Effective pain management is vital for critically ill patients, particularly post-surgery or trauma, as it can mitigate the stress response and positively influence morbidity and mortality rates. The suboptimal treatment of pain in Intensive Care Unit (ICU) patients is often due to a lack of education, apprehensions about side effects, and improper use of medications. Hence, the engagement of pain management and anesthesiology experts is often necessary. While opioids have been traditionally used in pain management, their side effects make them less appealing. Local anesthetics, typically used

In recent years, treatment options for opioid use disorder have expanded significantly with the availability of a wide range of medication formulations. Extended-release buprenorphine is a long-acting, injectable medication used to treat moderate-to-severe opioid use disorder by providing a steady, continuous dose of buprenorphine over several weeks. Needle-related anxiety and injection-site pain are frequent adverse events that can significantly impede patient adherence to extended-release buprenorphine therapy. Topical and injectable anesthetics can enhance patient comfort and support greate

The behavioral effects of anesthetics are highly conserved across species, hinting at shared and fundamental underlying mechanisms. Here we compile a dataset of multiscale neural activity during wakefulness and anesthesia, encompassing human, macaque, marmoset, mouse, zebrafish and nematode. Applying massive feature extraction, we characterize local neural dynamics across >6,000 time-series features. This reveals a conserved dynamical profile of anesthesia across species, characterized by shorter intrinsic timescales of neural activity and dampened inter-regional synchrony. Deep-brain stimulat

PURPOSE OF REVIEW: The increasing number of procedures done in the ambulatory surgical setting necessitates the need for analgesic modalities that enable the management of postsurgical pain with fast onset, predictable duration of action, and minimal need for management of undesirable side-effects.
RECENT FINDINGS: The novel strategies for administration of local anesthetics in the ambulatory setting include prolonging their analgesic action at the site of surgical trauma while reducing systemic effects that result from their metabolism. Development of opioids aims to address receptor sites t

The efficacy of diphenhydramine hydrochloride with epinephrine was tested against lidocaine with epinephrine in a double-blind study. Patients with allergic reactions to general local anesthetics were safely and effectively treated with diphenhydramine hydrochloride as an anesthetic alternative.

We anesthesized a patient susceptible to malignant hyperthermia (MH) three different times by epidural anesthesia with different types of local anesthetics. His skinned fiber test showed a marked acceleration of calcium (Ca2+)-induced Ca2+ release (CICR). When ester type local anesthetic was used for ankle contracture repair, MH signs appeared following the release of the tourniquet. CICR test is reliable for diagnosing different types of MH.

Infants in NICUs undergo a variety of painful procedures. The management of pain has become an integral part of newborn infant care with the use of both systemic and topical agents to provide analgesia and anesthesia for procedural pain. Tetracaine and prilocaine-lidocaine are the 2 topical anesthetics most frequently used. Tetracaine belongs to an ester group of local anesthetics available as a topical 4% gel (Ametop, Smith and Nephew, Canada). The major side effects reported when using topical anesthetics are cutaneous reactions. There are no definite reports of systemic toxicity in the publ

BACKGROUND: Fractional carbon dioxide (CO2) laser resurfacing is an effective treatment of various dermatological conditions. Pain, erythema, and edema following fractional ablative resurfacing are key limitations to patient comfort and satisfaction.
OBJECTIVE: This study aims to describe a novel technique using laser-assisted drug delivery to administer a combination of local anesthetics and epinephrine both during and after fractional CO2 laser resurfacing, enhancing procedural anesthesia and postprocedure recovery.
METHODS: We performed an observational review of patient-reported pain sco

This study compared the short-term efficacy of two treatments (local anesthetics (A) and local anesthetics and lavage (AL)) in patients with permanently displaced discs and temporomandibular disorder (TMD) pain. 45 patients participated in the single-blind randomized controlled trial. All patients had received: a Research Diagnostic Criteria/TMD diagnosis of disc displacement without reduction; and magnetic resonance imaging confirmation of non-reducing disc displacement. Participants were randomized to treatment with A or AL and were assessed at baseline and at 1 and 3 month follow-ups. The p

BACKGROUND: Epinephrine is currently added to local anesthetics for its advantageous vasoconstrictive effects. Traditionally, however, its use is not advised for digital block anesthesia due to the risk of digital gangrene.
OBJECTIVE: To observe vasoconstrictive effect of epinephrine in digital block anesthesia.
METHODS: Digital blocking with epinephrine containing lidocaine was performed in 24 subjects undergoing surgical procedure of their finger and toes. Digital artery blood flows were studied with color Doppler ultrasonography before digital blocking and at 10th, 60th, and if needed 90t

BACKGROUND: Reports of acquired strabismus caused by injection of local anesthetics during cataract surgery have increased recently. The authors proposed a mechanism to explain the occurrence of strabismus with apparent overactive muscles after cataract surgery.
METHODS: The authors studied 19 patients in whom strabismus developed after cataract surgery. Prism and cover test in the diagnostic positions of gaze and forced-duction testing were used to identify the affected muscles.
RESULTS: The deviation was greater in the field of action of the presumed tight muscle in 16 of 19 patients. An i

Postoperative pain management remains a challenge in clinical practice. Traditional local anesthetics often require repeated administration due to their short duration of action, which increases the risk of systemic toxicity and nerve damage. Herein, we developed an injectable, dual-tunable, low-ethanol lyotropic liquid crystal-forming system (LCFS) for sustained local ropivacaine (Rop) release, achieving long-lasting analgesia. Upon injection, the precursor solution transformed into a gel in situ, forming a drug reservoir for sustained release of Rop. The system was optimized by adjusting the

Adrenalectomized mice may develop local (skin) and systemic reactions on the introduction of immune complexes formed in vitro. Regularities in respect to the composition of immune complexes (in which their general, toxic, action and the capacity to produce local reactions was expressed) were revealed. Treatment with agents depressing the anaphylactic reaction (cortisone, antihistaminic drugs, anesthetics) diminished the reactions to the immune complex. Local reaction to the immune complex decreased in animals with artificially-induced fall of the blood complement level.

Pain states can be divided into three categories: acute, chronic not resulting from malignancy, and chronic malignant pain. A Pain Clinic can provide in-depth evaluation and treatment of difficult pain problems. A variety of oral and parenteral medications, ranging from nonsteroidal analgesics to narcotics, are available to control pain. Local anesthetics can be used for local infiltration, and peripheral and central nerve blocks can also be used as indications warrant.
