The trigger finger release was performed in 34 digits (11 thumbs and 23 fingers) of 24 patients through the thread transecting technique with the tip-to-tip approach, in which a 22-gauge needle inserts into a 18-gauge needle when both needles are inside the hand, guiding the 22-gauge needle to exit the hand at the same access point of 18-gauge needle. We prospectively evaluated the effectiveness and functional recovery of these patients. In all 34 digits, triggering and locking were resolved, and complete extension and flexion occurred immediately following the release. There were no complicat

Understanding the basis of brain function requires knowledge of cortical operations over wide-spatial scales, but also within the context of single neurons. In vivo, wide-field GCaMP imaging and sub-cortical/cortical cellular electrophysiology were used in mice to investigate relationships between spontaneous single neuron spiking and mesoscopic cortical activity. We make use of a rich set of cortical activity motifs that are present in spontaneous activity in anesthetized and awake animals. A mesoscale spike-triggered averaging procedure allowed the identification of motifs that are preferent

Anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis is a complex neuroinflammatory condition often triggered by underlying factors such as viral infections or malignancies, although idiopathic cases are not uncommon. Here, we present a unique case of a 27-year-old female patient diagnosed with anti-NMDAR encephalitis, where Lyme disease emerges as a plausible yet underrecognized trigger. The patient initially presented to the emergency department (ED) with acute psychosis and was later found to have positive Borrelia burgdorferi immunoglobulin G (IgG) titers, antibodies against the NR

Vancomycin is the first-line drug for infections of methicillin-resistant Staphylococcus aureus (MRSA) and multi-drug-resistant bacteria. The effective therapeutic concentration range of vancomycin is narrow, so it's essential to implement vancomycin therapeutic drug monitoring. However, conventional detection methods have disadvantages of expensive equipment, complicated operation, or poor reproducibility. Herein, a fluorescent sensing platform initiated by an allosteric probe was constructed for simple and sensitive monitoring of vancomycin at a low cost. The key point of this platform is th

BACKGROUND: Tissue factor (TF) is the most relevant physiological trigger of thrombosis contributing to the presentation of clinical ischemic events after plaque rupture. However, the role of human vascular smooth muscle cell (HVSMC) TF in vascular remodeling, restenosis and atherosclerosis is less known. We have hypothesized that TF contributes to atherosclerotic lesion formation, triggering smooth muscle cell migration through a specific yet unknown signaling pathway.
OBJECTIVES: The aim of this study has been to investigate the signal transduction mechanism by which TF may contribute to th

Side-locked headache is commonly regarded as a warning sign for secondary intracranial pathology; however, its musculoskeletal causes may be underrecognized. We report a series of seven patients with strictly unilateral headache consistently and temporally associated with the habitual clamping of a mobile phone between the neck and shoulder during hands-free multitasking. The headache was nonthrobbing, lacked migrainous or cranial autonomic features, and was reproducible with cervical palpation or sustained neck movements. Neurological examination and neuroimaging were normal in all cases. Cer

The STING pathway is pivotal in defense against RNA viruses; however, its involvement in swine acute diarrhea syndrome coronavirus (SADS-CoV) infection remains unclear. This study reveals a dual mechanism where in SADS-CoV triggers STING activation via nuclear envelope rupture but subsequently evades immunity through its nucleocapsid (N) protein. Mechanistically, this process involves chromatin leakage that activates the cGAS-STING pathway, triggering interferon responses. The endoplasmic reticulum-resident protein defender against cell death 1 (DAD1) plays a key role in promoting STING phosph

Plants activate pattern-triggered immunity (PTI) and effector-triggered immunity (ETI) to combat pathogens. However, how these systems coordinate immune activation while preventing autoimmunity remains poorly understood. In this study, we uncovered a regulatory mechanism in which surface immune signaling unlocks nucleotide-binding leucine-rich repeat (NLR) immune receptor activation through mRNA splicing. We identified an N-terminal prodomain in the potato late blight resistance protein Rpi-vnt1.1 that inhibits resistosome formation, preventing potential autoactivation of this NLR. Upon pathog

Mitochondria are the main source of energy for cellular activity. Their functional damage or deficiency leads to cellular deterioration, which in turn triggers autophagic reactions. Taking mitochondrial autophagy as a starting point, the present review explored the mechanisms of duodenal abnormalities in detail, including mucosal barrier damage, release of inflammatory factors, and disruption of intracellular signal transduction. We summarized the key roles of mitochondrial autophagy in the abnormal development of the duodenum and examined the in-depth physiological and pathological mechanisms

Atypical/complement-mediated hemolytic uremic syndrome (A-HUS/CM-HUS) is a hereditary or sporadic disease with thrombotic microangiopathy (TMA). Diarrhea is a trigger that can cause attacks of CM-HUS. Although there are opinions that complement system activation plays a role in intestinal inflammation in patients with inflammatory bowel disease, the association of TMA with inflammatory bowel disease (IBD) has rarely been reported. In our case, a CM-HUS case that developed without an additional triggering factor in the course of ulcerative colitis (UC) was successfully treated with eculizumab,

