Motion sickness: its pathophysiology and treatment.

The pathogenesis of motion sickness includes both inner-ear stimulation by body movement, especially a Coriolis-type stimulus, and optokinetic stimulation due to the shift of the surrounding visual fields. According to Kornhuber, Sakata and others, the vestibular cerebellum also participates in an important way. We conducted this study to elucidate the influence of the vestibular cerebellum on the development of motion sickness. We initially focused attention on the visual suppression test of Takemori et al. as a test for vestibular cerebellar function. We reported a modification of this test,

Peripheral vestibular disorders: an update.

PURPOSE OF REVIEW: To provide an update on the most frequent peripheral vestibular disorders. RECENT FINDINGS: The on-going classification of vestibular disorders by the Bárány Society represents major progress. The diagnosis of bilateral vestibulopathy (BVP) requires quantitative testing of vestibular function. 'Acute unilateral peripheral vestibulopathy' (AUPVP) is now preferred over 'vestibular neuritis.' Menière's disease is a set of disorders with a significant genetic contribution. The apogeotropic variant of horizontal canal benign paroxysmal positional vertigo (hcBPPV) and anterior ca

Review of basic medical results of the Salyut-7--Soyuz-T 8-month manned flight.

This paper presents the results of medical investigations performed in the Salyut-7 8-month mission in which a professional physician took part. The paper contains anthropometric measurements, results of investigating the vestibular function, cardiovascular function at rest and in response to multi-step tests (with emphasis on echocardiographic measurements), metabolic parameters and hormonal status. It also discusses medical aspects of the extravehicular activity. The medical investigations, although some new methods were applied, provided the continuity of methodical approaches and data accu

Meniere's disease 1978-1982.

Over 250 reports concerning or related to Meniere's disease, published in English from 1978 through 1982, are summarized. Reports have been classified into the categories of incidence of occurrence, analysis and reporting results, pathophysiology, etiologic theories, diagnostic methods, medical management, and surgical procedures. Major emphases in reporting from 1978 though 1982 have been in the areas of pathophysiologic studies, expansion of diagnostic procedures, and the use of sac decompression and neurectomy procedures. This literature review reveals the continuing disagreement regarding

Ocular vestibular evoked myogenic potentials induced by bone-conducted vibration in patients with unilateral inner ear disease.

CONCLUSION: Patients with vestibular neuritis (VN) with complete canal paresis (CP) showed a higher rate of abnormal ocular vestibular evoked myogenic potential (oVEMP) than those with partial CP. From these results, it is speculated that the superior vestibular nerve function mainly affects oVEMP. Significant correlation was found between the grades of the hearing outcome and oVEMP in sudden sensorineural hearing loss (SSHL). OBJECTIVE: We attempted to correlate the results of oVEMP with the results of cervical VEMP (cVEMP), results of subjective visual vertical (SVV), and clinical course in

Effect of M-current modulation on mammalian vestibular responses to transient head motion.

The precise role and mechanisms underlying efferent modulation of peripheral vestibular afferent function are not well understood in mammals. Clarifying the details of efferent action may lead to new strategies for clinical management of debilitating disturbances in vestibular and balance function. Recent evidence in turtle indicates that efferent modulation of M-currents is likely one mechanism for modifying afferent discharge. M-currents depend in part on KCNQ potassium conductances (Kv7), which can be adjusted through efferent activation of M1, M3, and/or M5 muscarinic acetylcholine recepto

Correlation between canal paresis and spontaneous nystagmus during early stage of acute peripheral vestibular disorders.

CONCLUSIONS: This study demonstrates that the resolution period of spontaneous nystagmus (SN) may provide an indication of vestibular dysfunction on a particular day in the primary care setting. OBJECTIVE: We aimed to predict canal paresis using fundamental observations of SN during the early stage of acute peripheral vestibular disorders. METHODS: The study involved 87 patients who had recently experienced their first episode of acute spontaneous vertigo and direction-fixed horizontal nystagmus. Although they did not exhibit any other neurological deficits, they had been hospitalized with s

Vestibular Disorders in Children With Congenital Cytomegalovirus Infection.

