White matter changes in elderly people: MR-pathologic correlations.

Magnetic resonance (MR) imaging features of white matter lesions, often seen in the elderly, are correlated with histologic findings. Dilatation of perivascular spaces is seen, especially in the frontal and/or parietal subcortical white matter; the spaces are less than 3 mm in diameter and have sharp margins with no perifocal abnormality. Old lacunar infarcts are larger than 3 mm in diameter and are irregularly shaped and accompanied by perifocal myelin pallor and gliosis. Periventricular hyperintensity, including cap and rim, histologically shows myelin pallor, dilatation of perivascular spac…

["White bowel". A sonographic sign of intestinal lymph edema?].

AIM: We recently introduced the term "white bowel" to describe the hyperechoic appearance of the bowel wall found in a patient with HIV-associated enteropathy. The aim of this study was to describe changes of the bowel wall and to demonstrate possible causes of this phenomenon. PATIENTS AND METHODS: 10 patients identified as showing this phenomenon were enrolled in this study. The ultrasound examinations of the patients were re-evaluated with special regard to the echogenicity of the different layers of the bowel wall, to mesenteric lymph nodes, and to thrombosis of the mesenteric vein. RESU…

Omphalocele and gastroschisis: Black-White disparity in infant survival.

BACKGROUND: Racial/ethnic variations in the occurrence of abdominal wall defects have been previously noted but it remains poorly understood whether race/ethnicity is a determinant of survival among affected infants. METHODS: Study was conducted on cases of gastroschisis and omphalocele recorded for the years 1983-1999 at the New York Congenital Malformation Registry. Adjusted and unadjusted hazard ratios were generated from a Proportional Hazards Regression model to compare survival among affected Blacks, Hispanics and Whites. The major end point of analysis was differences in all cause mort…

A reoperation of adult-Type Bland-White-Garland syndrome.

We present a 25-year-old female patient with Bland-White-Garland syndrome (anomalous origin of the left coronary artery from the pulmonary artery) who underwent mitral valve replacement (MVR) and coronary artery bypass grafting. She had previously undergone MVR and had been treated by the Takeuchi procedure for mitral valve regurgitation and a coronary artery anomaly, respectively, at 17 years of age. She was hospitalized with shortness of breath within 7 years of surgical intervention. Swan-Ganz catheterization revealed mild pulmonary hypertension and elevated mean pulmonary wedge pressure. E…

Neonatal jaundice in Asian, white, and mixed-race infants.

BACKGROUND: East Asians have inherently higher bilirubin levels at birth than whites. The potential for unnecessary treatment makes jaundice a problem of public health and clinical significance. OBJECTIVES: To report the occurrence of jaundice diagnoses in East Asian and mixed East Asian/white infants in Washington State in recent years, and to compare the risk of diagnosis with neonatal jaundice among these infants, relative to white infants. DESIGN: Population-based cohort study in Washington state. Participants were infants of full East Asian parentage (n = 3000), maternal Asian parentage…

MRI shows abnormal white matter maturation in classical holoprosencephaly.

In an attempt to assess white matter maturation in holoprosencephaly (HPE), MRI scans of 47 patients with HPE were retrospectively reviewed. White matter maturation was delayed in 25/47 patients, including 24/29 patients with classic HPE who were </=12 months old at the time of imaging. Of interest, all six patients with the middle interhemispheric (MIH) variant of HPE had normal myelination, regardless of age. This study provides further evidence that the classic and MIH variants of HPE have different underlying causes.

Abomasal ulcers in captive white-tailed deer (Odocoileus virginianus).

Abomasal ulceration was noted in 32 of 200 white-tailed deer. Ulceration was most common in the abomasal pylorus and at the abomasal-duodenal junction. Abomasal ulceration was characterized by focal to multifocal, sharply demarcated areas of coagulation necrosis and haemorrhage extending through the mucosa, with fibrin thrombi in mucosal blood vessels of small diameter. Ulcerated areas were often covered by a mixture of mucus, debris and neutrophils. Visible bacteria were not associated with ulcerative lesions. All deer with abomasal ulceration had intercurrent disease, including bacterial pne…

What are those white patches on my patient's skin?

Vitiligo is a skin disease that affects 1% of the population. Vitiliginous skin does not contain pigment-producing cells called melanocytes. This leaves the appearance of white patches on the skin, which can either be generalized or localized. It is important for dermatology nurses to understand the basic pathophysiology of vitiligo, the various treatment methods, how to protect vitiliginous skin, and the psychological impact of the disease.

Sick sinus syndrome in nine West Highland white terriers.

