Use of intracameral antibiotics prophylaxis in patients with posterior capsular rupture during cataract surgery: systematic review and meta-analysis.

TOPIC: Posterior capsular rupture (PCR) during cataract surgery predisposes patients to postoperative endophthalmitis (POE). However, little is known about the prophylactic use of intracameral antibiotics (ICA) to prevent POE after PCR. This systematic review and meta-analysis assesed the efficacy of ICA in preventing POE in patients with PCR. CLINICAL RELEVANCE: PCR increases the risk of POE by more than 6-fold, emphasizing the need for targeted prophylactic strategies. This study focused on the clinical importance of ICA in reducing the incidence of POE in this high-risk population. METHOD

Digital five-step questionnaire to enhance standard perioperative prophylaxis in surgical patients with reported allergy to beta-lactam-antibiotics.

BACKGROUND: Most self-reported beta-lactam antibiotic (BLA) allergies are inaccurate. This study evaluated a digital tool designed to reassess beta-lactam allergies and enable the use of standard perioperative antibiotic prophylaxis (PAP) without the need for prior allergy testing. METHODS: In this retrospective, observational single-center cohort study, a digital five-step questionnaire was utilized during preoperative anesthesia evaluations for surgical patients reporting beta-lactam allergies. The algorithm assessed the likelihood of a beta-lactam allergy and provided recommendations for e…

Use of Doxycycline and Other Antibiotics as Bacterial Sexually Transmitted Infection Prophylaxis in a US Sample of Primarily Gay and Bisexual Men.

BACKGROUND: Doxycycline used as postexposure prophylaxis (doxyPEP) within 72 hours of sex reduces the risk of bacterial sexually transmitted infections (STIs) in people assigned male sex at birth. Little is known about current use of antibiotics as STI prophylaxis in US populations likely to benefit from doxyPEP. METHODS: We conducted an online survey in September 2023 of US adults recruited via sexual networking apps used mainly by gay and bisexual men (GBM). Respondents were asked about the use of antibiotics around the time of sex to prevent bacterial STIs. RESULTS: Of 903 respondents, mo

Prophylaxis of bacterial infections with oral antibiotics in neutropenic patients. Lessons from the last two EORTC trials and prospects for the future.

It is well known that patients with granulocytopenia due either to bone marrow failure, acute leukemia or its treatment, or as a result of other intensive chemotherapy are at enhanced risk of serious infection. Several approaches have been designed to minimize the risk of infection in these patients by means of suppression of gastrointestinal flora. A retrospective review of infection in febrile neutropenic patients revealed a significant decrease in bacteremia in patients who had received some oral antimicrobial regimen compared with those who did not. In one large series, infection due to th

Surgical antibiotic prophylaxis in pediatric urology: Are we following the AUA guidelines? A multi-center evaluation of compliance and impact on morbidity.

INTRODUCTION: The American Urological Association (AUA) provides guidance on surgical antibiotic prophylaxis (SAP) for urologic procedures. In the absence of pediatric-specific recommendations, these guidelines are often extrapolated to children. We evaluated national compliance with SAP components (administration, antibiotic choice, timing, and re-dosing) across pediatric urologic procedures and examined associations with postoperative outcomes. METHODS: Using the ACS NSQIP-Pediatrics database (2021-2023), we analyzed five procedure categories: hypospadias/urethroplasty, endourologic stone p…

Antibiotic Prophylaxis and Postoperative Therapy in Tooth Extractions for Patients at Risk of Medication-Related Osteonecrosis of the Jaw (MRONJ): A Scoping Review.

Background/Objectives: Although several studies have reported antibiotic protocols for the prevention of medication-related osteonecrosis of the jaw (MRONJ) in patients receiving antiresorptive and/or antiangiogenic therapy following tooth extraction, it remains unclear which protocol is the most effective. Accordingly, this scoping review analyzed antibiotic use in dental extractions in these patients, focusing on whether antibiotic duration influences MRONJ occurrence. Methods: Two authors independently searched PubMed, Scopus, and Web of Science (2003-2025). Out of the 770 studies screened,

Population pharmacokinetic meta-analysis of five beta-lactams antibiotics to support dosing regimens in dogs for surgical antimicrobial prophylaxis.

The Pharmacokinetic/Pharmacodynamic (PK/PD) relationship of antimicrobial drugs (AMD) for surgical prophylaxis has been poorly studied, hampering evidence-based decision making around AMD dosing and timing. Our objective is to use PK/PD principles to inform (1) the timing of administration and (2) the interval for re-administration of AMD used peri-operatively in dogs. Raw plasma concentrations of cefazolin, cefuroxime, cefalexin, amoxicillin and ampicillin were retrieved from original intravenous studies performed in dogs. E. coli and methicillin-susceptible staphylococci were identified as p

Single-dose versus multiple-dose antibiotics prophylaxis for preventing caesarean section postpartum infections: A randomized controlled trial.

BACKGROUND: Caesarean section, a common obstetric surgical procedure, is a major predisposing factor for puerperal infections, requiring the need for antibiotic prophylaxis. Evidence suggests that single-dose antibiotic prophylaxis has comparable efficacy to multiple-dose antibiotic prophylaxis, but with a lower cost and risk of antibiotic resistance. However, single-dose antibiotic prophylaxis after caesarean section is not generally used in many centres in sub-Saharan Africa. OBJECTIVE: This study aimed to compare the effectiveness of single- versus multiple-dose antibiotic prophylaxis to p…
