Cast-brace treatment of ankle sprains.

Forty-two severely sprained ankles treated by a cast-brace have been reviewed. This method was devised to avoid the disadvantages of immobilization. Diagnosis was based on the history and clinical signs. Strain films were taken in 31 patients but it is submitted that this investigation may be misleading unless general anaesthesia is used. The results of the treatment by cast-brace compare favourably with those by other methods and it is considered that patients were able to return to sport in a shorter time.

Nonoperative management of athletic ankle injuries.

Few injuries in sports are more ubiquitous than those involving the ankle. Athletes in some endeavors, notably football and basketball, routinely have their ankles prophylactically taped at a cost of hundreds of thousands of dollars and, probably, millions of man hours. Other sports, such as skiing, involve encasement of the foot, ankle and lower leg in plastic and foam to the extent of almost complete exclusion of motion. In spite of these rather heroic measures, ankle injuries continue to constitute a significant threat to athletes in these and most other activities.

Cross-foot flap for ankle coverage.

The usual cross-leg flap procedures to cover composite wounds involving exposed ankle joint or bone require awkward positioning of the extremities with resultant complications. This is especially true in young, muscular patients with thick legs. A dorsalis pedis flap from the uninjured foot used as a cross-foot flap has been reliable and successful in our experience. A representative case history and the multiple advantages of this technique are described. We believe that this flap is an excellent alternative to the free-flap technique for coverage around the ankle area, especially in hospital…

Ankle joint synoviography in rheumatoid arthritis.

Twenty-six ankles of 23 patients with seropositive or negative arthritis and 24 ankles of 12 cadavers as a control group were studied by contrast synoviography. The main features in the patients were capsule enlargement, hypertrophic synovitis, and connections to tendon sheaths and to the posterior subtalar joint. None of the controls showed capsule distension or irregular outline of the joint cavity, but connections to tendon sheaths and/or to the posterior subtalar joint were present. Other features such as tibiofibular recess, early lymphatic filling, and joint destruction were seen.

Achilles Allograft Reconstruction for Ankle Multiligament Injuries in a Patient Presenting With Rotational Ankle Instability: A Surgical Technique.

The ankle comprises two main ligamentous complexes: the medial and lateral complex. In cases of chronic ankle instability following a sprain involving both complexes, rotational ankle instability can develop. We introduce a 25-year-old male with rotational ankle instability who reported having 20 episodes of "giving away" over the 6 months following an ankle sprain. Due to persistent instability, the patient was deemed a candidate for surgical reconstruction. An Achilles tendon allograft was utilized to reconstruct four ligaments-two from the lateral complex (the anterior talofibular ligament …

Radiographic evaluation of ankle alignment using the hip-to-calcaneus view in patients with end-stage ankle arthritis.

BACKGROUND: This study aimed to compare various ankle alignment parameters in the hip-to-calcaneus (H-C) view and conventional whole-leg standing radiographs. METHODS: We retrospectively reviewed 61 patients with end-stage ankle arthritis (Takakura stage 3b/4) who underwent ankle arthrodesis. Preoperative H-C views and conventional radiographs were used to measure the hip-knee-ankle angle, weight-bearing line percentage, talar tilt, and medial distal tibial angle. The percentage of the H-C axis crossing the talus was also analyzed for asymmetry and correlated with the tibiocalcaneal angle, ta…

Construction and validation of a nomogram model for predicting different sites of ankle pain in runners with chronic ankle instability.

This study aimed to establish a risk prediction nomogram model for anterolateral, mediolateral, and posterolateral ankle pain in runners with chronic ankle instability (CAI) and analyse the potential risk factors for pain at different ankle sites. Thirty recreational runners with CAI who reported ankle pain in the anterolateral, mediolateral, or posterolateral regions were recruited for this study. Kinematic, kinetic, and electromyographic data during running were collected using motion capture system, 3-D force platform, and surface electromyography system. These data were used to generate a …

Effects of Balance and Strength Training for Ankle Proprioception in People with Chronic Ankle Instability: A Randomized Controlled Study.

BACKGROUND: After an ankle sprain, the ligament and joint capsule are damaged, and as a result, proprioceptive sense is damaged, causing a feeling of giving away in the ankle and resulting in recurrent sprains. Given the relevant studies, it has been seen that people with chronic ankle instability (CAI) commonly have deficits in joint position sense and reinjury risks. Joint position sense plays an important role in ankle control, thereby reducing the risk of injury. Therefore, this study aims to compare the effects of balance and strength training on ankle proprioception in people with CAI. …

Arthroscopic treatment for anterolateral impingement of the ankle: Systematic review and exploration of evidence about role of ankle instability.

BACKGROUND: Arthroscopic debridement is a common surgical treatment for patients with anterolateral impingement (ALI) of the ankle. Although they often have a history of ankle sprain, information regarding the role of ankle instability in ALI is limited. The aims of this review were to: 1) assess the clinical outcomes of arthroscopic surgical treatment for ALI of the ankle; and 2) review the data regarding anterior talofibular ligament (ATFL) injury and lateral ankle instability in patients who underwent arthroscopic surgery for ALI. METHOD: A literature search of Pubmed and EMBASE was perfor…

Effects of solid ankle-foot orthoses with individualized ankle angles on gait for children with cerebral palsy and equinus.

PURPOSE: For children with cerebral palsy (CP) and equinus, the conventional practice of setting the ankle angle in an ankle-foot orthosis (AA-AFO) at 90∘ may not adequately accommodate gastrocnemius length/stiffness. Therefore, this study compared the effects of statically-optimized solid AFOs with individualized AA-AFOs (iAA-AFOs) and conventionally-prescribed AFOs on gait for children with CP and equinus. METHODS: Ten children with CP and equinus (15 limbs with AFOs), and 15 typically-developing (TD) children participated. For the children with CP, solid AFOs with iAA-AFOs (range = 5∘-25∘ …
