Multiple abscesses of the left brain hemisphere due to Listeria monocytogenes in an immunocompromised patient: a case report.

We describe a case of brain abscesses in a cirrhotic and diabetic 57-year-old woman showing fever, aphasia, right hemiparesis and seizures. Neuroradiological investigation revealed unilateral cerebritis evolving in multiple abscesses. From blood and surgical drainage samples Listeria monocytogenes grew in pure culture. Despite decompressive craniotomy, the patient died two months after hospital admission.

Tuberculous brain abscess and subdural empyema in an immunocompetent child: Significance of AFB staining in aspirated pus.

Tuberculous brain abscess and subdural empyema are extremely rare manifestations of central nervous system tuberculosis. Here, we report a case of an 11-year-old immunocompetent child who developed temporal lobe abscess and subdural empyema following chronic otitis media. A right temporal craniotomy was performed and the abscess was excised. The Ziehl Nielsen staining of the aspirated pus from the temporal lobe abscess yielded acid fast bacilli. Prompt administration of antituberculous treatment resulted in complete recovery of the child. Even though the subdural abscess was not drained, we pr…

Salmonella enteritidis brain abscess in a sickle cell disease patient: case report and review of the literature.

BACKGROUND: Focal intracranial abscesses due to Salmonella spp are rarely reported. They tend to occur in patients who are immunosuppressed and in those with other predisposing factors. We present herein the first reported case of Salmonella enteritidis brain abscess in a sickle cell disease (SCD) patient. METHODS: We describe the case of a 29-year-old black African female with SCD who presented to her local hospital with a left frontal abscess. She was treated with emergency burr hole aspiration of the abscess and antibiotics. The aspirate grew S. enteritidis PT 8 on culture. All investigati…

Echinococcosis presenting as an otogenic brain abscess: an unusual lesion of the middle ear cleft and temporal lobe.

This paper presents a case of a 28-year-old male with a seizure episode and a 4-year history of intermittent tinnitus on the left ear. On computed tomography and magnetic resonance imaging, a density with rim enhancement was found at the temporal lobe, associated with mastoid tegmen destruction and middle ear mass, indicating cholesteatoma with complicating brain abscess. Evacuation of the brain abscess was performed with a combined otolaryngologic and neurosurgical procedures (canal wall-down mastoidectomy and temporal craniotomy). The pathology turned out to be infestation with Echinococcus …

[Papilloedema and brain abscess associated with isolated left superior vena cava draining in to the left atrium].

BACKGROUND: Papilloedema and brain abscess associated with persistent left superior vena cava draining into the left atrium without sinus coronarius and atrium septum defect is rare and until now in only few cases have been reported. PATIENT: A 12 year-old girl was admitted because she had suffered from headache and blurred vision for 2 weeks. Fundus examination revealed papilloedema with peripapillary intraretinal haemorrhages. Pupillary reaction to light was normal. An afferent pupillary defect was negative. Neurological examination was without any focal neurological signs. Cerebral magneti…

Successful liver transplantation for congenital absence of the portal vein complicated by intrapulmonary shunt and brain abscess.

A 27-day-old boy had convulsion associated with brain abscesses and severe hypoxemia at the age of 3 months. Congenital absence of the portal vein (CAPV) and some associated anomalies were detected by radiological examinations. Brain abscess and hypoxemia were thought to be serious complications resulting from CAPV and were successfully corrected by living donor liver transplantation at the age of 4 months. This is the first report of a successful transplantation indicated for intrapulmonary shunt and brain abscess in an infant with CAPV.

Characterization of Streptococcus constellatus strains recovered from a brain abscess and periodontal pockets in an immunocompromised patient.

BACKGROUND: There have been a number of reports of brain abscesses suggesting an odontogenic etiology. However, no efforts have been made to compare brain abscess isolates with isolates from the oral cavity using highly discriminative methods. We report a brain abscess caused by Streptococcus constellatus in an immunocompromised patient where oral infection (periodontitis) was suspected to be implicated. METHODS: The brain abscess and oral isolates were compared by means of one phenotypic and three genetic (restriction fragment length polymorphism [RFLP], ribotyping, and random amplified poly…

Multiple tuberculous brain abscesses in an HIV-infected patient successfully treated with HAART and antituberculous treatment.

