No Differences Between White and Non-White Patients in Terms of Care Quality Metrics, Complications, and Death After Hip Fracture Surgery When Standardized Care Pathways Are Used.

BACKGROUND: Many initiatives by medical and public health communities at the national, state, and institutional level have been centered around understanding and analyzing critical determinants of population health with the goal of equitable and nondisparate care. In orthopaedic traumatology, several studies have demonstrated that race and socioeconomic status are associated with differences in care delivery and outcomes of patients with hip fractures. However, studies assessing the effectiveness of methods to address disparities in care delivery, quality metrics, and complications after hip f

Medical reports recommending work restrictions at a teaching hospital in Brazil.

This non-experimental and cross-sectional descriptive study aimed to evaluate medical reports recommending work restrictions for workers at a teaching hospital in Brazil. A form was used for data collection, characterizing the medical reports and work restrictions, with its content previously validated. A total of 176 medical reports was analyzed, containing 337 work restrictions, recommended from January 2001 to January 2004. The results demonstrated a high rate of work restrictions in the study hospital. The most restrictions frequent were ergonomic (78.6%), and the most often prescribed wer

A systematic review of post-traumatic growth in ambulance personnel: facilitators and prevalence rates.

INTRODUCTION: Ambulance personnel are exposed to traumatic and stressful situations, which can increase the risk of mental health conditions, such as post-traumatic stress disorder (PTSD). High rates of PTSD have been found in ambulance personnel (Petrie et al., 2018), but no review is available to examine post-traumatic growth (PTG - positive psychological change following a trauma) in this population. This literature review provides an overview of the prevalence rates and facilitators that may contribute to PTG in ambulance personnel. METHODS: A systematic search was conducted on EBSCOhost

COVID-19 and First Responder Social Workers: An Unexpected Mental Health Storm.

COVID-19 has disproportionally affected people living in poverty; new immigrants; and those living in healthcare settings (hospitals, clinics, and nursing homes), shelters, detention centers, and prisons. The pandemic has also significantly impacted social workers who face hidden mental health consequences. The resultant feelings and behaviors are similar to disaster-induced trauma, including excessive rumination, hypervigilance, exhaustion, and excessive crying as a result of the adrenaline-driven fight-or-flight response. This article aims to provide information for social workers to protect

Permanent Supportive Housing Design Characteristics Associated with the Mental Health of Formerly Homeless Adults in the U.S. and Canada: An Integrative Review.

The built environment directly and indirectly affects mental health, especially for people transitioning from long-term homelessness to permanent supportive housing (PSH) who often experience co-occurring behavioral health challenges. Despite a rapid increase in PSH availability, little research examines influences of architecture and design within this context. This integrative review synthesized limited research on PSH design in the U.S. and Canada to identify built environment characteristics associated with PSH residents' mental health, highlight gaps in the literature, and prioritize futu

Analysis of the Effect of Intraocular Lens Implantation on Retinal Nerve and Postoperative Nursing Outcome Based on Ultrasound Imaging.

Traumatic cataract is a common complication of ocular contusion. Caused by various forms of trauma, the course and prognosis of traumatic cataract are also different. Intraocular lens (IOL) implantation is the most effective treatment at the present time and can provide good visual quality and near, medium, and far vision. In this study, an adaptive medical ultrasound image denoising algorithm was constructed based on the biological vision mechanism and was applied to observe IOL implantation and postoperative care of 30 patients with traumatic cataract. Results showed that eyesight could be r

Inflammatory neonatal pain disrupts maternal behavior and subsequent fear conditioning in a rodent model.

Infants spending extended time in the neonatal intensive care unit are at greater risk of developing a variety of mental health problems later in life, possibly due to exposure to painful/stressful events. We used a rodent model of inflammatory neonatal pain to explore effects on fear conditioning, somatosensory function and maternal behavior. Hindpaw injections of 2% λ-carrageenan on postnatal days 1 and 4 produced an attenuation in conditioned freezing during the postweaning period, similar to our previous work with acute pain, but did not cause lasting impacts on contextual freezing nor som

The role of percutaneous US-guided drainage in the treatment of splenic abscess. Case report and review of the literature.

Splenic abscess is an uncommon but potentially life-threatening disease that generally occurs in patients with neoplasia, immunodeficiency, hemoglobinopathies, trauma, metastatic infection, splenic infarction and diabetes. Splenic abscess should be considered in a patient with fever, left upper abdominal pain, and leukocytosis. Splenectomy has been the gold standard treatment for splenic abscess, however, burdened by high morbidity rate related clinical conditions of the patient. With the recent development of minimally invasive techniques and percutaneous US- or CT-guided procedures, the plac

Borderline personality disorder across the lifespan.

