Enteral nutrition as a risk factor for nosocomial pneumonia.

Nutritional support of critically ill patients in the intensive care unit is important, since adverse effects of malnutrition are multiple a … ical illness, is related to gastric colonization secondary to alkalinization of stomach contents by antacids and H2-antagonists. Nosocomial pneumonia may also be related to enteral nutrition. Gastric microbial growth increases after the onset of enteral nutrition. Gastric organisms can be transmitted to the trachea and result in tracheal colonization and n …

Drug interactions with quinolones.

Numerous drug interactions with the new 4-quinolone antimicrobial agents have now been established. Many, but not all, quinolones are extens … e problems. Absorption of all quinolones from the stomach and small intestine is greatly reduced by antacids containing magnesium or aluminium salts, including sucralfate, probably as a result of the formation of nonabsorbable chelates. Cimetidine can reduce the clearance of pefloxacin (but not of ciprofloxacin) through its effects on liver metabolism, alt …

A critical review of current medical therapy for gastroesophageal reflux disease.

Gastroesophageal reflux (GER) disease is a common, multifactorial medical condition that may be difficult to treat. Simple lifestyle modifications decrease reflux episodes and should serve as the cornerstone for medical therapy. Antacids or alginic acid may be helpful in mild disease but recent studies suggest they may be no better than placebo. Metoclopramide improves symptomatic GER disease but side effects are a major limiting factor. Only bethanechol and the histamine H2 antagonists have been conclusively shown to improve esophagitis significantly. Therefore, more severe disease is current…

A comparison of rheumatoid arthritis in Australia and China.

A comparison was made of two series of consecutive outpatients with a presumptive diagnosis of rheumatoid arthritis (RA) attending referral … of the Australian series but in only 6% of the Chinese; although 25% of the Chinese were receiving antacids and 6% antiulcerants. X-rays of hands and feet showed more severe disease in the Australian series. The older age group and longer duration of the disease in the Australian patients, who had more chronic and less active disease, may have influenced …

Acute stress ulceration.

Stress ulceration is usually considered a mucosal abnormality of the oesophagus, stomach or duodenum in the critically ill. It is found to a … ossible. Other prophylactic measures currently used involve raising gastric pH above 4, with either antacids or H2 receptor antagonists. This is best achieved by measuring the gastric pH hourly and titrating it against an appropriate dose of either type of drug or a combination of both. Newer drugs, such as omeprazole, sucralfate and prostaglandins, are pr …

Hypophosphatemia.

Hypophosphatemia is a common laboratory abnormality that occurs in a wide variety of disorders. When severe and prolonged, it may be associa … ients who consume a phosphate-deficient diet in conjunction with large amounts of phosphate-binding antacids, in persons with severe, chronic alcoholism and in patients with wasting illnesses who are refed with substances containing an inadequate amount of phosphate. When severe hypophosphatemia occurs in such a setting, the clinical effects appear to be m …

[Therapy of duodenal and prepyloric ulcers with cimetidine (author's transl)].

In a double-blind randomized trial 91 patients with endoscopically confirmed duodenal and/or prepyloric ulcers received either 1000 mg cimet … he days of the second week only (p less than 0.05). Cimetidine patients consumed significantly less antacids than those receiving placebo (p less than 0.01). There was no significant correlations between ulcer healing and freedom from pain at the end of either cimetidine or placebo treatment in this study. No specific side effects referable to cimetidine w …

Double-blind study of carbenoxolone in gastric ulcer and erosions.

The effect of carbenoxolone on the healing of gastric ulcer and erosions was compared with that of placebo. The series consisted of 20 patients with chronic gastric ulcers and 20 patients with superficial erosions of the stomach. The diagnosis as well as the follow-up of the lesions were based on gastroscopic examinations. The ulcers were measured gastroscopically. A double-blind method was used. Besides carbenoxolone 50 mg or placebo three times daily, all the patients received antacids in fixed dosage for six weeks. Subjective symptoms and cardiovascular side-effects were recorded. Maximal a…

Treatment of chronic gastric ulcer a study of the treatment recived by 135 gastric ulcer patients in a western community.

A study was made of the treatment received by 135 gastric ulcer patients within one month and within six months of diagnosis. The treatment was divided into three types. Effective measures included surgery and those measures that have been shown favourably to influence the initial healing rate of chronic gastric ulcer (i.e. hospital admission and carbenoxolone sodium). Ineffective measures included those that have been shown convincingly not to accelerate ulcer healing--diet, antacids, sedatives and no treatmen at all. Anticholinergic drugs were included in the third group where the evidence i…

Interactions with the absorption of tetracyclines.

All tetracycline derivatives are bacteriostatics and their concentration in serum should not fall during the therapy below the generally accepted minimum therapeutic concentration of 0.5 to 1.5 mug/ml. Tetracyclines have a high affinity to form chelates with polyvalent metallic cations such as Fe+++, Fe++, Al+++, Mg++ and Ca++. Many of these tetracycline-metal complexes are either insoluble or otherwise poorly absorbable from the gastro-intestinal tract. Milk and other dairy products, antacids containing polyvalent cations, as well as various iron salts ingested simultaneously with tetracyclin…

Paradoxical hypomagnesemia caused by excessive ingestion of magnesium hydroxide.

Excessive ingestion of magnesium may lead to hypermagnesemia even without kidney dysfunction. Several cases of development of hypermagnesemia after overdose of magnesium hydroxide have been reported. Although magnesium hydroxide is widely used as laxative, its overdose may induce diarrhea, which is followed by excessive magnesium loss. I report a case of paradoxical hypomagnesemia developed after excessive ingestion of magnesium hydroxide. A 39-year-old woman was presented to the emergency department complaining of severe watery diarrhea and carpopedal spasm after ingesting a handful of magnes

Future opportunities for drug therapy in peptic ulcer disease.

Attempts to develop antisecretory agents showing a significant increase in potency and duration of action over currently available H2 antagonists have had to contend with the problem of gastric tumour induction during long-term toxicity testing in rats, and their clinical future is currently uncertain. A number of potential pharmacological approaches to ulcer therapy, unrelated to inhibition of acid secretion, can be identified upon which to base the search for new antiulcer drugs. Whether such agents are a commercially attractive proposition is debatable, given the disappointing early clinica
