Prophylactic antigen-matched donor blood for patients with warm autoantibodies: an algorithm for transfusion management.

BACKGROUND: Patients with warm autoantibodies are at high risk for delayed hemolytic transfusion reactions due to the presence of alloantibodies. To provide blood safe for transfusion and to avoid adsorption studies in some cases, the provision of prophylactic antigen-matched donor blood where feasible for patients with warm autoantibodies is advocated. STUDY DESIGN AND METHODS: Twenty consecutive adult patients with warm autoantibodies (January 1999 to February 2000) received chronic RBC transfusions by use of this protocol: the serology consistent with warm autoantibodies was confirmed; the

[New crossmatch procedure: computer crossmatch experience in a blood service division].

We established a computer crossmatch system in our hospital to commence 24-hour-a-day blood services. About 97% of red blood products were shipped using this system. Among them, only one patient developed a mild delayed hemolysis. ABO-mismatched transfusion or release of incompatible blood components was not reported at all. Since the turnaround time is shortened, cost reductions were achieved by reductions in blood units tied up in the crossmatched blood inventory, leading to reduced blood ordering.

Transfusion and lung injury.

The respiratory tree has been viewed as an infrequent site of injury arising as a complication of transfusion. In recent years, this view has changed as investigators have shown that two complications--circulatory overload and transfusion-related acute lung injury--are relatively frequent events. Circulatory overload is a result of hypertransfusion to individuals at risk, the very young or old recipient. The reaction is due to fluid infusion which overwhelms the capacity of the left ventricle, resulting in pulmonary edema. While rarely fatal, studies have shown that such incidents result in in

Predictors of Intraoperative Frozen Plasma Transfusion in Liver Transplantation.

BACKGROUND: Liver transplantation (LT) is often accompanied by large intraoperative transfusions of blood products and worsens the patient's prognosis. The aim of our study was to identify the predictors of intraoperative frozen plasma (FP) transfusion in LT. At the same time, the effect of intraoperative plasma transfusion on clinical outcomes was evaluated as well. METHODS: Our retrospective study included 114 adult patients undergoing LT between January 1, 2020 and November 30, 2021. Patient demographics, including age, gender, and weight, were acquired. Patients were classified into one o

A novel in vivo ovine model of transfusion-related acute lung injury (TRALI).

BACKGROUND AND OBJECTIVES: Even with the introduction of specific risk-reduction strategies, transfusion-related acute lung injury (TRALI) continues to be a leading cause of transfusion-related morbidity and mortality. Existing small animal models have not yet investigated TRALI resulting from the infusion of heat-treated supernatant from whole blood platelet concentrates. In this study, our objective was the development of a novel in vivo two-event model of TRALI in sheep. MATERIALS AND METHODS: Lipopolysaccharide (LPS; 15 μg/kg) as a first event, modelled clinical infection. Transfusion (es

Meningococcal septicaemia in a C6-deficient patient and effects of plasma transfusion on lipopolysaccharide release.

Patients whose blood is deficient in the terminal component of complement have an increased susceptibility to meningococcal infection. However, mortality from meningococcal infection is lower in these patients than in immunocompetent subjects. We studied a C6-deficient patient with meningococcal sepsis who received fresh frozen plasma (FFP). The patient's initial plasma endotoxin, C6, and terminal-complement-complex concentrations were low, but rose sharply after treatment with FFP. Samples of the patient's serum taken shortly after admission did not cause endotoxin release from Escherichia co

Biologic effects of leukocytes present in transfused cellular blood products.

A considerable literature has accumulated over the past decade indicating that leukocytes present in allogeneic cellular blood components, intended for transfusion, are associated with adverse effects to the recipient. These include the development of febrile transfusion reactions, graft-versus-host disease, alloimmunization to leukocyte antigens, and the immunomodulatory effects that might influence the prognosis of patients with a malignancy. Moreover, it has become evident that such leukocytes may be the vector of infectious agents such as CMV, HTLV-I/II, and EBV as well as other viruses. A

Anti-HBs positive occult hepatitis B virus carrier blood infectious in two transfusion recipients.

BACKGROUND/AIMS: Occult hepatitis B infection (OBI) in blood donations is not considered infectious when anti-HBs is present. METHODS: Four months after transfusion of eight blood components during coronary arterial bypass surgery, a 59-year-old patient developed acute hepatitis B. A second 71-year-old patient transfused with a red cell concentrate (RCC) from one of these donations had early HBV infection 7 months post-transfusion. Samples were tested for HBV serological markers and HBV DNA was quantified and sequenced. RESULTS: One implicated donation contained anti-HBc, anti-HBs (12 IU/L)

Mitigating the Risk of Transfusion-Transmitted Dengue in Australia.

Dengue viruses (DENV 1-4) are a risk to transfusion safety, with several transfusion-transmitted (TT) cases reported globally. DENV 1-4 are endemic in over 100 countries, with seasonal outbreaks occurring in northeastern Australia. To mitigate TT-DENV risk in Australia, fresh blood components are not manufactured from donors returning from any area (domestic/overseas) with known dengue transmission. Alternatively, TT-DENV risk may be mitigated using an appropriate blood donor screening assay. We aimed to determine the rate of dengue infection in donors during dengue outbreaks in Australia. Pla

Utilization and safety outcomes following the use of low titer group O whole blood in non-trauma patients.

