INTRAMEDULLARY TUBERCULOMA OF THORACIC SPINAL CORD IN AN APPARENTLY HEALTHY NIGERIAN: CASE REPORT.

Spinal intramedullary is an uncommon form of tuberculosis causing spinal cord injury in this environment. We report a case of thoracic intramedullary tuberculoma in an immunocompetent male Nigerian with negative screening for tuberculosis. He presented with 8 months history of back pain and 2 months history of progressive weakness in both lower limbs. Physical examination revealed a well-nourished man with spastic paraplegia. Chest radiograph and CT scan were normal but Magnetic resonance imaging (MRI) of the thoracolumbar region showed cord oedema and circumscribed intramedullary lesions at D

Tuberculous myelitis: a systematic review of published case reports and case series.

STUDY DESIGN: A systematic review. OBJECTIVES: Tuberculous myelitis, an uncommon disorder, often manifests as transverse myelopathy. The majority of the literature comprises isolated case reports, necessitating a systematic review for better understanding and management. SETTING: Uttar Pradesh India. METHODS: Our review followed PRISMA guidelines, searching PubMed, Scopus, Embase, and Google Scholar with no language constraints. Quality assessment of reports was based on selection, ascertainment, causality, and reporting. Data synthesis was qualitative with categorical and continuous data r

Cutaneous tuberculosis as a manifestation of Pott's Disease: A diagnostic challenge in patients with Non-HIV immunosuppression.

Tuberculosis (TB) is one of the most important public health issues in developing countries. The World Health Organization estimates that approximately 20%-40% of the world's population is infected. Pulmonary forms account for the majority of cases; however, it can manifest as extrapulmonary disease in 8.4%-13.7% of cases. Of these extrapulmonary forms of TB, only 1%-2% may have skin manifestations. Cutaneous tuberculosis (CTB) is relatively uncommon and is not a well-defined disease, which complicates diagnosis. We present two patients with Pott's disease that manifested as CTB, one with tube

Transpedicular fixation in the treatment of various spinal disorders.

36 patients with 157 inserted pedicular screws are presented. The patients are evaluated in 5 different groups; group I: fractures, group II: spinal tuberculosis, group III: spinal tumours, group IV: spondylolisthesis and group V: scoliosis. The mean age and the mean follow-up of the groups are respectively 42 years and 10.8 months. Cotrel-Dubousset instrumentation (CD), Alici Instrumentation, Fixateur Interne (Dick) or Steffee plate are used to stabilize the spine. The low complication rate of pedicular screw insertion is related to the used technique.

Infectious agents in spinal epidural abscesses.

Of 27 cases of spinal epidural abscess, 19 were caused by bacteria, 7 by Mycobacterium tuberculosis, and 1 by Echinococcus granulosus. Blunt trauma and cutaneous infections were the most frequent preceding events in bacterial cases. Tuberculous abscess was usually the sole manifestation of reactivation of dormant tuberculosis. Drug addiction, the most common cause in young adults, was associated with gram-negative infections. Whatever the infectious agent, paraparesis for longer than 4 days led to a poor outcome. Myelography was the best diagnostic test, whereas lumbar puncture and percutaneou

Multilevel vertebral body tuberculosis: a case report.

We report a rare case of multifocal vertebral body tuberculosis with an isolated affection of vertebral bodies and uncommon findings in radiographies that were more suggestive of vascular or tumor lesion. This case is reported to illustrate the complexity of diagnostic procedures necessary to reveal multilevel vertebral body tuberculosis and the place of modern imaging, especially computed tomography and magnetic resonance imaging, to differentiate vertebral tuberculosis from other differential diagnosis.

Computed tomography-guided percutaneous transpedicular biopsy of the thoracic spine.

BACKGROUND: Despite the recent, widespread use around the world of computed tomography (CT)-guided spinal biopsy for presurgical evaluation, its use has remained uncommon in Taiwan. Using 9 patients, we attempted to evaluate the safety of its use for the thoracic spine biopsy. METHODS: In the past 3 years (May 1996 to May 1999), 9 patients with known thoracic or multiple spinal lesions underwent CT-guided biopsy. The patients comprised 4 men and 5 women; with ages ranging between 28 and 85 years. For the transpedicular approach, patients were laid prone on the CT table. A trephine bone biopsy

Tuberculosis of the neural arch. A report of four cases.

Four cases of spinal tuberculosis involving the posterior neural elements are reported; all the patients were from Africa. The condition is rare and its incidence may be different in different races. Neural arch involvement is likely to be associated with neurological complications.

A rare case of intramedullary tuberculoma: Complete resolution after medical treatment and role of magnetic resonance imaging in diagnosis and follow-up.

Intramedullary tuberculoma (IMT) is a rare form of spinal tuberculosis. IMT has an incidence of 2 cases per 2000 cases of central nervous system tuberculosis. Magnetic resonance imaging (MRI) can be helpful for diagnosing IMT at an early stage and it is also very useful in follow-up. Three stages of IMT have been described in MRI based on the evolution of the lesion. Medical therapy is the mainstay of treatment for IMT though there are surgical options for select patients. Here, we describe a patient with dorsal IMT who improved clinically as well as radiologically with antituberculous treatme

Intradural extramedullary tuberculoma of the thoracic spine: paradoxical response to antituberculous therapy.

Intradural extramedullary tuberculoma of the spinal cord (IETSC) is a rare complication of tuberculosis, which can occur as a paradoxical response to antituberculous therapy. A 46-year-old woman with tuberculosis meningitis developed an acute sensory disturbance and paraplegia eight weeks after the antituberculous treatment was started. MRI revealed a cystic lesion at the Th 2 and 3 vertebrae levels, and continuous dural thickening. Laminectomy was performed; soft granulomas were unexpectedly observed inside the dura matter. After the operation, the patient experienced progressive improvement

Spondylodiscitis associated with recurrent E. coli bacteraemia in an elderly patient: a case report highlighting uncommon pathogen presentation.

Spinal infections are defined as infections within the vertebral column, intervertebral disc space, spinal canal, and surrounding soft tissues. Mycobacterium tuberculosis is the most prevalent pathogen worldwide, and Staphylococcus aureus accounts for more than half of European cases. However, Escherichia coli is a rare organism causing spondylodiscitis. Early diagnosis is vital for the effective management of spinal infections, as it helps prevent mortality and significant morbidity. We present a case of spondylodiscitis caused by E. coli bacteraemia, an atypical pathogen in this setting. The

[Acute toxic encephalopathy in patients with tuberculosis].

A hundred and thirty five patients admitted to Moscow Tuberculosis Hospital No. 7 for disseminated and progressive forms of tuberculosis were examined. Among neurological disorders in tuberculosis, acute toxic encephalopathy (ATE) should be placed in the first place in terms of their severity, problems of diagnosis and treatment. In patients with acutely progressive forms of tuberculosis, the development of ATE is brought about by two factors: 1) significant tuberculous toxemia concurrent, in 37% of cases, with severe alcoholic intoxication that leads to generalized toxic and allergic vasculit