We present here the integration of brain-computer interfacing (BCI) technology with functional electrical stimulation therapy to restore voluntary function. The system was tested with a single man with chronic (6 years) severe left hemiplegia resulting from a stroke. The BCI, implemented as a simple "brain-switch" activated by power decreases in the 18 Hz - 28 Hz frequency range of the participant's electroencephalograpic signals, triggered a neuroprosthesis designed to facilitate forward reaching, reaching to the mouth, and lateral reaching movements. After 40 90-minute sessions in which the

Modern anaesthetic techniques have resulted in the clinical presentation of malignant hyperthermia to be more often indolent and/or insidious than truly fulminant, as previously known in the anaesthetic community. We present four recently referred cases to illustrate this point: one late-onset case, two patients with slowly progressive hypercapnia as the sole sign and a fourth patient with postoperative myalgias and elevated creatine kinase. We also discuss the reasons for the shift in typical clinical presentation. The more insidious character of malignant hyperthermia is most likely due to t

INTRODUCTION: Adult intussusception is rare and typically associated with an underlying lead point. Inflammatory pseudotumor, a rare benign lesion of uncertain etiology, can serve as such a lead point in the small bowel. The occurrence of double intussusception in this context is exceptionally uncommon.
PRESENTATION OF CASE: A 55-year-old woman presented with signs of small bowel obstruction. Imaging(Fig. 1-11) revealed ileo-ileal intussusception. Intraoperatively(Fig. 16-18), a Double intussusception configuration was observed where one intussuscepted segment acted as the lead point for a se

BACKGROUND: Women with stress urinary incontinence (SUI) often present with very weak pelvic floor muscles (PFM); this deficit reduces the effectiveness of pelvic floor muscle training (PFMT).
AIM: To determine whether adding sEMG-triggered intravaginal electrical stimulation to supervised PFMT improves PFM strength and continence outcomes compared with PFMT plus sham stimulation in women with SUI and very weak PFMs.
METHODS: In this multicentre, randomized, double-blind, parallel-group trial, 42 women with SUI were allocated to: (1) PFMT + sEMG-triggered electrical stimulation, or (2) PFMT

BACKGROUND & AIMS: Autoimmune liver disease (AILD) is thought to result from a complex interplay between genetics and the environment. Studies to date have focussed on primary biliary cholangitis (PBC) and demonstrated higher disease prevalence in more urban, polluted, and socially deprived areas. This study utilises a large cohort of patients with PBC and primary sclerosing cholangitis (PSC) to investigate potential environmental contributors to disease and to explore whether the geo-epidemiology of PBC and PSC are disease-specific or pertain to cholestatic AILD in general.
METHODS: All adul

This voluminous nucleus extends from the upper part of the medulla oblongata to the 4th cervical segment of the spinal cord. This topography puts it in relation with numerous other spinal and cerebral centers = ventral and dorsal horns of the spinal cord, including spinal nucleus of the accessory nerve, nucleus solitarius, motor dorsal nucleus of the vagus nerve, reticular formation, etc... Even if the longitudinal aspect of this nucleus is not uniform, it must be pointed out that the three branches of the trigeminal nerve are represented along this course through the trigeminal spinal tract.

Thyroid pathology is quite common worldwide. Thyroid storm is a severe form of hyperthyroidism, particularly in Graves' disease, which is rare. It is in association with various triggering factors (therapy discontinuation, infections, surgical interventions) and can lead to multiorgan dysfunction, particularly liver and cardiovascular injury. Prompt diagnosis and early therapy are fundamental to solving the thyroid storm and its complications. Multiple therapies are needed for adjustment according to the particular case, including the use of synthetic antithyroid drugs, beta-blockers, corticos

The work aimed to explore the clinical hypothesis on the possible alternative to Opiorphin and its stable analogues for treating fibromyalgia pain. Fibromyalgia is a condition characterized by chronic pain triggered by an interplay of biological and psychosocial variables, although the exact pathogenesis is still controversial. Standard therapy for low threshold tender point pain includes NSAIDs and opioid analgesics, both of which have serious adverse profiles after long-term exposure, highlighting the need for an intermediate compound capable of bridging the gap between NSAIDs and opioid ana

Malignant hyperthermia (MH) is a rare clinical syndrome, triggered in susceptible subjects by a variety of anaesthetic agents and muscle relaxants, and is the commonest cause of death due to general anaesthesia. Previous studies have reported that inherited mutations in the ryanodine receptor (RYR1) gene co-segregated, in some families, with MH susceptibility; lack of linkage between MH and the RYR1 gene in some other families indicates a heterogenous genetic basis for the syndrome. The in vitro contracture test (IVCT) on muscle biopsy specimens is considered to be the most reliable test for e