BACKGROUND: Low Titer Group O Whole Blood (LTOWB) use has expanded to include patients with non-traumatic bleeding etiologies. While a large body of evidence supports a potential survival benefit of LTOWB in trauma patients, data demonstrating the safety and efficacy of LTOWB in non-trauma patients are lacking. STUDY DESIGN AND METHODS: Non-trauma adult patients at two hospitals who received at least one unit of LTOWB were included in the study. Patient demographics, transfusion and laboratory data, diagnoses, mortality, and hospital length of stay were collected between January 1, 2018, and

Fibrin glue in managing intractable gingival bleeding in patients with inherited bleeding disorders-a quasi-experimental pilot study.

This study presents the clinical outcomes of using inhouse prepared fibrin glue for controlling gingival bleeding in patients with inherited bleeding disorders (IBD). The objective of the study was to assess the reduction in transfusion days and improvement in compliance for dental evaluation over a one-year period in a low-to-middle-income country. The quasiexperimental pilot study included 40 IBD patients with gingival bleeding. These were divided into two groups: Group A received fibrin glue (n=20), while Group B did not (n=20). The study compared outcome metrics, including the number of tr

Seven years (2015-2021) of blood donor screening for HEV-RNA in France: lessons and perspectives.

BACKGROUND: The French health authorities are considering expanding the current selective hepatitis E virus (HEV)-RNA testing procedure to include all donations in order to further reduce transfusion-transmitted HEV infection. Data obtained from blood donors (BDs) tested for HEV-RNA between 2015 and 2021 were used to assess the most efficient nucleic acid testing (NAT) strategy. MATERIALS AND METHODS: Viral loads (VLs) and the plasma volume of blood components, as well as an HEV-RNA dose of 3.85 log IU as the infectious threshold and an assay with a 95% limit of detection (LOD) at 17 IU/mL, w

HIV seroprevalence in hematologic patients other than hemophiliacs at Ramathibodi Hospital.

Recently there have been increasing reports of HIV infection acquired through transfusion of HIV seronegative blood in Thailand due to high incidence of HIV new infection in blood donors. Blood or blood components (BC) prepared from HIV seronegative blood donation pose significant hazards to recipients because of the risk of viremia during the "window period" of HIV infection. This paper presents the HIV seroprevalence in hematologic patients other than hemophiliacs who received multiple blood transfusion at Ramathibodi Hospital. The retrospective analysis was done on 167 patients: 132 thalass

Viral infections transmitted by blood and its products.

Modern transfusion practice is associated with an increased risk of transmitting viral agents because of the changing nature of the patients and of the therapeutic blood products. More immunosuppressed patients are receiving blood released faster and with more elaborate blood components. In addition to the classically recognized importance of hepatitis B virus (itself disseminated most efficiently by contamination of products derived from large pools of plasma containing many donations) other agents are assuming increasing importance. They frequently display one or more of the predisposing cha

Implementation and evaluation of a community walking blood bank for haemorrhagic shock when banked blood is unavailable: protocol for a mixed-methods, effectiveness-implementation study - phase 2 of the Local Initiative for emergency blood (LIFE-Blood) study.

INTRODUCTION: Trauma, obstetric haemorrhage and severe anaemia lead to millions of deaths every year. Many of these deaths occur in regions known as 'blood deserts' where there is virtually no access to blood transfusions. A community or civilian walking blood bank (CWBB) is a low-resource strategy that can provide just-in-time, point-of-care tested blood transfusions in blood deserts when banked blood is not readily available and the alternative is almost-certain death. This protocol is designed to evaluate the effectiveness and implementation of a CWBB at Lodwar County Referral Hospital loca

Clinical presentation of delayed hemolytic transfusion reactions and hyperhemolysis in sickle cell disease.

Red blood cell (RBC) transfusion therapy is a key component in the comprehensive management of patients with sickle cell disease (SCD). Consequently, most adult SCD patients will receive at least one, and many will receive more than a hundred RBC transfusions in their lifetime. SCD patients develop RBC alloantibodies much more frequently than non-SCD transfused patients, which often make the selection of compatible RBCs extremely difficult, in addition to placing patients at significantly higher risk of suffering from delayed hemolytic transfusion reactions (DHTRs). Similar to alloimunization,

Testosterone-Associated Erythrocytosis and Blood Donation: Therapeutic Phlebotomy, Donor Safety, and Recipient Exposure.

Testosterone replacement therapy (TRT) is increasingly utilized and associated with erythrocytosis, leading some individuals to undergo therapeutic phlebotomy or present for blood donation. This emerging intersection between clinical management and blood donation raises important questions regarding donor eligibility, donor safety, recipient exposure, and regulatory oversight. This review examines evidence on testosterone-associated erythrocytosis in the context of blood donation. We review the pathophysiologic mechanisms underlying testosterone-induced erythrocytosis, summarize the current re
