroids. In cases of Hashimoto's thyroiditis with overt hypothyroidism, replacement therapy with L-thyroxine will be adapted to the T

Development of adrenocortical failure in non-Addisonian patients with antibodies to adrenal cortex. A clinical follow-up study.

Out of 1036 sera tested for the presence of antibodies to adrenal cortex using the indirect immunofluorescence (IFL) method, fifteen out of 323 patients with Graves' thyrotoxicosis, 6/105 Hashimoto's thyroiditis, 3/29 myxoedema, 2/49 asymptomatic autoimmune thyroiditis, 1/12 subacute thyroiditis, and 4/130 with myasthenia gravis were positive. Thirty antibody-positive non-addisonian patients were tested for subclinical adrenocortical failure and followed up over a period of 1-3.5 years. Three women with Graves' disease had biochemical signs of early Addison's disease at the first Synacthen tes

I saw the "thyroid inferno" sign: A fiery Doppler signature of Graves' disease.

Color Doppler ultrasonography was first used in 1988 by Ralls et al. to evaluate blood flow to the thyroid gland in patients with Graves' disease [1]. The "thyroid inferno" is the imaging demonstration of markedly increased vascularity and arteriovenous shunting seen with Graves' Disease (Fig. 2 and 1B) b[1]. This pattern consists of multiple small pulsatile areas of intrathyroidal flow seen diffusely throughout the gland in both systole and diastole. This hypervascularization reflects the hemodynamic changes of hyperfunctioning thyroid and/or an ongoing inflammatory process and is similar in

Iodine-induced subclinical hypothyroidism in euthyroid subjects with a previous episode of amiodarone-induced thyrotoxicosis.

Amiodarone-induced thyrotoxicosis (AIT) occurs most frequently in patients with underlying thyroid disease and is generally believed to be due to the iodine contamination of amiodarone and iodine released by the metabolism of the drug. We and others have suggested that the thyrotoxicosis may also be secondary to amiodarone-induced thyroiditis. To further determine the etiology of AIT, we administered large doses of iodides [10 drops saturated solution of potassium iodide (SSKI) daily] to 10 euthyroid patients long after an episode of AIT believed to be due at least in part to amiodarone-induce

The hypothalamus-pituitary-thyroid axis is disrupted by exposure to a mix of tributyltin and bisphenol S.

Tributyltin is a biocide and bisphenol S is a plasticizer. The effects of the TBT + BPS mix on thyroid axis function are unknown. This study evaluated the effects of subacute exposure to TBT and BPS, both in mix and alone, in female young Wistar rats. Thyroid morphophysiology, gene expression, oxidative stress and collagen deposition were evaluated. TBT and BPS exposure resulted in a decrease in thyroid hormone levels, whereas TBT alone resulted in a decrease in TSH levels. The TBT + BPS group exhibited an increase in T4 levels, a decrease in T3 levels, a decrease in TPO activity, and an incre

Hyperuricemia in patients with hyperthyroidism due to Graves' disease.

The effects of hyperthyroidism on uric acid metabolism were investigated. First, the serum uric acid level was measured in 92 patients with hyperthyroidism due to Graves' disease, eight patients with subacute thyroiditis, six patients with hypothyroidism, and 70 sex- and age-matched controls. Second, the correlation between serum thyroxine (T4) and serum uric acid was obtained in hyperthyroid Graves' disease patients before and during antithyroid drug therapy. Finally, uric acid clearance (CUA) was determined in untreated patients with hyperthyroidism due to Graves' disease. Serum uric acid wa

A rare case of cholesterol granuloma in the thyroid without an abnormality of lipid metabolism.

This report describes a cholesterol granuloma (CG) of the thyroid. The patient was a 79-year-old male. The mass in the thyroid was observed by chest computed tomography (CT). Initially, he had no clinical symptoms and the mass was not palpable. However, it started and became palpable and painful. He was diagnosed to have subacute thyroiditis. Although he was administered prednisolone (PSL), the mass grew larger and more solid. Then thyroid left lobectomy was performed under general anesthesia. The histological diagnosis was CG of the thyroid. After surgery his clinical course was favorable.

Repeated misdiagnosis of squamous cell carcinoma of the thyroid: a case report.

Squamous cell carcinoma of the thyroid (SCCT) represents a rare subtype of thyroid malignancy characterized by a poor prognosis and is frequently misdiagnosed as subacute thyroiditis (SAT). This report discusses a case of SCCT that was recurrently misdiagnosed with neck pain as the initial clinical presentation. An 80-year-old Asian female presented multiple times to the department of emergency, department of vascular and thyroid surgery, and department of endocrinology due to a painful neck mass. Ultrasonography identified a cystic solid mass in the right thyroid lobe, measuring 4.55 cm*3.2 c

Thyroiditis de Quervain. Are there predictive factors for long-term hormone-replacement?

