Prevalence and risk factors of sleep bruxism and wake-time tooth clenching in a 7- to 17-yr-old population.

Sleep-related bruxism (SB) and wake-time tooth clenching (TC) have been associated with temporomandibular disorders (TMDs), headache, and sleep and behavioral complaints. This study aimed to assess the prevalence and risk factors of these signs and symptoms in a 7- to 17-yr-old population (n = 604) seeking orthodontic treatment. Data were collected by questionnaire and by a clinical examination assessing craniofacial morphology and dental status. Sleep-related bruxism was reported by 15% of the population and TC was reported by 12.4%. The SB group (n = 58) was mainly composed of children (67.3

A pilot randomized trial of a novel digital mandibular advancement device with customized lip bumper: preliminary evaluation of treatment efficacy and relief of lower anterior dental pain in obstructive sleep apnea-hypopnea syndrome.

BACKGROUND: Mandibular advancement devices (MADs) are effective alternative therapies for obstructive sleep apnea-hypopnea syndrome (OSAHS). However, traditional monoblock mandibular advancement devices (M-MADs) often cause lower anterior dental pain and tooth displacement during long-term use, impairing compliance. Digital technology enables optimized appliance design. Therefore, we designed a novel digitally fabricated mandibular advancement device with a customized lip bumper (B-MAD/CLB) and compared its efficacy to the digitally fabricated M-MAD in treating OSAHS. METHODS: A randomized con

Remineralisation effect of a dual-phase calcium silicate/phosphate gel combined with calcium silicate/phosphate toothpaste on acid-challenged enamel in situ.

OBJECTIVES: To test if a novel dual-phase gel system (calcium silicate and phosphate with 1450 ppmF, as NaF/MFP; TG) combined with a toothpaste (calcium silicate and sodium phosphate with 1450 ppmF, as MFP; TG) was able to re-harden previously acid-challenged enamel to a greater extent than other toothpastes. METHODS: The study consisted of a double-blind, randomised, cross-over design with four 7-day treatment legs. In each leg, subjects wearing a partial denture holding four demineralised enamel specimens (25 min in 0.3% citric acid, pH3.8) used either the test regimen (TG+TP) or one of the

Knowledge and Prevalence of Bruxism and Its Association With Stress Among Clinical-Year Dental and Medical Students at the University of Ghana.

BACKGROUND: Bruxism is a repetitive jaw-muscle activity characterised by clenching or grinding of the teeth and/or by bracing or thrusting of the mandible. Current international consensus regards it as a motor behaviour rather than a disorder, and one that may act as a risk factor for tooth wear and temporomandibular disorders; psychosocial stress is among its most consistently reported associated factors. Clinical-year dental and medical students face intense academic and clinical pressure that may predispose them to both stress and bruxism, yet no published Ghanaian data on this relationship…

A pilot randomised controlled trial evaluating mini and conventional implant retained dentures on the function and quality of life of patients with an edentulous mandible.

BACKGROUND: Total tooth loss (edentulism) can be a debilitating condition, impacting on ability to chew, speak and interact with others. The most common treatment is with complete removable dentures, which may be successful, but in the lower jaw, bone resorption that worsens over time makes denture-wearing difficult. Two dental implants in the mandible to retain the lower denture has been advocated as the gold standard of treatment, but has not been universally provided due largely to financial constraints and also patient fear. Mini implants (MI) are cheaper and less invasive than conventiona

Performance of PEEK based telescopic crowns, a comparative study.

OBJECTIVE: Telescopic crowns are suitable components of partial dentures to efficiently anchor dental supra-structures to teeth or dental implants and achieve high chewing performance and wear comfort. Usually alloy- or metal-based structures are used for the primary and the secondary crowns. The advantage is the possibility to produce precise structures with a high perfection and sufficient friction force, but the disadvantage is the corrosion instability. The recent introduction of zirconia ceramics has enabled the fabrication of ceramic primary crowns, thus reducing corrodibility. The novel

Effect of occlusal wear on bone loss and Periotest value of dental implants.

PURPOSE: Occlusal overload may contribute to the loss of osseointegration of oral implants, so some clinicians are reluctant to place implants in patients with signs of bruxism. This study evaluated the effect of occlusal wear as a probable sign of bruxism on bone loss and implant stability. MATERIALS AND METHODS: The study investigated 379 patients who had worn implant-retained or implant-supported restorations for many years. Occlusal wear, patient age and gender, time of prosthetic loading, jaw, location in the dental arch, implant diameter, prosthesis construction, occlusal material, peri

Risk management in clinical practice. Part 8. Temporomandibular disorders.

Temporomandibular disorders can arise apparently idiopathically, as a result of macro-trauma or micro-trauma such as parafunction, as a result of a separate disease process or as a consequence of dental treatment. The objectives of this chapter are to make the practitioner aware of his/her responsibilities in any of these situations. Precise record keeping and careful risk management are essential. Guidelines are given to protect not only the practitioner but also the patient.

Digital Design for Lower Incisor Position Correction in a Growing Patient with Mandibular Retrusion with ClinCheckÒ Software: A Case Report.

Background: The majority of Class II malocclusions stem from mandibular deficiency, leading to chin retrusion. In growing patients, the ideal correction-aiming for a skeletal mandibular response-should avoid common pitfalls such as "Point B" dropping postero-inferiorly, excessive labial proclination of mandibular incisors, or the lingual tipping and extrusion of maxillary incisors. When planning mandibular advancement (MA) using clear aligners with integrated advancement features, biomechanical forces are not the only consideration; precise management of the lower incisor position is critical

Comparative Efficacy of Botulinum Toxin Type A Versus Antihomotoxic Therapy in the Treatment of Bruxism-Related Masticatory Muscle Hyperactivity: A Randomized Clinical Trial Using Electromyographic Measurements.

BACKGROUND: Bruxism is currently defined, according to the international consensus of Lobbezoo and colleagues, as a repetitive masticatory muscle activity characterised by clenching or grinding of the teeth and/or bracing or thrusting of the mandible. It is a motor behaviour-not a disorder in itself-that may act as a risk factor for orofacial pain, tooth wear, masticatory muscle hypertrophy and temporomandibular disorders in susceptible individuals. Pharmacological management is indicated only when such sequelae are present. Botulinum toxin type A (BTX-A) has emerged as a widely investigated o…
