[Rehabilitation of persons with psychic disorders or handicaps in transitional facilities and its place within the social security system (author's transl)].

Rehabilitation in transitional facilities (transitional residential homes) is a possible approach for those persons with psychic disorders or handicaps who are not in need of institutional care but cannot be treated adequately on an outpatient basis either. The cost of such rehabilitative programmes is carried by the social welfare authorities in more than 85 percent of the total number of clients. From the point of view of those concerned, who previously had been contributing to social insurance schemes, referral to social assistance is deemed an incompatibility. A change to his situation cou

Bridge Healing: A Pilot Project of a New Model to Prevent Repeat "Social Admit" Visits to the Emergency Department and Help Break the Cycle of Homelessness in Canada.

Homelessness continues to be a pervasive public health problem throughout Canada. Hospital Emergency Departments (EDs) and inpatient wards have become a source of temporary care and shelter for homeless patients. Upon leaving the hospital, homeless patients are not more equipped than before to find permanent housing. The Bridge Healing program in Edmonton, Alberta, has emerged as a novel approach to addressing homelessness by providing transitional housing for those relying on repeated visits to the ED. This paper describes the three essential components to the Bridge Healing model: partnershi

Pediatric to Adult Hydrocephalus: A Smooth Transition.

INTRODUCTION: Pediatric patients treated for hydrocephalus, regardless of etiology, require continuous access to care to address the long-term sequelae from the disease progression itself and from the interventions undertaken. The challenge for all pediatric neurosurgeons is providing comprehensive and coordinated care for these patients in order to achieve a smooth and seamless transition into adult health care. METHODS: A review of the literature was conducted regarding the overall concept of pediatric patients with chronic conditions transitioning to adult care. We also specifically review

[Essen transition model for neuromuscular diseases].

BACKGROUND: Advances in healthcare systems with new therapeutic options improve the life expectancy of patients with neuromuscular diseases. With this, a shift in the phenotype of the diseases from the neuromuscular system towards other organs is more frequently observed, requiring closer interdisciplinary cooperation in caring for these young adults. Therefore, the transition to the adult caring system is nowadays a multilayered transfer with the need for complex care of these patients. OBJECTIVE: How can the transitional process be efficiently structured to combine the therapeutic effort of

Congenital diaphragmatic hernia: survival treated with very delayed surgery, spontaneous respiration, and no chest tube.

This report suggests that stabilization of the intrauterine to extrauterine transitional circulation combined with a respiratory care strategy that avoids pulmonary overdistension, takes advantage of inherent biological cardiorespiratory mechanics, and very delayed surgery for congenital diaphragmatic hernia results in improved survival and decreases the need for extracorporeal membrane oxygenation (ECMO). This retrospective review of a 10-year experience in which the respiratory care strategy, ECMO availability, and technique of surgical repair remained essentially constant describes the evol

Collaboration, coordination and communication as facilitators of transitions for patients with advanced cancer: a scoping review linked to the Pal-Cycles project.

BACKGROUND: Person-centred care is becoming increasingly recognised as an important element of palliative care. The current review syntheses evidence in relation to transitions in advanced cancer patients with palliative care needs. The review focuses on specific elements which will inform the Pal-Cycles programme, for patients with advanced cancer transitioning from hospital care to community care. Elements of transitional models for cancer patients may include, identification of palliative care needs, compassionate communication with the patient and family members, collaborative effort to es

Strengths and limitations of healthcare transition programs for patients with spina bifida: a systematic review.

BACKGROUND: Due to the complex needs of patients with spina bifida, many pediatric clinics have adopted a multidisciplinary approach, which allows the patient to see multiple specialists in one setting. However, patients with spina bifida require ongoing care throughout their lifetime, and many patients experience interruptions in care when they transition to adulthood; therefore, many centers have also begun to develop programs to streamline the transition process. The goal of this review is to summarize the existing healthcare transition programs for patients with spina bifida that have been

Serious Mental Illness-Associated Socio-Medical Needs and Imprisonment Risk in Reentry Clients.

OBJECTIVE: To examine whether the socio-medical needs and imprisonment risk among transitional care clients with serious mental illness (SMI) differ from other transitional care clients. METHODS: A retrospective cohort study was conducted of adults who entered the Formerly Incarcerated Transition (FIT) Program from 2018 to 2023 following a period of incarceration. At program entry, FIT clients were screened for socio-medical needs and diagnoses, including those reflecting SMI. Imprisonment outcomes were assessed using administrative records. Cumulative imprisonment risk and relative risk of i…

Assessing the clinical relevance of point-of-care ultrasound for hospitalists: Influence on clinical reasoning and decision-making.

INTRODUCTION: Point-of-care ultrasonography (POCUS) is increasingly recognized in internal medicine, yet its influence on hospitalists' clinical reasoning remains underexplored. OBJECTIVES: This study aimed to evaluate the influence of POCUS on diagnostic hypotheses, management strategies, and confidence levels among hospitalists in a transitional care unit with limited access to advanced imaging. METHODS: Prospective descriptive study in a transitional care unit enrolling 19 hospitalists. During routine care, clinicians used POCUS at their discretion and, immediately afterward, completed a …

The Effect of Hybrid Cognitive Behavioural Therapy-Based and Peer Support Programme on Depression Among Transgender Women.

Transgender women (TGWs) experience high rates of depression due to stigma, discrimination and limited access to inclusive healthcare. This study aimed to examine the effect of a hybrid Cognitive Behavioural Therapy (CBT)-based and peer support programme on depression among TGWs and to examine whether intervention effects differed by transitional process (gender-affirming surgery [GAS] vs. gender-affirming hormone therapy [GAHT]). A randomized controlled trial with repeated measures ANCOVA was conducted. Participants were assigned to either an experimental group receiving the hybrid programme

Evaluating Transition Readiness in Older Adolescents and Young Adults With Type 1 Diabetes.

For older adolescents and young adults (AYAs) with type 1 diabetes, successful transition from pediatric to adult diabetes care requires ongoing planning and support. Yet, the transition to adult care is not always smooth. Some AYAs struggle to leave pediatric care or experience significant gaps between pediatric and adult diabetes care. The use of diabetes-specific transition readiness assessments can inform transition planning and support successful preparation for adult care. This study evaluated transition readiness in a diverse sample of AYAs nearing transition to adult diabetes care. Fin

Facilitating exit from encampments: combining low-barrier transitional housing with stabilizing treatment for substance related problems.

BACKGROUND: Tent encampments in the neighborhood surrounding Boston Medical Center (BMC) grew to include 336 individuals at points between 2019 and 21, prompting public health concerns. BMC, the City of Boston, and Commonwealth of Massachusetts partnered in 2/2022 to offer low-barrier transitional housing to encampment residents and provide co-located clinical stabilization services for community members with substance use disorders (SUDs) experiencing homelessness. METHODS: To meet the needs of some of the people who had been living in encampments, BMC established in a former hotel: 60 beds
