INTRODUCTION: Primary hypoparathyroidism and adhesive capsulitis of the shoulder in the same patient does not seem to have been reported previously. We report two cases in first-degree relatives.
PATIENTS: Case 1: In 1999, a 41-year-old woman experienced seizures simulating epilepsy. She had been treated 5 years earlier for idiopathic adhesive capsulitis of both shoulders. Computed tomography of the brain showed calcifications in the basal ganglia meeting criteria for Fahr's disease, and incipient bilateral cataract was found. A diagnosis of idiopathic primary hypoparathyroidism was given. Af

Tornwaldt's bursa results from the embryological relationship of the notochord to the developing foregut. Its embryology and anatomical location is separate from Rathke's pouch which is the primordial anterior hypophysis. Trauma and subsequent inflammation of the defect leads to a complex of symptoms known as 'Tornwaldt's disease' which may be quite confusing to the clinician. Appropriate therapy which includes excision or marsupialization of the cyst will provide complete relief of the symptoms. The embryology, anatomy and therapy of this entity are discussed in detail. Other abnormalities wh

Frozen shoulder (FS) is a condition primarily marked by chronic inflammation and progressive fibrosis of the glenohumeral capsule. Clinically, it presents with persistent shoulder pain and limited joint mobility, often leading to impaired upper limb function and reduced quality of life. In recent years, research on the molecular mechanisms of fibrosis in FS has deepened; however, there remains a lack of systematic focus on the dynamic regulation of the immune microenvironment, particularly the role of macrophage heterogeneity. The remarkable functional plasticity of macrophages allows them to

BACKGROUND: Adhesive capsulitis (AC) is a debilitating condition with a higher prevalence and severity in individuals with diabetes mellitus (DM). Despite known associations, the impact of DM on AC prognosis and treatment responses remains underexplored. This structured narrative review synthesises current evidence using a structured search and selection approach to determine whether diabetes status impacts the course and treatment response of AC, and whether it should be considered when planning management strategies.
METHODS: The current review conducted a structured literature review acros

BACKGROUND: Frozen shoulder, also known as adhesive capsulitis, is a common and disabling condition that causes shoulder pain and progressive stiffness. Patient information leaflets (PILs) are produced by UK National Health Service (NHS) Trusts to help patients understand frozen shoulder and treatment options. However, the content and consistency of these PILs and their alignment with national clinical guidance are currently unclear.
OBJECTIVES: This study aimed to identify, analyse and describe the non-surgical management recommendations presented in publicly available NHS Trust PILs for fro

Adhesive capsulitis, commonly known as frozen shoulder, is a condition characterized by pain and progressive loss of shoulder motion. This condition typically affects individuals between 40 and 60 years of age and can severely impact daily functioning and quality of life. The etiology of adhesive capsulitis is multifactorial, with contributions from systemic conditions such as diabetes and thyroid disorders, as well as idiopathic factors. This review provides an overview of current concepts in the diagnosis and management of adhesive capsulitis, highlighting advancements in both conservative a

BACKGROUND: Patient reported outcome measures (PROMs) are essential to measure the patients' perspective in clinical studies. Like any measurement instrument, a PROM should be valid, reliable, and responsive. Adequate content validity relies on patient input, while construct validity can only be ensured by Modern Test Theory (MTT) models. Inadequate PROMs induce a significant risk of measurement errors. Currently, there is no thorough analysis of PROMs used in clinical research on idiopathic adhesive capsulitis (AC). The aim was to identify all PROMs used to evaluate AC, analyze their content

Frozen shoulder is a spontaneously self-resolving chronic inflammatory fibrotic human disease, which distinguishes the condition from most fibrotic diseases that are progressive and irreversible. Using single-cell analysis, we identify pro-inflammatory MERTKlowCD48+ macrophages and MERTK + LYVE1 + MRC1+ macrophages enriched for negative regulators of inflammation which co-exist in frozen shoulder capsule tissues. Micro-cultures of patient-derived cells identify integrin-mediated cell-matrix interactions between MERTK+ macrophages and pro-resolving DKK3+ and POSTN+ fibroblasts, suggesting that

BACKGROUND: Adhesive capsulitis (CA; also called Frozen shoulder) is a common, usually unilateral disease of the shoulder joint primarily affecting middle-aged women. Primary, idiopathic, and secondary forms are distinguished. Painful active and passive movement restriction are the clinically leading symptoms.
COURSE OF THE DISEASE: The disease usually progresses in three successive stages: freezing phase, frozen phase, and thawing phase.
CLINICAL DIAGNOSIS AND IMAGING: CA is primarily diagnosed clinically, with imaging being used to assess or exclude differential diagnoses. Radiography as p

