Tactile, acoustic and vestibular systems sum to elicit the startle reflex.

The startle reflex is elicited by intense tactile, acoustic or vestibular stimuli. Fast mechanoreceptors in each modality can respond to skin or head displacement. In each modality, stimulation of cranial nerves or primary sensory nuclei evokes startle-like responses. The most sensitive sites in rats are found in the ventral spinal trigeminal pathway, corresponding to inputs from the dorsal face. Cross-modal summation is stronger than intramodal temporal summation, suggesting that the convergence of acoustic, vestibular and tactile information is important for eliciting startle. This summation

Effects of rotation on the sleep state-dependent midlatency auditory evoked P50 potential in the human.

Sopite syndrome, characterized by loss of initiative, sensitivity to normally innocuous sensory stimuli, and impaired concentration amounting to a sensory gating deficit, is commonly associated with Space Motion Sickness (SMS). The amplitude of the P50 potential is a measure of level of arousal, and a paired-stimulus paradigm can be used to measure sensory gating. We used the rotary chair to elicit the sensory mismatch that occurs with SMS by overstimulating the vestibular apparatus. The effects of rotation on the manifestation of the P50 midlatency auditory evoked response were then assessed

Effect of white noise "masking" on vestibular evoked potentials recorded using different stimulus modalities.

Short latency vestibular evoked potentials (VsEPs) to linear acceleration impulses (L-VsEPs) are initiated in the otolith organs (saccule and utricle). Some of the saccule afferents have been reported to respond not only to linear acceleration, but also to high intensity acoustic stimuli. If so, the L-VsEP recorded from the saccule (elicited with the stimulus orientated relative to the head so as to optimally activate the saccule, i.e. stimulus in the vertical plane, Z-VsEP) should be reduced during high intensity broad band noise (BBN) "masking". Conversely, the utricular afferents have been

The vestibulo-collic reflex.

Until now there has no been no simple, reliable clinical test that assesses the function of the otolith organs. However, recent work has suggested the occurrence of an easily identifiable electromyographic (EMG) waveform in isometrically contracting sternomastoid muscles in response to loud clicks. This is thought to represent an otolith evoked disynaptic reflex. Equipment routinely used for evoked response audiometry (ERA) has been adapted to investigate this response. A short latency (8 ms) positive-negative EMG waveform has been recorded in 31 of 32 normal subjects. The latency of the posit

Vestibular hypersensitivity to clicks is characteristic of the Tullio phenomenon.

OBJECTIVES: The frequency of pathologically reduced click thresholds for vestibular activation was explored in patients with the Tullio phenomenon (sound induced vestibular activation). METHODS: Seven patients (eight affected ears) with symptoms of oscillopsia and unsteadiness in response to loud external sounds or to the patient's own voice were examined. In all but one patient, vestibular hypersensitivity to sound was confirmed by the fact that eye movements could be produced by pure tones of 110 dB intensity or less. Conventional diagnostic imaging was normal in all cases and three of the

[Cortical vertigo].

Dizziness of cortical origin is the subjective correlate of a disturbance of spatial orientation resulting from cerebrocortical dysfunction. Cortical dizziness in the form of vertigo is rare. If present, it most probably reflects a dysfunction of a vestibular representation in the insula. It may be accompanied by tinnitus, sensory disturbance and possibly also spontaneous nystagmus. The dysfunction of this region may result either from a focal seizure or from a lesion, for instance due to ischemia. Nondirectional, visual dizziness is most probably much more common than vertigo. This latter typ

Effects of long duration centrifugation on head movements and a psychomotor task.

Previous investigations suggest strong similarities between aftereffects of an 1.5-hour 3 gx centrifuge run (Sickness Induced by Centrifugation; SIC) and the Space Adaptation Syndrome (SAS). Head movements proved to be especially nauseogenic in both cases. Until recently, during SIC experimentation, the performance of head movements was not adequately operationalized, which hampered a statistical evaluation. For this purpose, a test was designed by which head movements in four directions (left, right, up, and down) were provoked and registered, once before and once after a 1-h 3 g centrifuge r

[The vestibular system: from structure to function].

The two vestibular receptors, right and left, hidden in the petrous part of the temporal bone with the cochlear receptors, receive information from angular and linear movements of the head and transform them into a nerve message sent to the nuclei of the brainstem, which are connected directly on the one hand to the oculomotor nuclei, at the origin of the oculovestibular reflex (induction of nystagmus), and on the other hand, to the spinal motor neurons, at the origin of the vestibulospinal reflex. These reflexes are associated with responses to visual and somaesthetic information for maintena

The Tullio phenomenon in patients with Menière's disease as revealed with posturography.

The effect of low frequency sound (LFS) on postural stability was studied in 55 healthy volunteers and in 50 patients with Menière's disease. The sound levels ranged from 130 to 132 dB and were given at frequencies of 25, 50 and 63 Hz. The patients with Menière's disease displayed increased body sway during stimulation. In the individual response, 26% of the patients with Menière's disease experienced significant body sway. The results demonstrate that subjects with Menière's disease are sensitive to low frequency sound. The activation of vestibulo-spinal responses by low frequency sound seems

[Effects of intense noise on the statokinetic system of guinea pigs].

