Conduction block in neuropathies with necrotizing vasculitis.

Thirty-two patients with a mononeuropathy multiplex associated with a systemic necrotizing vasculitis were studied. The main abnormality was a loss of motor and sensory axons confirmed by electrophysiological and histological methods. A conduction block was observed in five patients, but only one was at a usual site of compression. Based on previous pathological studies and the experimental data in human and animals, the mechanism of the block is proposed to be ischemic. It is suggested that a conduction block in one nerve in a neuropathy with two or more individual nerves affected and with el…

Cerebral vasculitis and hemorrhage associated with phenylpropanolamine.

A 20-year-old woman using an oral diet aid preparation containing phenylpropanolamine presented with an intracerebral hemorrhage and angiographic evidence of cerebral vasculitis. Gas chromatography demonstrated phenylpropanolamine in the urine. Even when used within recommended guidelines, phenylpropanolamine usage should be included in the differential diagnosis of patients presenting with intracranial hemorrhage.

Lymphocytic vasculitis: is it a specific clinicopathologic entity?

Seventy-one biopsy specimens taken at the Mayo Clinic from June 1977 through May 1981 demonstrated lymphocytic vasculitis. All specimens met the criteria for lymphocytic vasculitis, defined as (1) a predominantly lymphocytic infiltrate that involves and surrounds blood vessel walls, (2) fibrinoid necrosis of blood vessel walls, and (3) endothelial cell hyperplasia. Other histologic findings such as thrombosis, extravasation of erythrocytes, ulceration, epidermal infarction, and evidence of nuclear dust were seen only occasionally. Hypocomplementemia and other serologic abnormalities were very …

Rheumatoid vasculitis of the colon presenting as pancolitis.

This report describes a patient with pancolitis resulting from rheumatoid vasculitis. Corticosteroid treatment reduced colonic inflammation but colectomy was required to control hemorrhage. The case illustrates the need to recognize colitis as a manifestation of rheumatoid vasculitis so that new treatment modalities may be employed to achieve a more successful medical management. The appearance of discrete colonic ulcerations following corticosteroid treatment of pancolitis in patients with rheumatoid arthritis should further suggest the diagnosis of intestinal vasculitis.

[Nodular vasculitis in the current practice of phlebology].

In current phlebological practice nodular vasculitic lesions are described as being characterised by hypodermal perivascular inflammation. The aetiology is diverse: in this article hypodermatitis of varicose and postphlebitic origin, both of which are due to basic vein disorders and which are certainly the most often encountered phlebopathies in daily practice, are studied and then there is a further study of acute and sub-acute nodular hypodermatitis which in practice represents a differential diagnosis to the former but are less frequently encountered. Finally, other cases of hypodematitis d

Clinical and immunological aspects of cutaneous vasculitis.

Thirty-five patients with cutaneous vasculitis were studied. Sixteen had raised serum levels of cryoglobulins. In all but one the cryoglobulin was an immune complex. Eleven patients had anticomplementary sera and five had a low serum IgM. Clinically, the cryoglobulinaemic patients were not readily distinguishable from the non-cryoglobulinaemic in that cold sensitivity occurred in only six of sixteen patients with cryoglobulins. Peripheral neuropathy and lesions in the upper respiratory tract were confined to those with cryoglobulinaemia. An immunopathological classification for the purpuric va…

Long-term damage assessment in patients with microscopic polyangiitis and renal-limited vasculitis using the Vasculitis Damage Index.

OBJECTIVES: The Vasculitis Damage Index (VDI) is used to define the degree of damage occurring in patients with systemic vasculitis. We conducted a retrospective study of 30 patients with microscopic polyangiitis (MPA) and renal-limited vasculitis (RLV). METHODS: The clinical data and VDI of the 30 patients enrolled in the study were collected and assessed for a period of 5 years. RESULT: The VDI score, which was 2.5 at 1 year after the initial diagnosis, increased gradually to 4.3 at 5 years post-diagnosis. The degrees of musculoskeletal and ocular damage significantly increased during the …

Single-organ gallbladder vasculitis: characterization and distinction from systemic vasculitis involving the gallbladder. An analysis of 61 patients.

Systemic vasculitis (SV) involving abdominal structures usually has a poor prognosis. Gallbladder vasculitis (GV) has been reported as part of SV (GB-SV) and focal single-organ vasculitis (GB-SOV). We analyzed clinical and histologic characteristics of patients with GV to identify features that differentiate GB-SOV from the systemic forms of GV. To identify affected patients with GV we used pathology databases from our institution and an English-language PubMed search. Clinical manifestations, laboratory and histologic features, treatment administered, and outcomes were recorded. Patients were…

Understanding the burden of ANCA-associated vasculitis.

Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is a group of frequently relapsing systemic autoimmune disorders characterized by vasculitis-related organ damage and multiple comorbidities related to chronic inflammation. Advances in immunosuppression-based therapies for AAV have considerably improved remission rates, reduced the risk of relapse, and improved survival in patients with AAV. However, mortality remains high compared with the general population and the benefits of treatment are often offset by treatment-related comorbidities, organ damage and adverse effect…

A Case of Immunoglobulin A Vasculitis Induced by Ethambutol.

Immunoglobulin A vasculitis (IgAV) is a systemic vasculitis characterized by IgA deposition within the vessel walls. In recent years, an increasing number of IgAV cases associated with tuberculosis infection have been reported. We present the case of a 72-year-old man who developed IgAV after the initiation of treatment for pulmonary tuberculosis, with ethambutol considered to be the causative agent based on the clinical course. In most cases, it is difficult to identify the exact etiology of IgAV, and no standardized management strategy has yet been established. Nevertheless, when drug-induce…

Advances in the treatment of ANCA-associated vasculitis.

Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) consists of a group of small-vessel vasculitides that often present with organ-threatening or life-threatening manifestations. Current immunosuppressive treatments have improved survival and rates of remission, but are not curative, have frequent toxicities, and do not effectively prevent relapse. Clinical trials have established the role of rituximab, an anti-CD20 B cell-depleting monoclonal antibody, in both the remission-induction and maintenance phases of the disease and demonstrated that glucocorticoid doses can be sub…

A case of cutaneous vasculitis triggered by naproxen.

Cutaneous vasculitis is an uncommon inflammatory condition affecting the small blood vessels of the skin. Although most cases are self-limiting, it is important that the condition is recognised and treated promptly, as progression can lead to systemic complications. This case study reports on the care of a 54-year-old man who developed bilateral lower-limb purpura a few weeks after starting naproxen for knee pain. Following assessment and investigations, naproxen was discontinued and oral prednisolone 40mg was prescribed. Within 2 weeks the lesions improved significantly, and the patient has r…
