Midfoot injury in children related to mini scooters.

The remarkable rise in popularity of mini scooters has been accompanied by an increase in the number of foot and ankle injuries. We have observed unique pediatric foot injuries related to the use of mini scooters. An association may be present linking the design of the mini scooter and the riding practices of children. We report on two unique midfoot injuries in children occurring with mini scooter use and discuss how these foot injuries may be related to scooter design and children's riding practices. Safety guidelines aimed at the prevention of foot and ankle injuries while riding mini scoot…

Lower Extremity Orthopedic Augmentation Utilizing a Synthetic Hybrid-Scale Fiber Matrix.

Introduction Lower extremity injuries represent about 15% of all emergency room visits in the United States, with ankle injuries accounting for 20% of these. Surgical site infections resulting from ankle reconstructive surgery can result in severe complications, including amputation. Depending on the procedure performed, surgical site infections of ankle reconstructive surgery can occur at a rate as high as 14%. A synthetic hybrid-scale fiber matrix with an engineered design resembling a native human extracellular matrix could be utilized to augment these difficult surgical procedures. Materia…

Avulsion fractures of the medial and lateral malleoli of the ankle in cadavers: new radiographic projections for improved diagnosis.

The malleolar attachment sites of the tibionavicular (TN), tibiocalcaneal (TC), posterior tibiotalar (PTT), anterior talofibular (ATF), and calcaneofibular (CF) ligaments of 3 cadaveric ankles were dissected. Standard and new radiographic projections of the ankle were obtained with the foot in different positions and various degrees of beam angulation. Simulated avulsion injuries related to these ligaments were created, and the visibility of these structures was assessed. Avulsion injuries of the TN ligament were better assessed in the plantar-flexed radiographs with lateral beam angulation. S

Ankle Fracture in Hereditary Sensory Neuropathy Type 1.

Ankle fractures account for approximately 9% of all adult fractures annually. The ankle anatomically is particularly vulnerable to significant skin compromise in the setting of trauma. Significant fracture blistering and soft tissue compromise is predominantly seen in high-energy ankle injuries. Hereditary sensory autonomic neuropathy type I is a rare progressive neurological disorder resulting in distal sensory loss and autonomic disturbances with variable motor involvement. We present a case involving a hereditary sensory autonomic neuropathy type I patient with unexpected significant soft t…

Factors affecting healing of ankle fractures.

Ankle fractures have been widely described in orthopedic and podiatric studies. These injuries have been associated with significant patient morbidity, infection, malunion, nonunion, and arthritis. Avoiding complications and optimizing outcomes demands an awareness of the factors affecting the healing of ankle fractures. The prognosis of surgical treatment of these injuries has varied according to patient factors or injury severity, or a combination. Cigarette smoking, obesity, and diabetes are some of the factors linked to the prognosis of ankle fractures. We conducted a retrospective cohort

Instability of the distal tibiofibular syndesmosis.

The distal tibiofibular syndesmosis (DTFS) is more frequently injured than previously thought. Early diagnosis and appropriate treatment is essential to avoid long term complications like chronic instability, early osteoarthritis and residual pain. Management of these injuries require a complete understanding of the anatomy of DTFS, and the role played by the ligaments stabilizing the DTFS and ankle. High index of suspicion, appreciating the areas of focal tenderness and utilizing the provocative maneuvers help in early diagnosis. In pure ligamentous injuries radiographs with stress of weight

Clinician-friendly lower extremity physical performance tests in athletes: a systematic review of measurement properties and correlation with injury. Part 2--the tests for the hip, thigh, foot and ankle including the star excursion balance test.

OBJECTIVE: To review the quality of literature and measurement properties of physical performance tests (PPTs) of the lower extremity in athletes. METHODS: Using the PICOS method we established our research question as to whether individual PPTs of the lower extremity have any relationship to injury in competitive athletes ages 12 years to adult (no limit). A search strategy was constructed by combining the terms 'lower extremity' and synonyms for 'performance test' and names of performance tests with variants of the term 'athlete'. After examining the knee in part 1 of this 2 part series, th

Test-retest reliability [corrected] of center of pressure measures of postural stability during quiet standing in a group with musculoskeletal disorders consisting of low back pain, anterior cruciate ligament injury and functional ankle instability.

Reliability is a population-specific property, but to the authors' knowledge there has been no study to determine the test-retest reliability of the postural stability measures such as center of pressure (COP) measures in the population of patients with musculoskeletal disorders (MSDs), while their clinical applications have been presented in literature. So, 33 patients with low back pain (LBP), anterior cruciate ligament (ACL) injury and functional ankle instability (FAI) randomly completed postural measurements with three levels of difficulty (rigid surface-eyes open, rigid surface-eyes clos

The sprained ankle.

The sprained ankle is the most common musculoskeletal injury seen by physicians caring for active youngsters and adults. It accounts for approximately one fourth of all sports-related injuries and is commonly seen in athletes participating in basketball, soccer, or football. It has been shown that one third of West Point cadets suffer an ankle sprain during their 4 years at the military academy. While diagnosis and management of the sprained ankle is usually straightforward, several serious injuries can masquerade as an ankle sprain, and it is important for the clinician to recognize these to

Symptomatic ossification of the tibiofibular syndesmosis in professional football players: a sequela of the syndesmotic ankle sprain.

Ankle syndesmosis sprains are common injuries in collegiate and professional football. Several reports have documented that patients with syndesmosis injuries require a longer time to return to full athletic participation than patients with lateral ankle sprains. Here we present the cases of two professional football players with ankle pain secondary to syndesmosis ossification following documented syndesmosis ankle sprains. Both patients eventually required resection of the heterotopic ossification to allow a pain-free return to football. We conclude that syndesmosis ossification may be sympt

MRI of trauma to the foot and ankle.

Traumatic injuries involving the foot and ankle are very common. With the advent of magnetic resonance imaging (MRI) and its unsurpassed ability for soft tissue characterization, its utility in the investigation of these patients with foot and ankle trauma has rapidly expanded over the last decade. MRI has the unique capability to evaluate osseous, ligamentous, tendinous, and muscular injuries about the foot and ankle, with a single imaging study. MRI also allows for characterization of injury based on known biomechanical patterns. Injuries to specific soft tissue structures such as the Lisfra

[The complicated ankle sprain].

Inadequate healing and rehabilitation after an ankle sprain may lead to chronic ankle instability, predisposing patients to impingement symptoms. The initial injury may also cause osteochondral lesions of the talus or peroneal tendon injuries, which are frequently underdiagnosed because symptoms overlap early with those of a simple ankle sprain. This review finds that rehabilitation remains the cornerstone of treatment for instability and impingement, while peroneal injuries are typically managed surgically. Management of osteochondral lesions depends on the type and severity.