The scanning electron microscope and the light microscope were used to examine the inner ear vestibular organs of guinea pigs for structural damages after exposure to continuous intense noise for one hour. The functional test of the vestibular system was assessed with the electronystagmography. The results indicated that vestibular organs could be affected by noise. The sensory cell cilia were mostly damaged or lost in the middle part of the cristae. No remarkable cell alterations were found in the epithelia of the otolithic organs. We also observed that otoconia in the maculae were reduced or

Inner ear effects of topical gentamicin treatment in patients with Menière's disease.

Several efforts have been made to treat patients with Menière's disease with ototoxic drugs. Lately the aminoglycoside antibiotic, gentamicin, has been favored. Several reports have proved its efficacy. In a series of 12 patients treated at the Karolinska Hospital in Stockholm, two patients failed to show improvement regarding their vertigo in spite of repeated courses of treatment. These patients both underwent labyrinthectomy where their vestibular sensory epithelia as well as portions of the cochlea were removed. The findings in these two cases were quite unanimous in that the vestibular se

Otosclerosis involving the vestibular aqueduct and Menière's disease.

The coexistence of otosclerosis and endolymphatic hydrops in the temporal bone have been described; however, the mechanism for the development of endolymphatic hydrops in otosclerosis remains unknown. Among 128 temporal bones with otosclerosis, involvement of the vestibular aqueduct by otosclerosis was observed in four temporal bones from two patients. In all four, the vestibular aqueduct was filled with active otosclerotic foci; the lumen of the endolymphatic duct and sac was narrowed as a result of fibrosis, and endolymphatic hydrops, more severe in the pars inferior than the pars superior,

The pathology of peripheral vestibular disorders in the elderly.

We describe a clinical classification of vertigo commonly seen in the elderly and caused by peripheral vestibular disorders. The classification includes inducible transient vertigo and noninducible protracted vertigo. The peripheral vestibular disorder includes abnormalities of sensory, neural, and mechanical structures and is often asymmetric. Determination of the pathogenesis of these disorders is often speculative. Pathogenesis may include degenerative, infectious, neoplastic, and vascular insults to the vestibular labyrinth.

[Vestibulo-protective properties of regulatory peptides].

Endogenous opioid peptides take an active part in the pathogenesis of motion sickness (MS). Some regulatory peptides (substances P, gamma and des-tyr-gamma-endorphins) manifest antiemetic properties, unlike opioid peptides and morphine. Thus, we have examined the vestibulo-protective properties of some regulatory peptides during simulation of MS in cats. Vestibulo-protective properties have been discovered in some peptides: sweet water hydra undecapeptide, substances P, gamma and des-tyr-gamma-endorphins. It has been suggested that regulatory peptides take an active part in the genesis of vest

Craniocorpography in the diagnosis and therapeutic indication of endolymphatic hydrops.

The glycerol test helps to establish the differential diagnosis between Ménière's disease and other vertigo disturbances, but the test becomes very long and in its primary form informs us only about the cochlear response to the osmotic changes produced by glycerol in the inner ear, and tells us nothing about the vestibular response. In patients with unilateral endolymphatic hydrops we have performed simultaneously the glycerol test with audiometric and craniocorpographic (CCG) control. The response of the vestibular apparatus to the glycerol becomes evident in 84.5% of cases on average in 30 m

Vestibulospinal function in two syndromes with spontaneous attacks of vertigo: evaluation by posturography.

Two vestibular syndromes comprising attacks of spontaneous vertigo were studied and disturbance of the vestibulospinal reflex was evaluated. Patients with Menière's disease were examined between attacks and patients with a sudden unilateral vestibular loss syndrome at least 3-4 weeks after the initial attack. Posturography, (recording and analysing the postural sway) was used to evaluate the influence of these disorders upon the vestibulospinal reflex. In 50% of the patients, the postural sway was disturbed compared with the performance of a normal control group. Menière's disease had a more p

Vestibulo-spinal response modification as determined with the H-reflex during the Spacelab-1 flight.

Our laboratory at the Johnson Space Center has employed the H-reflex recorded from the soleus muscle as a method of monosynaptic reflex testing in conjunction with vertical linear acceleration to assess modification of utriculo-saccular function induced through prolonged exposure to microgravity. It was hypothesized that exposure to free fall would reduce the necessity for postural reflexes in the major leg muscles, and that postural modification would reflect a change, not in the peripheral vestibular organs, but more centrally. This postural adjustment would reflect a sensory motor rearrange

Dynamic modification of the vestibulo-ocular reflex by the nodulus and uvula.

The time constant of the decay of slow-phase eye velocity of postrotatory nystagmus or optokinetic after-nystagmus is reduced during exposure to a stationary visual surround (visual suppression). It is also reduced after tilting the head (tilt suppression). A "dump" mechanism in the vestibulo-ocular reflex has been proposed to rapidly discharge activity from the central vestibular system during both types of suppression. Monkeys lost this mechanism after lesions of the nodulus and uvula. They also lost the ability to habituate the time constant of nystagmus on repeated exposure to optokinetic