INTRODUCTION: Haglund's deformity, an abnormality of the postero-superior corner of the calcaneum, is a common, critically debated cause of posterior heel pain. Several radiological indices such as Fowler-Philip angle, Ruch pitch, Chauveaux-Liet angle, calcaneal pitch angle, parallel pitch lines, and X-Y ratio have been described to measure this deformity. However, most of these lack specificity and have variable intra- and inter-observer reliability.
PURPOSE: The study aims to describe a new radiological "angle of BRINK" (Botchu-Reilly-Iyengar-Nischal-Kakarala) to measure Haglund's deformity

PURPOSE: Adhesive Capsulitis (AC) is a musculoskeletal disorder initially described by Codman in 1934. The disease is characterized by pain-limited restriction in active and passive glenohumeral range of motion (ROM) despite the lack of a structural deficit. In the last decades, arthroscopy and magnetic resonance imaging (MRI) has been the only diagnostic tools able to highlight the characteristic alterations of the glenohumeral capsular-ligament apparatus in AC; nevertheless, both arthroscopy and MRI are burdened by intrinsic limitations. The aim of this narrative review is to summarize the m

Frozen shoulder (FS), also known as adhesive capsulitis, is a painful inflammatory fibrotic disease of the glenohumeral joint capsule. While it's frequently self-limiting, patients can be symptomatic for years. The clinical course is often divided into three phases: the freezing phase with predominantly pain, the frozen phase with mainly stiffness, and the thawing phase during which the complaints slowly resolve. Diagnosing FS can be challenging during the freezing phase as the symptoms in this phase are similar to other common shoulder conditions (such as subacromial pain syndrome). Treatment

BACKGROUND: Greater trochanteric pain syndrome (GTPS), including gluteal tendinopathy and bursitis, is a common, painful and debilitating condition. It is thought to involve overload of the gluteal tendons, which may be amplified through increased hip adduction moment (HAM) seen during gait in these individuals. Reducing the aberrant HAM may improve pain and function in this population. We aimed to determine if medially-posted foot orthoses immediately reduce HAM and pain in females with GTPS during walking gait.
METHODS: A double-blind, repeated-measures trial with randomised intervention or…

Arthrofibrosis (AF) is an exaggerated immune response to a proinflammatory insult leading to pathologic periarticular fibrosis and symptomatic joint stiffness. The knee, elbow, and shoulder are particularly susceptible to AF, often in the setting of trauma, surgery, or adhesive capsulitis. Prevention through early physiotherapeutic interventions and anti-inflammatory medications remain fundamental to avoiding motion loss. Reliable nonoperative modalities exist and outcomes are improved when etiology, joint involved, and level of dysfunction are considered in the clinical decision making proces

BACKGROUND: Greater trochanteric pain syndrome (GTPS) is a musculoskeletal condition which can cause disability and reduce quality of life. However, limited evidence is available on the long-term outcomes of people with GTPS. Our aims were to determine the long-term prevalence of GTPS; to calculate the proportion of people with GTPS who had developed hip osteoarthritis (OA); and to determine the level of function and quality of life, 11-years after initial GTPS diagnosis.
METHODS: A prospective 11-year natural history study. Two groups [GTPS group (n = 24), asymptomatic control (ASC) group (n

INTRODUCTION: Frozen shoulder is a common condition, and current guidelines state that it is a diagnosis of exclusion. Along with a history and clinical examination, routine X-ray is mandated, to rule out any "masquerading" pathology such as fracture, dislocation, metastatic lesions or severe osteoarthritis. Despite the certainty of the guidelines there is a lack of evidence to support the use of routine X-rays in this situation.
METHODS: A retrospective review was performed of all X-rays obtained in the diagnosis of frozen shoulder between February 2014 and August 2017 in an integrated muscu

OBJECTIVES: The purpose of this study was to provide an updated systematic review and meta-analysis regarding the effectiveness of mobilization with movement (MWM) techniques on range of motion (ROM).
METHODS: An electronic search strategy of the Physiotherapy Evidence Database, PubMed, Cochrane Library, Embase, Google Scholar, and CINAHL was performed between August 2008 and January 2018. Two independent reviewers selected the studies. Only randomized controlled trials were included. The methodology was independently assessed by 2 reviewers using the Physiotherapy Evidence Database scale. Th

Protothecosis is a rare infection caused by achlorophyllic algae that are members of the genus Prototheca. They are ubiquitous in nature in organic material. The clinical manifestations can be acute or chronic and local or disseminated. The disease is classified as cutaneous, causing bursitis or disseminated/systemic, affecting both immunocompetent and immunosuppressed patients, with more severe and disseminated infections occurring in immunocompromised individuals. Prototheca wickerhamii and Prototheca zopfii are the most frequent organisms reported in humans. Diagnosis is made by observing a…
