Strict Anatomical Colocalization of Vitiligo and Elastolytic Granulomas.

Vitiligo is the most common depigmenting disorder, with a worldwide occurrence of 0.1-2% in the general population. Multiple conditions have been described colocalized in vitiligo patches, like psoriasis or lichen planus. However, actinic granuloma has not been described in association with vitiligo lesions so far.

Is there a relationship between homocysteine and vitiligo? A pilot study.

BACKGROUND: Pigmentary dilution is observed in patients with homocystinuria. Therefore, it is possible that an increase of local homocysteine (Hcy) interferes with normal melanogenesis and plays a role in the pathogenesis of vitiligo. Vitamin B12 and folic acid, levels of which are decreased in vitiligo, are important cofactors in the metabolism of Hcy. Consequently, a nutritional deficiency in either of these two vitamins will result in an increase in homocysteine in the circulation, a finding that we expect to find in vitiligo. OBJECTIVE: To determine the level of Hcy in the blood of patien…

Clinical analysis of hearing levels in vitiligo patients.

Vitiligo is a common, often inherited disorder resulting from a loss of functional melanocytes. The mechanism by which skin melanocytes disappear can also affect other melanocytic organs. Although melanocyte-associated diseases have racial differences, there have been no studies of hearing loss associated with vitiligo in Asian populations, and no analysis of any relationship between hearing loss and severity of vitiligo. So we tried to assess the hearing differences between Korean vitiligo patients and normal subjects. Pure tone audiometry, auditory brainstem response, and electrocochleograph…

A clinical trial and molecular study of photoadaptation in vitiligo.

BACKGROUND: Photoadaptation to ultraviolet (UV) B phototherapy is due to both pigmentary and nonpigmentary influences. OBJECTIVES: To measure photoadaptation in vitiliginous skin and to compare it with normal pigmented skin. METHODS: Seventeen patients with Fitzpatrick skin phototypes III-VI with vitiligo received six to nine UVB treatments, two to three times weekly. Minimal erythema dose (MED) testing was done at baseline and after all treatments; the percentage change in MED was analysed as a measure of photoadaptation. The percentage decrease in cyclobutane pyrimidine dimers (CPDs) over …

Topical immunotherapy with diphenylcyclopropenone in vitiligo: a preliminary experience.

BACKGROUND: Despite recent significant therapeutic advances, vitiligo remains a clinical conundrum. Topical immunotherapy has been extensively tested in the treatment of various dermatologic disorders, especially those believed to have an immunologic basis. AIM: To evaluate the role of topical diphenylcyclopropenone (DPCP) in the treatment of vitiligo. METHODS: Nineteen patients with limited vitiligo lesions were enrolled in this study. After sensitization with 2% lotion of DPCP in acetone, progressively higher concentrations beginning at 0.001% up to 2% were applied weekly for 6 months to t…

Association of vitiligo with HLA-A2: a meta-analysis.

BACKGROUND: Linkage and association studies suggest that the human leucocyte antigen (HLA) region may be involved in the genetic susceptibility of vitiligo. HLA-A2 has been reported to be associated with vitiligo in some, but not all, studies. OBJECTIVE: To identify sources of the heterogeneity among studies and to quantify effect estimates, we examined the association of HLA-A2 with vitiligo in a meta-analysis of all observational studies comparing the frequencies of HLA-A2 between vitiligo individuals and controls during 1966-2005. METHODS: The summary odds ratio (OR) was calculated by usi…

The course of melanoma-associated vitiligo: report of a case.

The appearance of vitiligo-like lesions in patients with malignant melanoma is a well-known yet uncommon phenomenon. This finding is especially reported in patients undergoing immunotherapy with or without chemotherapy for malignant melanoma and is generally believed to be associated with a better prognosis. We report a case of preexisting vitiligo in a 48-year-old man, aggravated after chemo-immunotherapy of pulmonary metastatic melanoma with interferon-alpha, vinblastine and dacarbazine. Skin lesions remained stable after discontinuation of the treatment, and repigmentation heralded the recu…

[Cuprum sulfate and vitix in the treatment of vitiligo in children].

Incidence rate of vitiligo among children, its adverse effect on psycho-emotional state of children, uncertainty in the etiology and pathogenesis and relative ineffectiveness of existing treatment approaches stimulates attempts to elaborate new methods of management of this disease. It is well known, that melanin is formed from tyrosine by enzyme tyrosinase. Cuprum is a cofactor of this photochemical process. In a number of experiments it was shown that keratinocytes derived from vitiligo lesions produce increased number of superoxide anions (hyperactive oxygen and nitric oxide). In patients w…

Unilateral poliosis of the eyelashes in children associated with vitiligo.

Two children developed sudden progressive unilateral poliosis of their eyelashes during the course of a 1-month period. The poliosis occurred in the absence of any associated condition apart from vitiligo, both of which have remained isolated findings after 2 and 4 years of follow-up, respectively. Although it is important to exclude associated conditions, poliosis of the eyelashes may be a benign condition that may remain unilateral.

Decreased phenylalanine uptake and turnover in patients with vitiligo.

The human epidermis has the full machinery for autocrine L-phenylalanine turnover to L-tyrosine in keratinocytes and melanocytes. Phenylalan … t was shown that the uptake of this amino acid is regulated by calcium. The depigmentation disorder vitiligo provides a unique model to follow impaired L-phenylalanine turnover in the skin as well as in serum because affected individuals hold an impaired epidermal 6BH4 de novo synthesis/recycling and regulation including low epidermal PAH activities. After …

The genetics of generalized vitiligo and associated autoimmune diseases.

Generalized vitiligo is an acquired disorder in which patches of depigmented skin, overlying hair, and oral mucosa result from progressive autoimmune loss of melanocytes from the involved areas. Although vitiligo is perhaps the most common pigmentary disorder, insufficiently clear clinical definition of the disorder and lack of a good laboratory animal model have inhibited progress in understanding its pathobiology, its environmental triggers, and in developing specific and effective therapeutic approaches. Vitiligo results from a complex interaction of environmental, genetic, and immunologic …

[Vitiligo. Treatment of 12 cases with topical tacrolimus].

INTRODUCTION: Vitiligo is a frequent dyschromia, characterized by achromic macules that reflect the absence of melanocytes. The cause of this selective destruction seems to be due to an autoimmune phenomenon. Tacrolimus is an immunomodulator produced by Streptomyces tsukubaensis, whose topical use has been approved for atopic dermatitis. It has been tested in other dermatoses where immunological phenomena are involved. MATERIAL AND METHODS: During the period from September 1, 2003 to April 30, 2004, an open study was carried out on 12 cases of vitiligo treated with topical tacrolimus 0.1 % tw…
