Hirayama disease is a rare and self-limiting cervical myelopathy related to neck flexion in adolescents, predominantly male, originally described by Hirayama in 1959 and until today reported worldwide. It characteristically presents with asymmetric distal upper extremity muscular weakness and atrophy without objective sensory disturbance or lower extremity involvement, which spontaneously halts within 3-5 years after onset with some sequelae. Neuroradiological and autopsy/neuropathological studies have shown that the condition is caused by chronic ischemic changes to the anterior horn cells of

OBJECTIVE: Quantify the anatomic variation of the ansa cervicalis and evaluate neurostimulation of the infrahyoid musculature.
STUDY DESIGN: Intraoperative physiology study.
SETTING: Tertiary referral center.
METHODS: Adult patients undergoing lateral neck dissection including level IV for head and neck cancer were recruited. Ansa cervicalis plexus branches were documented during surgical dissection. In a subset of participants, an electrode was placed on branches of the common sternothyroid trunk innervating the sternothyroid and sternohyoid muscles. Hyolaryngeal excursion with neurostimul

We present the case of a patient with lung cancer who developed brachial plexitis following treatment with immunochemotherapy, which included atezolizumab and palliative radiotherapy for a metastatic cervical spine tumor. This case highlights the importance of considering immune-related adverse events (irAEs) when initiating immune checkpoint inhibitor therapy, in addition to other recognized causes such as radiation, tumor compression and invasion, and paraneoplastic syndromes. While brachial plexitis has been reported as an irAE associated with programmed cell death 1 (PD-1) antibodies, this

The perineural spread of the cancer through the lumbosacral plexus gives an appearance of 'trident sign' on contrast-enhanced CT scan and MRI. It is associated with adverse survival rates and carries a poor prognostic value.

A 24-year-old active duty female Soldier complained of right shoulder burning, stinging, electrical shock-like pain with radiation to the right hand after completing a ruck march. She also complained of swelling and feelings of her cold right hand. Examination showed a deficit in the deltoid, upper trapezius, supraspinatus, and also right winging of the scapula. She also exhibited weakness to right arm, weak right hand grip, and decreased sensation over the dorsal right hand. The right hand was also noticed to be colder to touch than the left one. She had tenderness to palpation over right par

Radical hysterectomy with pelvic lymphadenectomy is considered to be the cornerstone in the treatment of early-stage cervical cancer. Although survival in early-stage cervical cancer is up to 95%, long-term morbidity with regard to bladder, bowel, and sexual function is considerable. Damage to the pelvic autonomic nerves may be the cause of these long-term complications following radical hysterectomy. Some authors have presented surgical techniques to preserve the autonomic nerves (ie, the hypogastric nerves and the splanchnic nerves) without compromising radicality. Safety, efficacy, and the

BACKGROUND: Raynaud's syndrome is a known complication of cold injuries. Stress, smoking and metabolic diseases may further aggravate the disease course. The purpose of this study was to determine the efficacy of Cervico-thoracic sympathectomy as compared to conservative management in severe Raynaud's syndrome after frostbite.
METHODS: This non-randomized controlled trial was conducted at Railway Hospital, Rawalpindi and Islamic International Medical Complex, Islamabad between January 1999 and June 2006. All patients sustained severe cold trauma in the mountain ridges of Himalayas in Kashmir.

The risk of sustaining a stinger, CCN, or a more serious catastrophic injury to the cervical spine increases with increasing stenosis. The RR of a player sustaining a second stinger or CCN increases exponentially when compared with the risk of a player sustaining an initial stinger or CCN. Intravenous steroids have no role in the management of stingers or CCN. Players who remain symptomatic after a stinger, players with persistently abnormal diagnostic studies after a stinger, and any player who experiences a CCN should be excluded from further participation in contact sports.

A clinical case of neuroma resulting from an amputation which occurred following parotid surgery is reported. The present paper discusses the importance of differential diagnosis relating to this pathology and the surgical techniques that may contribute to setting a limit on its onset. Traumatic neuroma is a possible complication of parotid surgery together with motor deficiencies and Frey's syndrome and the less frequent salivary fistulas. It may also occur following oncological or plastic-reconstructive surgery. During parotidectomy operations, section of the great auricular nerve may be req

The authors have included blockade of the upper complex of nervous structures of the neck in the program of treatment of patients after operations on the organs of the abdominal cavity. Studies on the urine-eliminating function of the kidneys and radioisotopic rhenography were performed in 105 patients before and after operation. The results show that the blockade contributes to prevention of functional disorders in the cells of renal tubules and to the improvement of the urine-eliminating and general hemocatharsic function of the kidneys.

OBJECT: A desire to prevent complications resulting from spinal surgery led to the development of intraoperative monitoring. Intraoperative electromyography (EMG) provides useful diagnostic information regarding nerve root function during spinal and peripheral nerve surgeries. The C-5 nerve root is considered particularly vulnerable to injury during cervical surgery. Despite advances in techniques, the incidence of postoperative C-5 palsy has not changed.
METHODS: The authors reviewed prospectively collected data obtained in 161 patients who underwent 171 cervical procedures. In 116 procedure

Whether to perform surgery for acute traumatic central cord syndrome (ATCCS) without bone fracture or dislocation but with spinal cord compression due to preexisting cervical canal stenosis remains controversial. We present a case of ATCCS without bone injury in which neurologic deterioration was possibly caused by traumatic cerebrospinal fluid rhinorrhea (TCFR). An 82-year-old man with ATCCS (American Spinal Injury Association (ASIA) Impairment Scale (AIS) C) and an anterior skull base fracture was urgently transported to our hospital and was initially treated conservatively. However, on the

BACKGROUND: Anterior cervical discectomy and fusion (ACDF) has become a common procedure for cervical spine surgeries, since it is safe and effective in most patients. However, some patients develop life-threatening problems such as respiratory obstruction arising from rare postoperative hematoma and edema, although intraoperative bleeding caused by the connective tissue splitting procedure or bleeding caused by postoperative insertion of a suction tube has rarely been reported. Investigation of the requirement for indwelling drains in patients who undergo cervical spine surgery is necessary b

Chimpanzee attacks can be vicious, mutilating, and disabling if not fatal. Stereotypically, the hands and face are targeted, and in male victims, genitalia are mutilated. The authors present a case highlighting the difficulties with early neurological assessment following such an attack. This 55-year-old woman was attacked by a 14-year-old chimpanzee. She suffered mutilation of both hands, severe midface bony, soft-tissue, and eye injuries, and scalp degloving. An emergency tracheotomy was performed at the scene, with an unclear duration of hypoxia. The patient was unresponsive without spontan

Regional cerebral blood flows and shunting of microspheres with four different sizes (9,12, 16 and 25 microns) into the superior sagittal sinus were determined in twelve dogs. Venous blood was collected from the superior sagittal sinus for 120 min after the injection of microspheres, and the dogs were then sacrificed immediately. Results on blood flow measurements and venous shunting determinations were similar between left ventricular and left atrial injections. Blood flows measured by 12, 16 and 25 microns spheres were comparable in various brain tissues, except the choroid plexus. 9 microns
