Vacuum assisted closure of sternal wounds is a sound strategy for patients with severe poststernotomy mediastinitis and sternal bone necrosis. As a bridging therapy to reconstructive surgery it provides an adequate temporary stabilization of the thorax. It enhances lung function and reduces pulmonary complications.

The aim of this study was to evaluate accuracy of 11 G vacuum-assisted percutaneous biopsy (VAPB) carried out on digital stereotaxic table, on breast non-palpable lesions (NPLs), non-visible by US. Prospective study on 132 consecutive NPLs (126 patients) not reliably found by US; 82% showed microcalcifications. Two-hundred and ninety-three patients without a previous vaginal delivery were randomized to intracervical/extra-amniotic application of 0.5 mg prostaglandin E2 (PGE2) or to gel only. Of the patients who received PGE2, 18.7% were admitted before the next morning due to spontaneous abortion, bleeding or pains. No other side-effect was observed. A statistically significant dilatation of the cervical canal was found in the prostaglandin group. A 70-year-old woman was treated with thoracic endovascular aortic repair for an enlarging descending thoracic aortic aneurysm. Intraprocedural angiography after stentgraft positioning revealed superior mesenteric and right renal artery occlusion due to parietal thrombus displacement. This unusual complication was promptly treated with thrombus aspiration, which avoided visceral ischaemia by utilizing a minimally invasive technique. Treatment success was confirmed with intraprocedural angiography and computerized tomography a few days postoperatively.
