PURPOSE: The aim of our study was to assess the outcome of locking plate fixation of distal tibia fractures and evaluate which surgical approach and method of plate fixation is related to better functional result and lower complication rate.
METHODS: A retrospective analysis of treatment of 45 patients was made. Patients were divided into two subgroups regarding surgical approach (medial vs anterior) and fixation method (bridge plating vs rigid fixation). Time from injury to full bone union was noted, and clinical outcome was assessed by AOFAS score.
RESULTS: Nonunion was the most prevalent

It is often the case that screws cannot be firmly anchored in small, marginal joint fragments. This is especially relevant in osteoporotic bone. By redesigning the last plate hole of a one-third tubular plate to function as a hook, these bone fragments can be securely grasped and stabilized. Placing two plates on top of one another further improves mechanical strength.

Rickets and osteomalacia are diseases characterized by impaired mineralization of bone matrix. The same causes can result in rickets and osteomalacia. Of these, rickets develops before the closure of growth plates and presents bone deformities and growth retardation. Diagnostic criteria for rickets and osteomalacia have been published in Japan.

Hook plate fixation and locking plate fixation are two standard internal fixation implants for treating distal clavicle fractures. We aimed to clarify the following: 1) Does the locking plate offer better clinical outcomes than the hook plate? 2) Is bone union better with a locking plate than hook plate? and 3) Are complications different between the locking plate and hook plate? We conducted a retrospective multicenter study of 338 patients who underwent surgery from 2014 to 2018 in our 10 hospitals, which comprise the TRON group. Of them, 208 patients treated using any plates were eligible.

Park-Harris lines are radiographically apparent linear opacities that occur in a metaphyseal bone in proximity to a physis. Lines correspond to a temporary interruption of the normally ossifying physis with a sclerotic line corresponding to transversely oriented trabeculae. They were first appreciated in the long bones of diabetic children and have since been described in various metabolic, endocrinologic, infectious, neoplastic, and posttraumatic conditions as well as in response to systemic medications. Park-Harris lines are clinically useful in demarcating notable events in whole-body or in

Locking plates were initially designed to provide improved stability to ankle fractures with poor bone quality but are currently widely used. The aim of this study was to compare the reoperation risk when using locking plates compared with nonlocking plates in patients with simple ankle fractures. This study was a population-based register study. Data regarding patients with AO type 44A1/2 and 44B1/2 injuries who were treated with either locking or nonlocking plates were obtained from the Danish Fracture Database. The follow-up period was 24 months. Major complications were defined as complica

The treatment of skeletal Class III malocclusion in adolescents is challenging. Maxillary protraction, particularly that using bone anchorage, has been proven to be an effective method for the stimulation of maxillary growth. However, the conventional procedure, which involves the surgical implantation of mini-plates, is traumatic and associated with a high risk. Three-dimensional (3D) digital technology offers the possibility of individualized treatment. Customized miniplates can be designed according to the shape of the maxillary surface and the positions of the roots on cone-beam computed t

Nonunion and secondary reduction loss complicate open distal femur fractures with bone loss. The authors hypothesized that locking plates decrease subsequent bone grafting yet maintain alignment and immediate postfixation radiographic features predict primary union. A retrospective chart/radiographic review was performed at a Level 1 university trauma center. Thirty-four adults with 36 open AO/Orthopaedic Trauma Association (AO/OTA) C-type distal femur fractures were studied. All fractures were treated with open reduction, internal fixation with a lateral locked implant with or without antibio

Fracture fixation with standard locked plates can suppress interfragmentary motion beneficial for secondary bone healing. To address this limitation, dynamic fracture fixation plates have been developed which seek to maintain bending and torsional rigidity while providing controlled axial micromotion. This article provides a comprehensive systematic review of the history and current state of proposed dynamic plating technologies to better inform future development. 59 records (51 articles, 8 patents) describing 26 unique dynamic plating devices were identified across three literature and paten

