A Cross-cultural Investigation of the Factors and Biases Involved in the Allocation of Scarce Medical Resources.

In this study Spanish and English lay participants were given minimal demographic information regarding 16 hypothetical patients, and were required to rank them in order of priority for a kidney dialysis machine. The patients differed in terms of their gender, smoking behaviour, political affiliation and place of birth. These factors were combined factorially, so that each of the 16 patients presented a unique combination of attributes. The English sample showed a preference for local, female, non-smoking patients. The results for the Spanish sample were similar with respect to place of birth

The incidence of ovarian cancer in Sweden, 1960-1984.

Trends in the incidence of ovarian cancer among 23,239 cases reported to the Swedish Cancer Registry from 1960 through 1984 were analyzed. Trends in age-standardized incidence rates were best described by a nonlinear model with a highly significant negative second-order term, which showed an initial increase followed by stabilization and, in later years, a decline. In multivariate models, both time period of diagnosis and birth cohort were found to have significant effects. These separate models provided consistent evidence for an increasing risk in women born during the last decades of the 19

Multilevel modelling of factors influencing eight or more antenatal care visits among women of reproductive age in South Africa.

Globally, thousands of women die due to pregnancy-related complications. The World Health Organization recommends at least eight or more ant … sits. The findings revealed a low use (17.1%) of eight or more antenatal care visits. Age at birth, birth order, population group, marital status, employment status, media exposure, place of residence, household wealth and province were statistically associated with eight or more antenatal care visits. Women whose age at birth was 20-34, those exposed to the …

Prevalence of and Risk Factors for Iron Deficiency in Twin and Singleton Newborns.

Iron deficiency (ID) in utero and in infancy can cause irreversible neurocognitive damage. Iron status is not routinely tested at birth, so the burden of neonatal ID in the United States is unknown. Infants born from twin or higher-order pregnancies may be at elevated risk of inadequate nutrient endowment at birth. The present study sought to compare the burden of neonatal ID in cord blood serum samples from twin (n = 54) and singleton pregnancies (n = 24). Iron status (serum ferritin (SF), soluble transferrin receptor (sTfR), hepcidin) and inflammation (C-reactive protein (CRP) and interleuki

Objective. The increase of BMI before pregnancy and during pregnancy will lead to hypertensive disorder in pregnancy (HDP) and abnormal glycolipid metabolism, as well as increase the risk of neonatal weight abnormalities and adverse pregnancy outcome. This study retrospectively analyzed the clinical data of 358 women who were admitted to the obstetrics department of our hospital from January 2018 to July 2019. And the relationship between prepregnancy BMI, BMI increase during pregnancy and delivery methods, postpartum hemorrhage, neonatal weight, premature delivery, neonatal asphyxia, gestatio

Maternal disease and gasotransmitters.

The three known gasotransmitters, nitric oxide, carbon monoxide, and hydrogen sulfide are involved in key processes throughout pregnancy. Gasotransmitters are known to impact on smooth muscle tone, regulation of immune responses, and oxidative state of cells and their component molecules. Failure of the systems that tightly regulate gasotransmitter production and downstream effects are thought to contribute to common maternal diseases such as preeclampsia and preterm birth. Normal pregnancy-related changes in uterine blood flow depend heavily on gasotransmitter signaling. In preeclampsia, endo

[Gastroschisis and omphalocele. Treatment and results].

Between 1976 and 1988, 28 neonates with gastroschisis and 44 with omphalocele were treated. In 44 patients primary closure was achieved, while silastic prostheses were used in 23 patients, and dura implant in three patients. Two patients were not treated. The mortality rate was 8% (4/52) among the patients without serious congenital defects and birth weights over 1,800 g. This mortality was mainly caused by bowel infarction and, in infants with birth weights below 1,800 g, by the respiratory distress syndrome. Our experience suggests that ventilatory assistance with total paralysis is mandator

Estimation of Shear Wave Speed in the Rhesus Macaques' Uterine Cervix.

Cervical softness is a critical parameter in pregnancy. Clinically, preterm birth is associated with premature cervical softening and postdates birth is associated with delayed cervical softening. In practice, the assessment of softness is subjective, based on digital examination. Fortunately, objective, quantitative techniques to assess softness, and other parameters associated with microstructural cervical change are emerging. One of these is shear wave speed (SWS) estimation. In principle, this allows objective characterization of stiffness because waves travel more slowly in softer tissue.

