[Urinary catheter biofilm infections].

Urinary tract infections, most of which are biofilm infections in catheterized patients, account for more than 40% of hospital infections. Bacterial colonization of the urinary tract and catheters causes not only infection but also other complications such as catheter blockage by bacterial encrustation, urolithiasis and pyelonephritis. About 50% of long-term catheterized patients face urinary flow obstruction due to catheter encrustation, but no measure is currently available to prevent it. Encrustation has been known either to result from metabolic dysfunction or to be of microbial origin, wi…

[Methodologic comparison of the analysis of urinary calculi. Roentgen diffractometry, polarization microscopy and microscopic-microchemical component analysis].

Not only instrumental, apparative or operative stone extraction is necessary for patients with urolithiasis (particularly those with recurrent urolithiasis); preventive treatment is also called for, and for this exact analysis of the composition of the stones is essential. The procedures currently accepted as standard for stone analysis are: X-ray diffraction, infrared spectroscopy, polarisation microscopy and the complicated chemico-quantitative analyses performed in some institutes. Recently a new, so-called microscopic-microchemical, analysis of stone components has been presented (the Harz…

The association of idiopathic hypercalciuria and asymptomatic gross hematuria in children.

Seven children with asymptomatic gross hematuria are described. Six had recurrent hematuria; one had a single episode. Occasional global glomerulosclerosis and/or mesangial electron dense deposits were present in the three patients in whom renal biopsy was performed; the changes were felt to be insufficient to account for the hematuria. None of the patients had urolithiasis or any significant urinary tract abnormality. One was an adopted child; a family history of urolithiasis was obtained in the other six. Idiopathic hypercalciuria was documented in six patients; the seventh subsequently pass…

[Spontaneous fistulae and abscesses of the upper urinary tract (author's transl)].

Spontaneous cutaneous fistulae and abscesses of the renal pelvis and ureter have become rare. Six case reports demonstrate their etiology and differential diagnostic problems. If there is no underlying urologic disease, above all no urolithiasis, other causes of fistulae and abscesses should be kept in mind, the most frequent of these being Crohn's disease.

Antiurolithic efficacy of a phenolic rich ethyl acetate fraction of the aerial parts of Aerva lanata (Linn) Juss. ex Schult. in ethylene glycol induced urolithic rats.

OBJECTIVES: The study was carried out to evaluate the in vivo antiurolithic efficaciousness of an ethyl acetate fraction of Aerva lanata (EAFAL) derived from the hydromethanolic extract of its aerial parts (HMEAL). METHODS: In vivo pharmacological potency of EAFAL was assessed by ethylene glycol (EG) induced urolithiasis model in male Wistar albino rats. Urine samples of the animals were analysed for physical parameters, stone promoters, inhibitors along with an evaluation of the biochemical parameters of serum and kidneys. Histopathological investigation of the kidneys was done. The fraction…

Mucosal folding in upper urinary pathways following ureterolithiasis.

Mucosal folds in the ureter or renal pelvis were demonstrated in 2 children and 27 adults with urolithiasis. It appeared from the sequence of events observed in these cases that the folds occurred in a redundant mucosa following an episode of mural stretching. Urinalysis, including bacterial culture in the majority of cases, showed infection in only 3 of the adults.

Idiopathic recurrent calcium urolithiasis (IRCU): variation of fasting urinary protein is a window to pathophysiology or simple consequence of renal stones in situ? A tripartite study in male patients providing insight into oxidative metabolism as possible driving force towards alteration of urine composition, calcium salt crystallization and stone formation.

BACKGROUND: In IRCU it is uncertain whether variation of urinary protein, especially non-albumin protein (N-Alb-P), is due to the presence of stones or reflects alteration of oxidative metabolism. AIMS: To validate in a tripartite cross-sectional study of 187 ambulatory male patients, undergoing a standardized laboratory programme, whether stones impact on N-Alb-P or the state of oxidative metabolism interferes with IRCU pathophysiology. METHODS: In part 1 the strata low and high of fasting urinary excretion rate per 2 h of N-Alb-P, malonedialdehyde, hypoxanthine, xanthine, pH and other urin

[The urological indication. Thoughts on the indication for diagnostic and operative measures in urology: (author's transl)].