Radius is a critical bone for functioning of the forearm and therefore its reconstruction following fracture of its shaft must be anatomical in all planes and along all axes. The method of choice is plate fixation. However, it is still associated with a number of unnecessary complications that were not resolved even by introduction of locking plates, but rather the opposite. All the more it is surprising that discussions about anatomical and biomechanical principles of plate fixation have been reduced to minimum or even neglected in the current literature. This applies primarily to the choice

PURPOSE: To examine growth plates of the distal femur and tibia with magnetic resonance (MR) imaging to detect bone bridges and other deformities in children.
MATERIALS AND METHODS: Thirteen children (nine boys and four girls, aged 5-13 years; mean age, 9.8 years) were referred because of suspected or known bone bridging of the growth plate. Among the 13 patients, 10 had Salter-Harris fractures of the knee or ankle, two had Blount disease, and one had neonatal sepsis. Fat-saturated spoiled gradient-recalled images enabled reconstruction of a three-dimensional model of the growth plate. Patien

Carbon fiber-reinforced polyetheretherketone (CFR-PEEK) plates have emerged as alternatives to titanium locking plates for fixation of upper-limb fractures. Advantages may include an elastic modulus closer to bone, fewer imaging artefacts, and reduced stress shielding. This systematic review synthesizes comparative clinical evidence. We analyzed data from five comparative studies (n = 210 patients; CFR-PEEK = 108, titanium = 102) provided in the dataset. Searches of PubMed and the Cochrane Library identified relevant trials. Data were extracted for demographics, interventions, functional outco

This study retrospectively analyzed 112 patients who underwent mandibular reconstruction with vascularized iliac crest flaps using different fixation strategies in routine clinical practice. Under comparable defect lengths and heights, reconstruction using mini plates was associated with the shortest operative time (p < 0.001), while postoperative drainage duration showed no significant difference among groups (p = 0.958). The number of iliac bone segments varied significantly among fixation methods: reconstruction plates (alone or combined with mini plates) were predominantly applied in multi

Self-reinforced polylactic acid or polylactide (SR-PLA) is a biodegradable polymer, which is strong enough to fix weight-bearing cortical bone fractures and osteotomies. We report our experience and follow-up of the use of SR-PLA plates in 15 clinical cranioplasties. Two of the cases are described in greater detail. In one of them SR-PLA plates were used in addition to titanium plates, and in the other an SR-PLA plate fixed with mini-titanium screws was used as the only fixation material. In the other patients SR-PLA plates or wire were used in addition to titanium. So far no complications hav

Background and purpose - Temporary hemiepiphysiodesis for growth modulation in skeletally immature patients is a long-known technique. Recently the use of tension-band devices has become popular. This study compares 2 tension-band implants (eight-Plate and FlexTack) regarding their effects on the growth plate.Animals and methods - 12 pigs in 2 equally sized groups (A and B) were investigated. The right proximal medial tibia was treated with either eight-Plate or FlexTack. The left tibia of the same pig was treated with the opposite implant. After 9 weeks all implants were removed. Animals in g

Vascularized medial femoral condyle (MFC) flap transfer is a technique that may be used to reconstruct recalcitrant distal radius nonunion. Although union rates are high, the flap provides thin cortical bone requiring rigid fixation. The long-term strengthening and structural integrity of the MFC flap bone remains understudied. We describe two cases of delayed structural failure following distal radius MFC reconstruction. Two patients underwent distal radius reconstruction using vascularized corticocancellous MFC flaps stabilized with spanning fixed-angle volar plates. Both achieved radiograph

Current fixation plates for bone fracture treatments are built with biocompatible metallic materials such as stainless steel, titanium, and its alloys (e.g., Ti6Al4V). The stiffness mismatch between the metallic material of the plate and the host bone leads to stress shielding phenomena, bone loss, and healing deficiency. This paper explores the use of three dimensional topology-optimization, based on compliance (i.e., strain energy) minimization, reshaping the design domain of three locking compression plates (four-screw holes, six-screw holes, and eight-screw holes), considering different vo