Tormented by ghosts from their past': a meta-synthesis to explore the psychosocial implications of a traumatic birth on maternal well-being.

BACKGROUND: women can experience an array of serious and enduring morbidities following a difficult or traumatic childbirth. These complications have a negative impact on maternal behaviours and infant and family well-being. OBJECTIVE: to undertake a meta-synthesis of existing qualitative research to explore the psychosocial implications of a traumatic birth on maternal well-being. METHOD: a systematic review across 10 databases was undertaken: Nursing and Allied Health Source, Medline, the Allied and Complementary Medicine Database (AMED), Embase, PsychINFO, Cumulative Index of Nursing and

Obstetric-induced incontinence: a black hole of preventable morbidity.

There is a detailed literature comprising clinical and anorectal physiological studies linking faecal incontinence to vaginal delivery. Specific risk factors are high infant birthweight, forceps delivery and prolonged second stage of labour. The onset of symptoms may be delayed for many years. Faecal incontinence occurs in more than 10% of adult females and urinary incontinence in about a third of multiparous women. This places a very large economic burden on the Australian health system. A conservative estimate for overall management of incontinence would be in excess of $A700 million but the

[Methods for parturition monitoring in the mare - an overview].

Various systems are available for birth monitoring in horses, whereby a distinction must be made between methods for more accurate prediction of the date of birth in order to intensify monitoring of the mare in a timely manner as well as methods for detecting individuals that are in labor. Basically, it should be noted that there are almost no studies that compare different methods on the same population of mares. As the time of birth approaches, physiological parameters of mare and fetus change, but their variability is too high to predict the exact parturition time point prospectively. The b

Effects of Extreme Prematurity on Numerical Skills and Executive Function in Kindergarten Children: An Application of Partially Ordered Classification Modeling.

Although mathematics disabilities (MD) are common in extremely preterm/extremely low birth weight (EPT/ELBW) children, little is known about the nature of these problems. In this study partially ordered set (POSET) models were applied to classify 140 EPT/ELBW kindergarten children (gestational age <28 weeks and/or birth weight <1000 g) and 110 normal birth weight (NBW) controls into profiles of numerical and cognitive skills. Models based on five numerical skills and five executive function and processing speed skills provided a good fit to performance data. The EPT/ELBW group had poorer skill

A wider pelvis does not increase locomotor cost in humans, with implications for the evolution of childbirth.

The shape of the human female pelvis is thought to reflect an evolutionary trade-off between two competing demands: a pelvis wide enough to permit the birth of large-brained infants, and narrow enough for efficient bipedal locomotion. This trade-off, known as the obstetrical dilemma, is invoked to explain the relative difficulty of human childbirth and differences in locomotor performance between men and women. The basis for the obstetrical dilemma is a standard static biomechanical model that predicts wider pelves in females increase the metabolic cost of locomotion by decreasing the effectiv

Short Stature Screening by Accurate Length Measurement in Infants with a Birth Weight <9th Centile.

BACKGROUND/AIMS: Intrauterine growth restriction is an indication for growth hormone treatment. Birth length (BL) is needed to evaluate the influence of birth size on childhood short stature. However, BL is commonly measured only approximately, if at all. A single-centre study was undertaken to determine the value of measuring accurate and targeted BL and parental height (PH) for neonates with a birth weight (BW) ≤9th centile, identifying short [BL ≤-2 standard deviation scores (SDS)] and light newborns (BW ≤-2 SDS), and remeasuring short neonates at 2 years in order to detect those not showin

In multivariate analysis, babies delivered via Cesarean section had more CD34+ cells and volume, but lower TNCs. Heterogeneous rates for birth defects in Latin America: hints on causality.

The aim of this work was to disclose risk factors associated with birth defects which were heterogeneously distributed in the different geographic regions sampled by the Latin American Collaborative Study of Congenital Malformations (ECLAMC). The material included 2,159,065 hospital births, delivered in the 1967-1989 period in 24 geographic regions of Latin America. Birth defect types with 50 case-control pairs or more were analyzed. A risk factor was defined as that available variable with differential geographic rates, correlated with those of a given birth defect type. Identified factors we