Sick sinus syndrome is a clinical term used to describe the clinical signs of sinus node dysfunction. This paper describes the clinical data from nine West Highland white terriers, eight females and one male, in which a diagnosis of sick sinus syndrome was made. The most common clinical signs were episodic weakness and presyncope. Electrocardiographic findings included sinus bradycardia, sinus arrest with or without escape complexes, disturbances of atrioventricular conduction, paroxysmal supraventricular tachycardia, or some combination of these dysrhythmias. The main radiographic changes wer…

Late onset white matter disease in peroxisome biogenesis disorder.

OBJECTIVE: To report late onset cerebral white matter disease as a distinctive phenotype in peroxisome biogenesis disorder (PBD). BACKGROUND: There is phenotypic and genetic overlap among the PBD known as Zellweger syndrome (ZS), infantile Refsum disease (IRD), and neonatal adrenoleukodystrophy (NALD). Distinctive external features are variable among these three disorders, and neurologic deficit has its onset at birth or in infancy. In a structured follow-up cohort of 25 patients with PBD, not including ZS, three patients had an unusual pattern of cerebral white matter disease with onset past…

Mange epizootic in white-nosed coatis in western Mexico.

From November of 1994 to June of 1996 an epizootic of mange, probably caused by the mite Notoedres cati, occurred in white-nosed coatis (Nasua narica) in the tropical dry forests of the Chamela-Cuixmala Biosphere Reserve in western Mexico. A monitoring scheme to determine the extent and severity of the epizootic within coatis was implemented. Trapping periods and transects were conducted for 2 yr. To control the spread of the disease, all captured infected coatis were either euthanized or treated with acaricides such as Butox and Ivomec-F, depending on the severity of their infection. Four oth…

Blood Pressure Monitoring. Task force V: White-coat hypertension.

TERMINOLOGY: Two terms are in current use to describe patients whose blood pressures are high only in a medical setting (white-coat hypertension and isolated office or clinic hypertension). The term white-coat effect is also commonly used to describe the pressor response to the clinic setting. DEFINITIONS: White-coat hypertension is generally defined as a persistently elevated clinic blood pressure in combination with a normal ambulatory blood pressure (ABP). There is disagreement regarding the optimal cutoff point for ABP. The white-coat effect is operationally defined as the difference betw…

Circadian rhythm of white blood cells during clozapine treatment.

RATIONALE: The atypical antipsychotic clozapine is effective in the treatment of patients with refractory schizophrenia. It carries a well-known risk of neutropenia and agranulocytosis, which necessitates the immediate discontinuation of clozapine. OBJECTIVE: We report a patient who developed neutropenia on clozapine, but behind the cell count decrease showed to be a diurnal variation of the white blood cells (WBC). METHODS: Due to the lack of efficacy of subsequent treatment of conventional and other atypical neuroleptics, treatment with clozapine was restarted after discontinuation. When t…

Cognition and white matter hyperintensities in older depressed patients.

OBJECTIVE: The authors compared amounts of white matter hyperintensity in late- and early-onset depressed patients and never-depressed older subjects, compared neuropsychological function in these groups, and investigated the association between white matter hyperintensities and cognitive function in depression. METHOD: Sixty currently depressed patients whose first depression occurred after age 50 years, 35 depressed patients over age 50 whose first depression occurred before age 35, and 165 nonpsychiatrically ill subjects over age 50 underwent magnetic resonance imaging (MRI) and neuropsych…

Impact of theophylline use in Wolff-Parkinson-White syndrome.

Methylxanthine use in the treatment of apnea of prematurity is well documented. This drug is avoided in patients with aberrant pathways of conduction such as Wolff-Parkinson-White syndrome. In theory, methylxanthines enhance precipitation and exacerbation of tachyarrhythmias to which these patients are predisposed. This article reports a case of Wolff-Parkinson-White syndrome in a preterm neonate with severe apneic episodes in which methylxanthines were used. However, no adverse effects on cardiac rate or rhythm were encountered.

Liver involvement in white patients with sickle-cell disease.

BACKGROUND: Liver involvement and cholelithiasis are common complications of sickle-cell disease. The incidence of clinically evident hepatic damage reported in the literature for black people varies from 15% to 30%, while no data are reported for white people. OBJECTIVE: To evaluate the liver involvement in 40 patients with homozygous sickle cell anemia (the beta 5 beta 5 form of sickle-cell disease) and 102 patients with double-heterozygous hemoglobin S and beta-thalassemia (65 with the beta 5 beta 0th and 37 with the beta 5 beta +th form of sickle-cell disease). SETTING: The Department of…