BACKGROUND: Congenital cytomegalovirus (CMV) infection is the leading infectious cause of neurologic disabilities and sensorineural hearing loss in children. Sensorineural hearing loss prevalence in CMV suggests a viral tropism for the inner ear. Vestibular disorders induced by CMV infection are underestimated. This is the largest and most thorough study to assess the incidence of vestibular disorders and their correlation with hearing thresholds in children with CMV. METHODS: This retrospective study assessed a cohort of 52 children with congenital CMV infection and sensorineural impairment

Motorist disorientation syndrome; clinical features and vestibular findings.

BACKGROUND: Motorist Disorientation Syndrome (MDS) is a term used to describe patients who primarily experience symptoms of dizziness/disorientation whilst in a motor car [21]. There is uncertainty about the relevance of vestibular dysfunction and whether this disorder could instead be a visually induced dizziness (VV) or part of a functional disorder similar to Persistent postural perceptual dizziness (PPPD). OBJECTIVE: We present the largest case-series to date of patients whose main complaint is of illusions of movement of self/vehicle when driving, characterising features of this group.

The potential dysfunction of otolith organs in patients after mumps infection.

OBJECTIVE: To investigate the relationship between mumps and the extent of hearing impairment and otolith organ damage. METHODS: A total of 27 patients with unilateral hearing impairment following mumps were enrolled. The degrees of hearing loss and otolith organ damage were confirmed by audiometric and vestibular evoked myogenic potential [VEMP] tests. All the results were compared and analyzed using Stata 13.0 software for Windows. RESULTS: The VEMP thresholds of the affected ears were significantly higher than those of the unaffected ears in both tests (cervical VEMP [cVEMP] test and ocul

Abnormal visuo-vestibular interactions in vestibular migraine: a cross sectional study.

Vestibular migraine is among the commonest causes of episodic vertigo. Chronically, patients with vestibular migraine develop abnormal responsiveness to both vestibular and visual stimuli characterized by heightened self-motion sensitivity and visually-induced dizziness. Yet, the neural mechanisms mediating such symptoms remain unknown. We postulate that such symptoms are attributable to impaired visuo-vestibular cortical interactions, which in turn disrupts normal vestibular function. To assess this, we investigated whether prolonged, full-field visual motion exposure, which has been previous

Visual dependency and dizziness after vestibular neuritis.

Symptomatic recovery after acute vestibular neuritis (VN) is variable, with around 50% of patients reporting long term vestibular symptoms; hence, it is essential to identify factors related to poor clinical outcome. Here we investigated whether excessive reliance on visual input for spatial orientation (visual dependence) was associated with long term vestibular symptoms following acute VN. Twenty-eight patients with VN and 25 normal control subjects were included. Patients were enrolled at least 6 months after acute illness. Recovery status was not a criterion for study entry, allowing recru

The vestibular evoked myogenic potential (VEMP): air- versus bone-conducted stimuli.

OBJECTIVE: Several studies have evaluated the effects of different stimulus and recording parameters on the cervical vestibular evoked myogenic potential (cVEMP); however, it is difficult to directly compare these studies as they have all used different recording methods, different sternocleidomastoid (SCM) muscle contraction/electromyography monitoring methods, and different stimulus parameters. DESIGN: : This study made a direct comparison of the cVEMP in response to air-conducted (AC) and bone-conducted (BC) stimuli in the same subjects, using the same stimulus/recording/electromyography m

Personal experiences with vestibular evoked myogenic potentials as a modern method of diagnosing vestibular organ lesion and monitoring treatment.

Vestibular evoked myogenic potentials (VEMPs) appear to represent a new and promising technique for the assessment of vestibulospinal reflex function. The primary aims of the study described in this article were (1) to record VEMPs in normal volunteers using available equipment and to establish a range of norms of VEMP parameter values (latency, amplitude); (2) to confirm the saccular origin of VEMPs; (3) to assess the diagnostic significance of VEMPs; and (4) to evaluate the usefulness of VEMPs in monitoring therapeutic results. The study population consisted of 252 patients representing vari

Videonystagmoscopy: its use in the clinical vestibular laboratory.

Vestibular function of a population including labyrinthine-defective patients and a control group of age-matched normal healthy volunteers was evaluated using videonystagmoscopy. This device is made of one or two CCD infra-red cameras mounted on diving glasses and allows observation of ocular movements on a video monitor and/or recording on a videotape. Eye movements are observed after rotations in a Bárány chair and during passive head tilts. With this simple and non-invasive test, a screening of vestibular function at bedside or during ENT clinical investigations can be performed. A further
