Classification of atrial fibrillation as a model of decisional analysis for the treatment of patients with current atrial fibrillation observed in the emergency department.

Atrial fibrillation (AF) is the most common cardiac arrhythmia observed in the emergency room (ER). We propose a new classification of AF which is useful for the standardization of terms to be used for future clinical trials and for clinical management of this arrhythmia in the ER. We recognized three categories: (1) atrial fibrillation lasting less than 72 hours (AF < 72 h); (2) persistent atrial fibrillation and (3) permanent atrial fibrillation. Atrial fibrillation lasting less than 72 hours can be reconverted to sinus rhythm spontaneously or with pharmacological or electrical cardioversion…

Prediction of transition to chronic atrial fibrillation in patients with paroxysmal atrial fibrillation by signal-averaged electrocardiography: a prospective study.

BACKGROUND: It is well known that paroxysmal atrial fibrillation (PAF) often precedes the establishment of chronic atrial fibrillation (CAF). However, there have been no definite methods to predict the transition from PAF to CAF. The purpose of this report was to determine prospectively whether P-wave-triggered signal-averaged ECG (P-SAE) is useful for the prediction of the transition to CAF in patients with PAF. METHODS AND RESULTS: One hundred twenty-two consecutive patients with PAF were prospectively followed after P-SAE, echocardiography, and 24-hour Holter monitoring at study entry. The…

Electrophysiologic effects and efficacy of cibenzoline on stimulation-induced atrial fibrillation and flutter and implications for treatment of paroxysmal atrial fibrillation.

The effects of intravenous cibenzoline (1.5 mg/kg) on atrial vulnerability and electrophysiology were assessed in 25 patients with documented paroxysmal atrial fibrillation (AF), in whom sustained (greater than 30 seconds) AF was induced by atrial stimulation with up to 2 extrastimuli and paced rates between 100 and 180 beats/min. In 7 patients AF persisted despite the application of cibenzoline, and in 8 patients the induction of sustained AF was prevented by cibenzoline. Intraatrial conduction time, flutter cycle length and shortest ventricular cycle length during AF were increased by cibenz…

Characterization of Paroxysmal Atrial Fibrillation in Atrial Septal Defects: Contribution of a Left Atrial Substrate.

BACKGROUND: Atrial septal defect (ASD) is associated with severe right atrial dilatation, which may contribute to the arrhythmogenic substrate underlying paroxysmal atrial fibrillation (PAF). However, detailed characterization of electrical properties has not been systemically performed in these patients with PAF. OBJECTIVES: The purpose of this study was to explore differences in the prevalence of potential fractionation and low-voltage areas between uncorrected adult ASD patients with PAF and control patients, to elucidate predilection sites underlying PAF in ASD patients. METHODS: Epicard…

The role of atrial electrical remodeling in the progression of focal atrial ectopy to persistent atrial fibrillation.

Although atrial fibrillation- (AF) induced changes in atrial refractoriness (atrial electrical remodeling) have been demonstrated in a number of different animal models, the clinical significance of this process is unknown. We describe a patient in whom there has been documented progression of atrial ectopy to persistent AF accompanied by evidence of atrial electrical remodeling, with reversal of remodeling following successful ablation of the focal source of AF. A second patient with focal AF, but with a "nonfocal" appearance on the ECG, is also described. These cases illustrate: (1) the poss…

Linking psoriasis to atrial fibrillation: Insights from "Association between psoriasis and atrial fibrillation: A systematic review and meta-analysis".

Patients with psoriasis indeed face an elevated risk of developing atrial fibrillation (AF), underscoring the need for vigilant monitoring to facilitate early detection and intervention. Given the current body of evidence linking psoriasis and AF is derived from observational studies, there is a clear need for more robust, multicenter, large-scale randomized studies. Such research would help to validate and reinforce the findings from observational data. Large-scale, randomized studies are necessary to confirm the association between psoriasis and AF, addressing potential biases and confoundin…

Biomarkers of Atrial Fibrillation Recurrence in Patients with Paroxysmal or Persistent Atrial Fibrillation Following External Direct Current Electrical Cardioversion.

Atrial fibrillation (AF) is associated with atrial remodeling, cardiac dysfunction, and poor clinical outcomes. External direct current electrical cardioversion is a well-developed urgent treatment strategy for patients presenting with recent-onset AF. However, there is a lack of accurate predictive serum biomarkers to identify the risks of AF relapse after electrical cardioversion. We reviewed the currently available data and interpreted the findings of several studies revealing biomarkers for crucial elements in the pathogenesis of AF and affecting cardiac remodeling, fibrosis, inflammation,…

Prolonged P-wave duration is associated with atrial fibrillation recurrence after successful pulmonary vein isolation for paroxysmal atrial fibrillation.

UNLABELLED: Pulmonary vein isolation (PVI) for paroxysmal atrial fibrillation (PAF) is successful in ∼70-80 % after repeated procedures. This suggests a subgroup of PAF patients where electrical abnormalities outside the pulmonary veins are important. Prolonged P-wave duration (PWD), a marker of atrial remodelling, may identify this subgroup. This study's aim was to assess the association of PWD on standard 12 lead ECG with AF recurrence post PVI. METHODS: Retrospectively, ECGs were blindly analysed on PVI patients from August 2007-August 2011; patients with persistent AF, mitral valve diseas…

Increased concentration of plasma cyclic GMP from aconitine-induced atrial fibrillation in dogs and patients with paroxysmal atrial fibrillation.

The concentrations of plasma cyclic nucleotides in patients with atrial fibrillation and in dogs with aconitine-induced atrial fibrillation were measured. Radioimmunoassay was used for the determination of cyclic nucleotides. The concentration of plasma cGMP in dogs with aconitine-induced atrial fibrillation was significantly elevated at 30 and 60 min after the onset of atrial fibrillation, whereas that of plasma cAMP showed no significant changes. After the injection of atropine in dogs with atrial fibrillation and with increased plasma cGMP, the plasma levels decreased to the normal level at…