Borderline personality disorder is characterized by affective dysregulation, intense, unstable interpersonal relationships, impulsivity and unstable identity. It overlaps considerably with both PTSD and bipolar spectrum disorders. Research on true late-life BPD is limited, but suggests that some of the core features of BPD including interpersonal difficulties, unstable affect and anger remain relatively unchanged, while impulsivity and identity disturbance decline or change their mode of expression in late life. Diagnosis of BPD in late life requires both flexible application of the standard d

Navigating Postacute Care Pathways Following Hospital Discharge in Plastic Surgery.

Discharge destination following hospitalization plays a critical role in surgical recovery, long-term outcomes, and healthcare resource use. Although postacute care facilities are increasingly used, there is limited literature within plastic surgery addressing their role and implications. This review outlined the continuum of discharge options available to plastic surgery patients, including long-term acute care hospitals, inpatient rehabilitation facilities, skilled nursing facilities, assisted living facilities, postoperative guest suites, and home with or without home health care. Each dest

Identifying Avoidable Emergency Department Visits and the Potential Role of Community Health Centers in Northern Italy: A Retrospective Observational Study.

BACKGROUND: Emergency departments (EDs) are experiencing a growing burden of non-urgent presentations, which frequently signal deficiencies in access to primary and community-based healthcare. Evidence suggests that a substantial proportion of ED visits could be safely and effectively managed within community care settings. The general aim of the study was to identify which ED visits could have been managed within primary and community care services in a scenario of full implementation of community health centers. METHODS: A retrospective observational study was conducted. This study examined

Factors associated with discharge to home after traumatic rib fractures.

BACKGROUND: Chest wall injury is common among trauma patients. Generally, patients with more medical problems tend to have worse outcomes with rib fractures. Our aim was to determine if surgical stabilization of rib fractures (SSRF) increases the likelihood of discharge to home. METHODS: We performed a retrospective cohort study of patients with ≥1 comorbidity, aged 18-99 with rib fractures from five Chest Wall Injury Society (CWIS) Collaborative Centers. Discharge disposition of patients who underwent SSRF was compared to those who did not undergo surgery. Discharge to home was considered th

Evaluating the Accuracy of Point-of-Care Ultrasound for Peripheral Intravenous Cannulation in Emergency and Trauma Patients: A Systematic Review.

This systematic review evaluates the accuracy and effectiveness of point-of-care ultrasound (POCUS) for peripheral intravenous (PIV) line placement in trauma and emergency department patients. PIV access can be challenging in patients with difficult vascular access (DIVA), often leading to delays in care. POCUS has emerged as a promising modality to improve success rates and reduce complications in such scenarios. A comprehensive literature search was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines across PubMed, EMBASE, S

Prevalence of Childhood Maltreatment and Posttraumatic Stress Symptoms Among Male Inmates in Iceland.

INTRODUCTION: Childhood maltreatment is highly correlated with adult criminal behavior, and the prevalence of childhood maltreatment within prison populations is high. Furthermore, studies have repeatedly shown a higher prevalence of posttraumatic stress disorder (PTSD) symptoms in samples of prisoners compared with the general public. PURPOSE AND METHODOLOGY: The aims of the current study were to determine the frequency of childhood maltreatment among male inmates in Iceland and the percentage of male inmates meeting screening criteria for PTSD. The Icelandic versions of the Childhood Trauma

From post-traumatic stress disorder to absolute dependence in an intensive care unit: reflections on a clinical account.

This paper tells the story of one man's experience of terrifying hallucinations and nightmares in an intensive care unit (ICU), drawing attention to the reality that intensive care treatment induces emotional suffering severe enough to be identified as post-traumatic stress disorder (PTSD). A body of international research, confirmed by South African studies, links life-saving critical care to symptoms which qualify for secondary psychiatric diagnosis including of post-traumatic stress. Risk factors include pre-ICU comorbid psychopathology. Early on in the clinical encounter with the patient i

Management of Tissue Loss After Agkistrodon Snakebite: Appropriate Use of Crotalidae-Fab Antivenin.

Although initially created for the treatment of rattlesnake (genus: Crotalus) bites, Crotalidae-Fab antivenin is used to treat many different pit viper envenomations. However, the efficacy of Crotalidae-Fab in preventing tissue loss from copperhead (Agkistrodon contortrix) or cottonmouth (Agkistrodon piscivorus) snakebites remains unclear. Recent reports show that Agkistrodon-related bites rarely require treatment beyond simple observation and pain control. The purpose of this study was to examine the amount of tissue loss in patients who received Crotalidae-Fab compared with those who did not
