OBJECTIVE: The purpose of this article is to illustrate the spectrum of sonographic findings in perforated pyloroduodenal peptic ulcer and discuss the potential role of sonography in the diagnosis.
CONCLUSION: Although sonography is not the first-line investigation of choice in suspected perforated peptic ulcer, understanding of the characteristic appearances seen during general abdominal sonography may aid the reader in the diagnosis of this important and sometimes overlooked cause of nonspecific abdominal pain. This may shorten time to the diagnosis and ultimate surgical management.

MATERIAL AND METHODS: Our study includes 4 cases of patients with multiple trauma, admitted in the "St. Pantelimon" Emergency Hospital, where, despite medication, they subsequently developed stress ulcer (Cushing ulcer). Laboratory tests were followed in the development the level of leuko…

INTRODUCTION: Previously published UK data on HIV transmitted drug resistance (TDR) shows that it ranges between 3 and 9.4% [1,2]. However, there are no recent data from populations where HIV transmission rates are increasing. The aim of this study was to assess the prevalence of TDR in untreated HIV-infected individuals attending three HIV specialist clinics under the HIV Directorate, Chelsea and Westminster Hospital and based throughout London - the Kobler Clinic, 56 Dean Street and West London Centre for Sexual Health.
METHODS: We included all patients with a HIV diagnosis, no history of a

We report the case of a 30-year-old woman presenting with a single, deep, asymptomatic frontal ulcer that had developed in the previous two months without preceding trauma. Blood tests revealed reactive Rapid Plasma Reagin and Treponema pallidum hemagglutination assays. The diagnosis of syphilitic gumma was confirmed after histological analysis of the ulcer border. No other manifestations of late syphilis were detected. Ulcer healing was observed after treatment with benzathine penicillin and there was progressive reduction of follow-up serum RPR titers. The relevance of this case arises from …

Treatment data of 1279 patients with gastroduodenal ulcer bleeding are analyzed. Outcomes are comparatively analyzed depending on the chosen treatment modality. Optimal treatment algorithm is worked out. The expediency of conservative ulcer bleeding treatment, including primary and repeated endoscopic hemostasis, is proved. It allows performing deferred planned reconstructive operations, thus decreasing the operative risk substantially. Indications for the operative treatment of ulcer bleeding are specified, as well as efficacy and safety of organ-saving methods is proved.

A chronic diabetic foot ulcer requires a search for the etiology. The three main causes to search for are poor off-loading compliance, osteomyelitis, and peripheral vascular disease. The level of severity is measured with the U.T. classification and the level of infection with the classification of the International Consensus on the Diabetic Foot. Peripheral vascular disease must be precisely evaluated by Doppler ultrasound, which describes all the arteries of the lower limb. Angiography is required only in case of revascularization. Treatment of the ulcer includes strict off-loading, topical …

Marjolin's ulcers are uncommon malignancies arising from previously traumatized, chronically inflamed or scarred skin. They are usually squamous cell carcinomas and arise most often after burns, and they may present decades after the original insult. Metastases are observed more often than in other cutaneous squamous cell carcinomas. Marjolin's ulcers may occur on stumps in patients using prostheses, in this situation the diagnosis may be delayed because of the false assumption that the ulcer is caused by an ill fitting prosthesis. A case of Marjolin's ulcer is presented occurring on a lower l…