Taking as examples the three most frequent diseases occurring in hospital and private practice, urolithiasis, prostatic hypertrophy and space-occupying processes of the kidney, the indications and contraindications of diagnostic and therapeutic measures are pointed out. It is found that even in future only a careful history and clinical findings lead to logical and rational use of technical possibilities.

Donor kidney lithiasis and back-table endoscopy: a successful combination.

INTRODUCTION: Urolithiasis in renal allografts is relatively rare with an incidence of 0.17-4.40%. It is nonetheless an important issue, as there is a risk of obstruction, sepsis and even loss of the renal allograft. The management of stones in renal allografts remains challenging because of the anatomy, the renal denervation and the use of immunosuppressive medication. CASE PRESENTATION: This report discusses the ex-vivo treatment of asymptomatic nephrolithiasis in a living donor kidney allograft. A CT abdomen revealed a lower pole stone (5.9 × 5.5 × 5.0 mm; 920 HU) in the right kidney of th…

Oxalates, urinary stones and risk of cardiovascular diseases.

Increased level of oxalates in urine and plasma can be attributed to endogenous overproduction, increased ingestion or excessive intestinal absorption. When a supersaturation status is reached, oxalates combine with calcium and crystallize to form 80% of the urinary stones. Several cardiovascular diseases such as coronary heart disease and stroke are thought to be associated with the formation of urinary stones via sharing the same pathogenesis and/or risk factors. This review investigated the evidence linking oxalates/urinary stones to cardiovascular diseases. Eventually, two theories can exp

[Retrograde intrarenal surgery: a modern view of the problem].

Retrograde intrarenal surgery (RIRS) is a rapidly developing field of endoscopic management of urolithiasis. Previously considered an additional technique in patients with lower calyx stones that do not respond to extracorporeal shock wave lithotripsy, now the RIRS along with percutaneous surgery is gaining a leading position in the management of kidney stones. Currently, it is recommended as the main surgical modality for treating stones less than 2 cm, especially in cases where extracorporeal shock wave lithotripsy is not feasible or not desirable. Retrograde intrarenal surgery has no specif…

Single use and disposable flexible ureteroscopes.

PURPOSE OF REVIEW: Flexible ureteroscopy has been widely developed in the last decade been nowadays one of the first line treatment options for urolithiasis and conservative management for upper tract urothelial tumors. Even though some unfavorable features of the reusable flexible scopes can limit the surgical procedure such as their availability, lack of sterilization or costly repairs. Disposable scopes have been developed with the intention of improving such problems. RECENT FINDINGS: To our knowledge, seven devices have been developed. Of the scopes features, size, deflection, image qual…

A giant vesical calculus.

Massive or giant vesical calculus is a rare entity in the recent urological practice. Males are affected more than the females. Vesical calculi are usually secondary to bladder outlet obstruction. These patients present with recurrent urinary tract infection, haematuria or with retention of urine. We report a young male patient who presented with defaecatory problems along with other urinary symptoms. The patient having an average built, non diabetic but hypertensive. The stone could be palpated by physical examination. His urea levels were within normal limits but urine examination shows infe

The use of shock waves in medicine--a tool of the modern OR: an overview of basic physical principles, history and research.

Extracorporeal-generated shock waves were first used in medical therapy, to disintegrate kidney stones, approximately 20 years ago. Since this time, shock waves have changed the treatment of urolithiasis substantially and are now the first-choice treatment for kidney and ureteral stones. First clinical investigations of the shock-wave treatment of Induratio Penis Plastica (IPP) are showing promising results. Shock waves have also been used in orthopaedics and traumatology, to treat insertion tendinitis, non-unions or delayed unions, avascular necrosis of the head of femur and other necrotic bo…

Retrograde intrarenal surgery with or without ureteral access sheath: a systematic review and meta-analysis of randomized controlled trials.

INTRODUCTION: The ureteral access sheath (UAS) is a medical device that enables repeated entrance into the ureter and collecting system during retrograde intrarenal surgery (RIRS). Its impact on stone-free rates, ureteral injuries, operative time, and postoperative complications remains controversial. Therefore, we performed a systematic review and meta-analysis comparing RIRS with versus without UAS for urolithiasis management. PURPOSE: To compare outcomes from retrograde intrarenal surgery (RIRS) for stone extraction with or without ureteral access sheath (UAS); evaluating stone-free rate (…
