Medical ethics in the 1990s. Emphasis on death-and-dying issues will continue but social concerns will command increased attention.

During the 1990s medical ethics will undergo changes. Individual clinical issues, especially those related to death and dying, will continue to create conflict and preoccupy hospital staffs. But professional ethicists will focus on social concerns more frequently than they have in the past. Following are some of the most crucial ethical issues and directions they are likely to take in this decade: Clinical practice and the law will move toward less demanding standards of proof regarding the withdrawal of treatment from patients who are no longer competent. Public policy will set more lenient s

An Order Protocol for Respiratory Distress/Acute Pain Crisis in Pediatric Palliative Care Patients: Medical and Nursing Staff Perceptions.

BACKGROUND: An order protocol for distress (OPD), including respiratory distress and acute pain crisis, has been established for pediatric palliative care patients at Sainte-Justine Hospital (SJH). After discussion with the patient/his or her family, the OPD is prescribed by the attending physician whenever judged appropriate. The OPD can then be initiated by the bedside nurse when necessary; the physician is notified after the first dose is administered. OBJECTIVES: The study objectives were to evaluate the perceptions and experience of the medical/nursing staff towards the use of the OPD.

Neural circuits in goal-directed and habitual behavior: Implications for circuit dysfunction in obsessive-compulsive disorder.

Goal-directed and habitual actions are essential for normal functioning in everyday life. Goal-directed behaviors are actions that are executed to achieve specific goals. With repetition, such as a daily routine, these goal-directed actions become automatized and habitual. However, these useful behaviors can become aberrant, manifesting as key symptoms in several psychiatric disorders, including obsessive-compulsive disorder (OCD). A comprehensive understanding of the neural circuits underlying both aberrant and non-pathological goal-directed and habitual behaviors can lead to improved treatme

Directional beam multiplexing using cylindrical holographic acoustic metasurfaces integrated with surface wave reflectors.

Recent advancements in acoustic metasurfaces have significantly improved beamforming and steering capabilities, with beam multiplexing emerging as a key enabler of multidirectional sound projection. This paper proposes a cylindrical holographic acoustic metasurface integrated with surface wave reflectors (SWRs) to realize efficient acoustic beam multiplexing. By transitioning from conventional planar designs to a cylindrical geometry, the proposed metasurface supports the simultaneous generation of multiple highly directional beams at distinct combinations of elevation and azimuthal angles. Conversion surgery after BRAF-directed therapy for unresectable papillary thyroid carcinoma with severe tracheal invasion.

Locally advanced papillary thyroid carcinoma (PTC) with invasion of the airway and major vessels presents a significant therapeutic challenge. Although multikinase inhibitors (mTKIs) are effective for unresectable thyroid carcinoma, their antiangiogenic effects may increase the risk of life-threatening hemorrhage in patients with major vessel invasion or intraluminal airway exposure. Recent advances in BRAF-directed therapy have suggested a potential role for conversion surgery in selected patients with advanced thyroid carcinoma. A 75-year-old man presented with PTC with extensive intralumina

Approach and Challenges for Patients With Advanced Radioiodine-Refractory Differentiated Thyroid Cancer.

OBJECTIVE: Differentiated thyroid cancers (DTCs), including papillary, follicular, and oncocytic subtypes, are generally associated with an excellent prognosis due to their responsiveness to conventional therapies, including surgery, thyroid-stimulating hormone suppression, and radioactive iodine (RAI) therapy. However, a subset of patients develops RAI-refractory disease, characterized by tumor dedifferentiation and loss of iodine avidity, resulting in disease progression despite standard interventions. This group poses a considerable therapeutic challenge and is associated with markedly poor

Direct cutaneous gene delivery in a human genetic skin disease.

The skin is an accessible somatic tissue for therapeutic gene transfer and, depending on therapeutic goals, a variety of cutaneous gene delivery approaches are currently available. Recent advances in direct injection of naked DNA into intact skin have shown promise and are less labor-intensive than approaches involving grafting of genetically modified cells. We have regenerated skin from transglutaminase 1 (TGase1)-deficient patients with the genetic skin disease lamellar ichthyosis (LI) on nude mice to examine the corrective impact of direct naked plasmid injection. Regenerated LI patient ski

Exploring Theranostic Avenues in Adrenocortical Carcinoma Using Chemokine Receptor and Prostate-Specific Membrane Antigen-Directed PET/CT.

We report on an adrenocortical carcinoma (ACC) patient, which has exhausted previous treatment options and was scheduled for prostate-specific membrane antigen (PSMA)- and C-X-C motif chemokine receptor 4 (CXCR4)-targeted PET/CT. We identified PSMA-avid pulmonary metastases exhibiting modest radiotracer accumulation, while chemokine receptor PET/CT provided intense uptake. This dual-tracer molecular imaging approach revealed that chemokine receptor PET appears to be more suitable in patients with advanced ACC, indicating that CXCR4-directed radioligand therapy may be considered in such patient

The mucopolysaccharidoses: a success of molecular medicine.

The mucopolysaccharidoses represent a devastating group of lysosomal storage diseases affecting approximately 1 in 25 000 individuals. Advances in biochemistry and genetics over the past 25 years have resulted in the identification of the key hydrolases underlying the mucopolysaccharidoses, with subsequent isolation and characterisation of the genes involved. Ultimately these advances have led to the recent development of specific treatment regimens for some of the mucopolysaccharidoses, in the form of direct enzyme replacement. Direct replacement of the defective gene product has been attempt

Paramedic laryngoscopy in the simulated difficult airway: comparison of the Venner A.P. Advance and GlideScope Ranger video laryngoscopes.

OBJECTIVES: This study assesses intubation times and potential trauma with two new portable video laryngoscopes, the GlideScope Ranger (GSR) and the Venner A.P. Advance (APA), in a simulated difficult prehospital airway. The GSR has a hockey stick shape and is inserted by a different (midline) technique compared with direct laryngoscopy and requires the use of a stylet. The APA has a handle similar to a direct laryngoscope, but with an angulated difficult airway blade. The APA is designed to have an intuitive insertion technique somewhat similar to that of direct laryngoscopy (lateral tongue d

Genetics of Congenital Heart Disease.

Congenital heart disease (CHD) is a common type of birth defect and a leading cause of infant and childhood mortality. Although recent advancements in genetic technologies have allowed for the discovery of new genomic variation associated with CHD, the prioritization and interpretation of variants for pathogenicity remain a challenge. Further, the underlying molecular genetic mechanisms for CHD remain incompletely defined. In this article, we summarize established genetic etiologies of CHD and highlight advances in knowledge of the underlying genetic architecture of CHD, along with challenges
