Classifying episodes in schizophrenia and bipolar disorder: criteria for relapse and remission applied to recent-onset samples.

Research on predicting and preventing episodes of schizophrenia and mood disorder lacks consistent, specific definitions of episodes. We present an operational system for identifying relapse, exacerbation, and remission of schizophrenia and bipolar disorder within longitudinal studies that involve repeated symptom assessments. Three major classes of episodic outcome are defined: relapse or significant exacerbation, nonrelapse, and stable, severe persisting symptoms. These major classes are further subdivided to distinguish nine categories of episodic outcome. To examine ease of use, interrater

Video testimony of long-term hospitalized psychiatrically ill Holocaust survivors.

OBJECTIVE: Many Holocaust survivors who have both psychotic disorders and residual symptoms of posttraumatic stress disorder (PTSD) remain chronically hospitalized in psychiatric institutions. This study investigated the clinical benefits of a therapeutic process facilitating a detailed videotaped account of traumatic experience (testimony method) in elderly long-term hospitalized Holocaust survivors. METHOD: Twenty-four schizophrenia patients (mean age=72.2 years) who were chronically hospitalized in Israeli state psychiatric hospitals underwent assessment by blind rating with a battery of p

Developing a 10-item mania scale from the Parent General Behavior Inventory for children and adolescents.

OBJECTIVE: Bipolar disorder is being diagnosed and treated in children and adolescents at a rapidly increasing rate, despite the lack of validated instruments to help screen for the condition or differentiate it from more common disorders. The goal of the present study was to develop and validate a brief (10 item) instrument to assess mania in a large sample of outpatients presenting with a variety of different DSM-IV diagnoses, including frequent comorbid conditions. METHOD: Parents presenting to a Midwestern academic outpatient medical center for psychiatric evaluation of their child comple

Undoing Self-Contempt in Borderline Personality Disorder: A Controlled Test of Change in Emotion Processing in Psychotherapy.

BACKGROUND: Emotion processing has been discussed as a mechanism of change in several forms of psychotherapy for a broad range of psychological disorders. The general conclusions of the field do not allow for a step-by-step granularity of relevant emotional experiences in clients. This is particularly important for personality disorders, especially borderline personality disorder (BPD). Using a controlled out-of-therapy assessment, we assume that the lessening of expressed self-contempt by clients gives way to proportionally more expressions of self-compassion and assertive anger after a brief

The prevalence of major depressive disorder and low self-esteem in medical inpatients.

OBJECTIVE: To determine the prevalence of major depressive disorder in acutely ill medical inpatients, and the relationship of this to low self-esteem. METHOD: A total of 186 patients were interviewed 6 or 7 days following admission to detect the presence of DSM-IV major depressive disorder (MDD). Patients were assessed using a new brief psychiatric interview, the Silverstone Concise Assessment for Depression (SCAD), which has previously been validated for use in the physically ill. The cognitive function of the patients was measured, using the Mini-Mental State Examination (MMSE), with patie

Toward a Speech-Based Model of Premanifest Huntington's Disease Progression Using Deep Neural Networks.

INTRODUCTION: Huntington's disease (HD) is a progressive neurodegenerative disorder characterized by motor, cognitive, and psychiatric decline. The Unified Huntington's Disease Rating Scale Total Motor Score (UHDRS-TMS) is standard for staging manifest disease, but is relatively insensitive to subtle premanifest changes. Speech abnormalities are emerging as candidate digital biomarkers; however, reliably separating premanifest HD (preHD) from healthy controls remains challenging. Here, we assess the feasibility of a speech-only approach by training and comparing multiple classifiers across div

Use of risperidone in-situ microparticles in real world: patterns of prescription and dose selection in patients with psychotic disorders.

Long-acting injectable (LAI) antipsychotics are important for improving adherence and preventing relapse. Risperidone in-situ microparticles … tiation was primarily intended to improve adherence in 77.7% of cases. Among hospitalized patients, Brief Psychiatric Rating Scale scores decreased between admission and discharge (59.35 vs. 33.48); however, causality could not be inferred. The 100 mg group had significantly more male patients and showed higher rates of substance use and involuntary hospitalization, while the 7 …

Predictors of cognitive changes in patients with schizophrenia undergoing electroconvulsive therapy.

INTRODUCTION: Previous studies on the effects of electroconvulsive therapy (ECT) on cognition in schizophrenia have been inconclusive. This study aimed to identify factors that may predict cognitive improvement or deterioration in patients with schizophrenia after-ECT. MATERIALS & METHODS: Patients with schizophrenia or schizoaffective disorder with predominantly positive psychotic symptoms, who were treated with ECT at the Institute of Mental Health (IMH), Singapore, between January 2016 and January 2018, were assessed. Montreal Cognitive Assessment (MoCA), Brief Psychiatric Rating Scale (BP…

Randomized comparison of W.A.R.A. (Wiring Affect with ReAttach) versus distraction: A pilot study assessing the efficacy of an ultrafast transdiagnostic intervention.

BACKGROUND: Generally, neuropsychiatric patients share different symptoms across nosological categories, such as, amongst other, psychological distress, mood alterations, anxiety, and self-regulation disturbances.ReAttach is a novel psychological intervention with its key elements being external affect and arousal regulation, stimulation of multiple sensory processing, conceptualization, affective mentalization, and associative memory processing. ReAttach has been hypothesized to be effective in reducing symptom severity in different psychiatric conditions. Given the limited information curren

Genetic variants in the BDNF gene and therapeutic response to risperidone in schizophrenia patients: a pharmacogenetic study.

Risperidone is a widely used atypical antipsychotic agent that produces considerable interindividual differences in patient response. We investigated the pharmacogenetic relationship between the brain-derived neurotrophic factor (BDNF) gene and response to risperidone in 127 Han Chinese schizophrenic patients. Three functional polymorphisms, (GT)(n) dinucleotide repeat polymorphism, C-270T, and the rs6265G/A single-nucleotide polymorphism (SNP), were genotyped and analyzed for association, with reduction of Brief Psychiatric Rating Scale (BPRS) scores following an 8-week period of risperidone …

Staged treatment and acceptability guidelines in early psychosis study (STAGES): A randomized placebo controlled trial of intensive psychosocial treatment plus or minus antipsychotic medication for first-episode psychosis with low-risk of self-harm or aggression. Study protocol and baseline characteristics of participants.

AIM: It is now necessary to investigate whether recovery in psychosis is possible without the use of antipsychotic medication. This study will determine (1) whether a first-episode psychosis (FEP) group receiving intensive psychosocial interventions alone can achieve symptomatic remission and functional recovery; (2) whether prolonging the duration of untreated psychosis (DUP) in a sub-group according to randomisation will be associated with a poorer outcome and thereby establish whether the relationship between DUP and outcome is causative; and (3) whether neurobiological changes observed in
