Elsberg Syndrome: A Case Report of a Novel Radiographic Presentation.

Introduction: Elsberg syndrome (ES) presents with bowel and bladder dysfunction, resembling cauda equina syndrome, and is classified as a clinicoradiographic syndrome most commonly associated with HSV-2 reactivation. Most cases reveal smooth and continuous nerve enhancement on imaging. Case Description: We present a unique case of ES that presented as several nodular, ring enhancing soft tissue masses along the cauda equina. An 81-year-old woman presented with several weeks of sacral sensory impairment. MRI of the lumbar spine at presentation showed several nodular, ring enhancing soft tissue …

Conus medullaris injury following both tetracaine and lidocaine spinal anesthesia.

Multiple reports of cauda equina syndrome and transient radicular nerve root irritation have suggested that lidocaine spinal anesthesia may be responsible. In this case report, a patient with a preexisting diabetic neuropathy received a partial block following a tetracaine spinal, which was followed by a lidocaine spinal. Following block resolution, a new conus medullaris syndrome was diagnosed. Because of the close proximity of the cauda equina and the conus medullaris, differentiation between these syndromes can be difficult. The preexisting diabetic neuropathy may have predisposed this pati…

MR imaging findings in cauda equina gnathostomiasis.

We report a case of cauda equina syndrome caused by Gnathostoma spinigerum, which was confirmed by an immunoblotting test. MR imaging of the lumbosacral spine showed long, segmented hyperintensity along the cauda equina with irregular enhancement on the postcontrast study. The conus medullaris was slightly enlarged with abnormal enhancement. The patient was treated with corticosteroids, and her clinical condition improved. MR imaging, 9 months after treatment, showed the condition to be completely resolved.

The Cauda Scale - Validation for Clinical Practice.

Purpose: The purpose of this study was to validate the cauda scale (TCS) in an external population. TCS was proposed as a tool to be used to predict the likelihood of cauda equina compression. Methods: We analysed the presenting condition of consecutive patients attending the emergency department undergoing a magnetic resonance scan with a clinical suspicion of cauda equina syndrome (CES). The findings were graded according to TCS for those with and without radiological compression of the cauda equina. Logistic regression was applied to the data in accordance with the original paper. Results: Pa…

Redundant nerve root syndrome mimicking an intradural spinal cord tumor: A case report.

BACKGROUND: The redundant nerve root (RNR) syndrome is a pathological condition in which the cauda equina develops into a severely flexed/tortuous spiral mass above a level of severe lumbar stenosis. CASE DESCRIPTION: A 70-year-old male presented with bilateral neurogenic claudication attributed to a MRI-documented intradural extramedullary lesion at the L1 level with severe adjacent level/inferior L2/3 stenosis. At surgery, intradural exploration at L1 revealed an edematous cauda equina consistent with the diagnosis of the RNR syndrome. CONCLUSION: The RNR syndrome should be included among

[Neurinomas of the cauda equina as a cause of lumbar sciatica].

The clinical and radiological characteristics together with the therapeutic results in 12 cases of Cauda Equina neurinomas are analyzed. In a high percentage of cases, Cauda Equina neurinomas manifest initially as a simple lumbar sciatic syndrome, frequently not different from those caused by other more common pathologies such as lumbar discal hernia. This article discusses the possible diagnostic problems encountered with the most common currently used radiologic procedures in this type of lesions such as CT scan, myelography and NMR.

Hemangioblastomas of the cauda equina: Clinical features and long-term surgical outcomes.

BACKGROUND: Spinal hemangioblastomas (HBs) that involving cauda equina are rare. Data on clinical characteristics and long-term intervention outcomes of patients harboring cauda equina HBs remain lacking due to its scarcity. OBJECTIVE: This study aims to present the clinical-radiological features and treatment results of this rare pathology by using cases from a single center. METHODS: A review of demographic data and intervention outcomes of patients harboring cauda equina HBs in our department between 2009 and 2020 was retrospectively carried out. RESULTS: Ten consecutive adult patients w

Posterior epidural migration of a lumbar disc herniation.

BACKGROUND: Posterior epidural migration of a lumbar disc fragment (PEMLDF) refers to the dorsal migration of disc material around the thecal sac that can lead to radiculopathy and/or cause a cauda equina syndrome. It is rare and the diagnosis is often just established intraoperatively. CASE DESCRIPTION: A 50-year-old male with a chronic history of low back pain and psychosis presented with PEMLDF originating at the L4-L5 level. CONCLUSION: Lumbar disc herniations rarely present as PEMLDF resulting in symptoms varying from radiculopathy to cauda equina syndrome. These should be included amon…

Surgical issues in the injured worker with lower back pain.

