During a period of four years, 386 patients with left sided varicoceles were treated by sclerotherapy of the left internal spermatic vein. This was successful in 93.8%. The most common complication was perforation of the vein in 6.2%. Technical success and complications depended on anatomical variations. Of particular significance was Bähren type IVb (competent main vein, incompetent collaterals) which occurred in 18.9%. This accounted for 66.5% of all perforations and 54.2% of unsuccessful interventions. The recurrence rate was 3.8%.

Varicocele appearing on the posterior surface of the scrotum is apt to be overlooked in a routine physical examination. Lateral inspection of the scrotum is important to detect a varicocele of this type. Intraoperative spermatic phlebography on high ligation for varicocele was valuable in confirming the ligation of a single vein or in visualizing the vessel(s) left unligated.

Ninety-seven patients received transfemoral sclerotherapy of the spermatic vein for varicocele. After observation for 2 to 5 years, follow-up was available in 69 patients (71.2%), in 11 of whom the procedure failed (16.0%). Complications during angiography (N = 12, 12.4%) or sclerotherapy (N = 31, 32.0%) did not require hospital treatment. In 3 patients with varicocele a pressure difference was found between the left renal vein and inferior vena cava. No pressure difference was found between the right renal vein and inferior vena cava in these same patients. In a control group of 5 patients right and left renal veins and inferior vena cava pressures closely approximated each other. These preliminary findings are discussed and found to be in disagreement with current concepts of the pathogenesis of varicocele.