BACKGROUND: Subacute thyroiditis is a usually self-limiting disease of the thyroid. However, approximately 0.5-15% of the patients require permanent thyroxine substitution. Aim was to determine predictive factors for the necessity of long-term hormone-replacement (LTH). PATIENTS, METHODS: We retrospectively reviewed the records of 72 patients with subacute thyroiditis. Morphological and serological parameters as well as type of therapy were tested as predictive factors of consecutive hypothyroidism. RESULTS: Mean age was 49 ± 11 years, f/m-ratio was 4.5 : 1. Thyroid pain and signs of hyperth

Steroid-responsive encephalopathy associated with autoimmune thyroiditis presenting with diffusion MR imaging changes.

Steroid-responsive encephalopathy associated with autoimmune thyroiditis (SREAT) presents with focal or diffuse nonenhancing MR imaging abnormalities in 50% of patients with SREAT during subacute exacerbation. Vasculitic changes in biopsy studies as well as the elevation of antithyroid antibodies and CSF protein suggests an inflammatory cause. We report the case of a patient with SREAT with changes on diffusion-weighted MR imaging, which improved with corticosteroid therapy and plasmapheresis, supporting the theory of inflammatory changes in exacerbation of presumptive SREAT.

[Case report: persistent fever after SARS-CoV-2 infection].

A 44-year-old man had persistent fever following a severe acute respiratory syndrome coronavirus type 2 (SARS-CoV-2) infection. Because of progressive sinus tachycardia, thyroid hormones were measured which showed hyperthyroidism. Thyroid sonography revealed enlargement of the thyroid gland with hypoechoic areas with blurred margins. We diagnosed subacute granulomatous thyroiditis associated with SARS-CoV‑2 infection and initiated therapy with prednisolone. This therapy resulted in rapid improvement of the patient's clinical condition and complete remission after three months.

[99mTc-pertechnetate neck uptake in the evaluation of thyroid function--simple method with gamma camera and pinhole collimator (author's transl)].

The neck uptake ratio of 99mTcO4- was measured using a gamma camera and pinhole collimator. The patient was sit 2 m apart from the collimator surface and radioactivity was counted 20 minutes after injection. The whole body count with lead shield (Cc) and without shield (Cd), and natural background count (CBG) were measured. As the shield was utilized the B filter for conventional thyroidal 131I uptake test (lead plate of 1.3 cmx10 cmx10 cm) and it was placed over the anterior neck to cover the thyroid gland at the measurement of (Cc). Then 99mTcO4- neck uptake ratio was calculated as following

Cortico-responsive encephalopathy associated with autoimmune thyroiditis (SREAT): about two case reports characterized by a gap between the diagnosis of autoimmune thyroiditis and neurological disorders.

We report two cases of steroid responsive encephalopathy associated with autoimmune thyroiditis (SREAT) often called "Hashimoto's encephalopathy" in which the neurological manifestations develop years before or after the Hashimoto's diagnosis. Because of this specific presentation, the etiological diagnosis of this type of encephalopathy can be a difficult task. In our patients there was a gap of 10 to 20 years between the proof of autoimmune thyroiditis and the neurological symptoms. Case reports of this type of presentation are rare in the literature. A dramatic responsiveness to steroids wi

Calcium oxalate crystals in the thyroid. Their identification, prevalence, origin, and possible significance.

Calcium oxalate crystals are not encountered in normal animal tissues, except for the human thyroid, where they were found in 79 of 100 routine consecutive autopsies. They appear during childhood, and numbers of crystals increase with age. In diffuse hyperplasia, prevalence was higher, but crystals were fewer than expected. In adenomas and carcinomas, crystals were decreased except for three cases with a striking focal increase. None was found in 22 adult primate thyroids. After Clorox digestion of human thyroids, calcium oxalate dihydrate was identified by x-ray diffraction and infrared spect

Hyperthyroidism from thyroid metastasis of pancreatic adenocarcinoma.

A nontender goiter rapidly developed in a 54-year-old patient with suspected disseminated carcinoma. Thyroid function tests showed increased thyroxine, triiodothyronine resin uptake, free thyroxine index, and free thyroxine. Radioactive iodine uptake by the gland was near zero, and thyroid-stimulating hormone (TSH) was undetectable. Histologic examination of the thyroid before and after death showed invasion and disruption of the thyroid follicles by adenocarcinoma (pancreatic primary). Release of thyroglobulin by follicular disruption probably resulted in hyperthyroxinemia and suppression of

[Increased serum soluble Fas ligand in hyperthyroid Graves' disease].

The interaction of Fas with its ligand (FasL) regulates a number of physiological and pathophysiological process of cell death or apoptosis. Recent studies suggest that Fas and Fas ligand (FasL) interactions among thyrocytes from patients with Hashimoto disease which is caused by thyroid autoimmunity may contribute to clinical hypothyroidism. The role of Fas-FasL interaction in the pathophysiology of Graves' disease has not well been determined. The serum levels of soluble Fas (sFas) and FasL (sFasL) were measured in 48 Japanese patients with Graves' disease (U; untreated hyperthyroidism, T; h