Although most workers who sustain lower back can be managed without surgery, a carefully selected subset of patients benefit from operative intervention. When evaluating these patients, care must be taken to identify surgical emergencies, including patients with cauda equina syndrome and progressive neurologic deficits and patients with cauda equina syndrome and progressive neurologic deficits and patients with certain historical red flags that should prompt further evaluation. Conditions that may benefit from surgery include lumbar disc herniations, discogenic back pain, spinal stenosis, and …

Lumboperitoneal shunt for treatment of dural ectasia in ankylosing spondylitis.

Neurological complications of ankylosing spondylitis (AS) are reported in 2.1% of patients. Cauda equina syndrome (CES) is rare and occurs at the ankylosing stage. MRI and CT of the lumbar spine show a cauda equina deformation with dural ectasia and bony erosion. We report three patients with AS presenting with progressive CES. These patients underwent lumboperitoneal shunting (LPS) surgery. The motor deficit improved in all cases. We suggest that CES develops from arterial pulsation of the CSF on a dural sac with reduced elasticity and that LPS reduces these intradural pressure shock waves. A…

Episodic cauda equina compression from an intradural lumbar herniated disc: a case of 'floppy disc'.

Intradural disc herniation (IDDH) is a rare complication of intervertebral disc disease and comprises 0.26-0.30% of all herniated discs, with 92% of them located in the lumbar region (1). We present a case of IDDH that presented with intermittent symptoms and signs of cauda equina compression. We were unable to find in the literature, any previously described cases of intermittent cauda equina compression from a herniated intradural disc fragment leading to a "floppy disc syndrome".

A Case of Dural Herniation of the Cauda Equina Caused by Enlarged Spinal Subdural Extra-arachnoid Hygroma Following Lumbar Microsurgical Decompression: Case Report.

We experienced a rare case of transdural herniation of cauda equina caused by increased pressure with spinal subdural extra-arachnoid hygroma (SSEH) following lumbar microsurgical decompression. A 68-year-old woman presented with complaints of right leg pain and intermittent claudication. By the diagnosis of L2/3 lumbar spinal stenosis, microsurgical decompression was performed. The surgery was successful with no issues arising such as damage to the dura mater. Lumbar magnetic resonance imaging (MRI) performed 8 days after the surgery confirmed asymptomatic SSEH on the ventral side of the caud

The post-irradiation lower motor neuron syndrome neuronopathy or radiculopathy?

It is not known whether the post-irradiation lower motor neuron syndrome results from radiation damage to motor neuron cell bodies or from damage to the nerve roots of the cauda equina. We studied six cases who had presented with testicular neoplasms, subsequently undergoing irradiation that encompassed inter alia para-aortic nodes with co-irradiation of the distal spinal cord and cauda equina. A predominantly motor disorder affecting the legs ensued after variable and often prolonged latencies (3-25 years). However, all patients also developed mild sensory features either initially or on prol

Intradural cyst with compression of the cauda equina in ankylosing spondylitis.

A cauda equina syndrome complicating long-standing ankylosing spondylitis was found to be due to a large multiloculated cyst. This differed from posterior dural diverticula identified in similar cases in that the cyst filled the lumbar canal with erosion of the upper lumbar vertebral bodies and chronic inflammatory changes were evident in the cyst wall. Cauda equina compromise in ankylosing spondylitis has several possible mechanisms including fibrosis following arachnoiditis, root damage by dural diverticula, and direct compression by arachnoid cyst.

[Chronic adhesive leptomeningitis of the cauda equina as a cause of ischaemic disturbances (author's transl)].

An almost typical sciatic syndrome can be occasionally associated with a thickening of the leptomeninges, producing a type of chronic fibroplastic meningitis in the region of the cauda equina. We report here four personal cases, which showed atypical myelographic patterns. The roots of the cauda equina were compressed both by the cystic and by the fibrosing types. In the cases described, the removal of the altered leptomeninges under the operating microscope produced a satisfactory result. A completely symptomfree outcome can rarely be achieved, as the pathological tissue cannot be completely

Symptomatic spinal epidural lipomatosis associated with Cushing's syndrome.

In cases of Cushing's syndrome, unusual amounts of fat may accumulate in the spinal epidural space, similar to centripetal fat deposits elsewhere in the body. The mass of excessive epidural fat may be responsible for neurological symptoms referable to compression of the spinal cord or cauda equina. We report three cases illustrating this unusual phenomenon and call attention to three cases reported previously. Although it is an unusual complication of Cushing's syndrome, epidural lipomatosis should be considered in the differential diagnosis of any such case with neurological symptoms referabl