Tuberculous brain abscesses (TBA) are an unusual clinical presentation of central nervous system tuberculosis occurring extremely infrequently in developed countries, and almost always in immunocompromised patients. Before the introduction of highly active antiretroviral therapy (HAART), TBA were associated with a high mortality rate in AIDS patients. We describe an HIV-infected patient presenting with multiple TBA who recovered completely with a combination of HAART and anti-tuberculous treatment.

Aggregatibacter aphrophilus Brain Abscess and Ventriculitis in a Young Healthy Woman: A Case Report and Literature Review.

Aggregatibacter aphrophilus is a rare cause of endocarditis and brain abscesses. Common potential risk factors include dental manipulation or disease, upper respiratory infections, proximity to animals, and heart disease. We present a case of a young woman without apparent predisposing factors who developed a deep-seated A. aphrophilus brain abscess complicated by ventriculitis and post-infectious low-pressure communicating hydrocephalus. Diagnosis was established by cerebrospinal fluid (CSF) 16S rDNA sequencing and a serum Karius® cell-free DNA test, suggesting hematogenous spread. Management…

Brain abscess as a rare complication of endovascular coiling for cerebral aneurysm: A case report and literature review.

BACKGROUND: Brain abscess following coil embolization of aneurysms is rare, with only few cases described in the literature. Herein, we report this specific case and review the existing literature on this rare infectious complication associated with the endovascular treatment of cerebral aneurysms. CASE DESCRIPTION: A 51-year-old patient developed a brain abscess 3 months after endovascular embolization of a giant internal carotid artery aneurysm. The patient experienced rapid clinical deterioration, requiring emergency surgical intervention for abscess drainage and decompressive craniectomy.…

Clinical features and outcomes of Staphylococcus aureus brain abscess in Denmark and the Netherlands: a multicentre cohort study.

BACKGROUND: Staphylococcus aureus is the second most common cause of brain abscesses, but only few studies describe the disease characteristics. OBJECTIVES: This study aimed to characterise the clinical features and outcomes of S. aureus brain abscess. METHODS: We identified all adults with a brain abscess caused by S. aureus in Denmark between 2007 and 2024, and at the Amsterdam UMC from 2016 to 2024. Unfavourable outcome was defined as Glasgow Outcome Scale scores of 1-4. Modified Poisson regression was used to assess relative risks (RR) with 95% confidence intervals (CI) for an unfavourab…

Early diagnosis of disseminated nocardiosis in a case of pyomyositis and brain abscesses exemplifies the importance of smear examination.

We report a 73-year-old immunocompromised man with multiple muscle abscesses, in a toxic state with neurological deficits, with a history of panhypopituitarism, diabetes, hypothyroidism, and recent SARS-CoV-2 infection. MRI showed abscesses in the right chest wall, right thigh (peri-implant), and left adductor. Elevated inflammatory markers, comorbidities and clinical presentation suggested disseminated fungal or Nocardial infection. Immediate aspiration of the anterior chest wall abscess and protocolized sample collection identified Nocardial filaments in the smear within 4 hours, allowing fo

Brain abscesses caused by Listeria monocytogenes in a patient with myasthenia gravis - Case report and systematic review.

BACKGROUND: Listeriosis is a known cause of community acquired bacterial meningitis/meningoencephalitis. It occurs more frequently in neonates, older people and immunocompromised hosts. Rarely, brain abscesses can complicate the course of infection, which poses a difficulty in the management and elevates morbidity and mortality. Myasthenia gravis is an autoimmune disease that often requires immunosuppressive treatment, which increases the risk for invasive infections. CASE DESCRIPTION: A 75-year-old myasthenic patient, treated with prednisone and pyridostigmine, was diagnosed with invasive li…

Nocardia cyriacigeorgica brain abscess in a patient on low dose steroids: a case report and review of the literature.

BACKGROUND: Nocardia cyriacigeorgica was first described in 2001. It is an emerging pathogen that mainly affects immunocompromised patients. A brain abscess caused by N. cyriacigeorgica has been reported only in immunocompromised hosts. We present a rare case of brain abscess caused by N. cyriacigeorgica in an adult male receiving low dose steroids. CASE PRESENTATION: A 75-year-old male weekend gardener without an immunocompromising condition presented with neurological complaints that were initially attributed to an ischemic stroke. Due to the unusual presentation and rapid progression, his …
