Incidence of bacteremia in patients with opisthorchiasis during recurrent cholangitis.

A total of 257 haemocultures were performed in 50 patients with opisthorchiasis when they presented signs and symptoms of biliary tract infection. 19 patients showed positive haemoculture. There are no significant relationship between the age of the patient and the incidence of positive haemoculture. Septic shock occurred in 5 patients, one patient died. Out of 221 aerobic cultures, 14% were positive and of the 36 anaerobic cultures 11% were positive. The most common organism was Staphylococcus followed by Klebsiella and Bacillus spp. Anaerobic bacterias were Streptococcus spp. Clostridia spp.

Papillary stenosis and sclerosing cholangitis in the acquired immunodeficiency syndrome.

Eight homosexual men with the acquired immunodeficiency syndrome (AIDS) presented with clinical, biochemical, and radiologic features of stenosis of the papilla of Vater and sclerosing cholangitis. This newly recognized complication of AIDS produces abdominal pain, nausea, and vomiting and may predispose patients to superimposed bacterial cholangitis. Marked elevation of serum alkaline phosphatase levels and lesser changes in hepatic aminotransferase levels are common. Although abdominal ultrasonography and computed tomography detect ductal abnormalities, endoscopic retrograde cholangiography …

[Value of laparoscopy in the diagnosis of chronic nonsuppurative destructive cholangitis].

39 female and 5 male patients with chronic non-suppurative destructive cholangitis (CNDC), excluding cases with the complete picture of a primary biliary cirrhosis, were examined. Besides clinical and immunological investigations in all patients laparoscopy was performed, in 18 cases repeatedly in order to control the course. As typical laparoscopic appearance in 43 of the cases we saw geographically outlined and generally communicating areas on the liver surface within somewhat brighter surroundings ("map-like-phenomenon"). Besides this a darker coloration in the center of the liver lobules w…

The pancreas in primary biliary cirrhosis and primary sclerosing cholangitis.

In patients with primary biliary cirrhosis, the chronic cholestasis, salivary, and lacrimal hyposecretion suggest that the disease is a "dry gland" syndrome. To determine whether or not pancreatic damage occurs in primary biliary cirrhosis and other forms of chronic cholestasis, we have studied pancreatic structure and function in primary biliary cirrhosis, and primary sclerosing cholangitis. In a retrospective study, retrograde pancreatograms were abnormal in 43% of 35 patients with primary biliary cirrhosis and 15% of 20 patients with primary sclerosing cholangitis (p less than 0.02). In a p…

The impact of male sex on outcomes in primary biliary cholangitis.

Limited evidence suggests men with Primary Biliary Cholangitis (PBC) experience worse outcomes than women. The aims of this study were to assess sex-related differences in outcomes in PBC, and to determine whether differences were more specific to PBC than other cirrhosis aetiologies. This study conducted at Charité Berlin analysed inpatient admissions from 2011 to 2022 with PBC. The primary outcome was a composite of inpatient mortality or liver transplant. Secondary outcomes included inpatient mortality, liver transplant, decompensated cirrhosis, and hospitalisation duration. Univariable and…

Expression and clinical characteristics of CD161 in primary biliary cholangitis.

Primary biliary cholangitis (PBC) is a chronic autoimmune cholestatic liver disease lacking noninvasive biomarkers that adequately reflect disease status. CD161 is an immunoregulatory molecule expressed on T and NK cells, but its soluble (sCD161) and membrane-bound (mCD161) forms remain poorly characterized in PBC. We measured serum sCD161 and cellular mCD161 in patients with PBC, patients with post-hepatitis B cirrhosis (PHBC), and healthy controls (HCs), and examined CD161 expression in a PBC mouse model. Serum sCD161 was markedly elevated in patients with PBC compared with both patients wit…

Outcome of Liver Retransplantation in Patients With Primary Sclerosing Cholangitis.

BACKGROUND AND AIMS: Primary sclerosing cholangitis (PSC) is among the most common indications for liver transplantation in the Nordic countries and with an increasing trend in Europe and North America. Due to post-transplant complications and high prevalence of disease recurrence this group is at risk of requiring retransplantation (re-LTX). Results from re-LTX for PSC are not extensively studied and there is a lack of knowledge regarding prognosis after re-LTX in this population. METHODS: Graft and patient survival after re-LTX for patients with PSC and a comparable comparison group from th…

Biliary stents reshape the bile microbiome in the absence of cholangitis.

BACKGROUND AND STUDY AIMS: Biliary stents are widely used in endoscopic retrograde cholangiopancreatography (ERCP), yet their impact on the native bile microbiome under non-infectious conditions remains unclear. We aimed to characterize stent-associated alterations in the biliary microbiome using 16S rRNA gene sequencing. PATIENTS AND METHODS: We analyzed bile samples collected during ERCP from 35 patients without clinical or laboratory evidence of acute cholangitis. Patients were categorized into a control group (n = 25; naïve papillae) and an endoscopic biliary stenting (EBS) group (n = 10;…

Innate immunity of bile and cholangiocytes in primary biliary cholangitis.

Primary Biliary Cholangitis (PBC) is a chronic autoimmune liver disease characterized by immune-mediated destruction of intrahepatic bile ducts. This review synthesizes current knowledge on the critical role of innate immunity, specifically involving cholangiocytes, bile components, and associated immune cells. Cholangiocytes function not only as passive targets but also as active immunomodulators through mechanisms including Toll-like receptor (TLR) signaling, antigen presentation, and immune cell recruitment. Dysregulated bile acid signaling via receptors like TGR5 disrupts immune homeostasi…

Primary sclerosing cholangitis: what is new in the therapeutic landscape.

Primary sclerosing cholangitis (PSC) is a rare, chronic cholestatic liver disease affecting the intra- and extra-hepatic bile ducts, leading to progressive inflammation and fibrosis. Its pathogenesis is complex and involves genetic predisposition, environmental triggers, and their interaction through the gut-liver axis, mediated by the microbiota. To date, no approved therapies modify the natural history of PSC, and liver transplantation remains the only curative option. This review provides an overview of current investigational therapies for PSC, emphasizing their mechanisms of action and re…

The Treatment of Primary Biliary Cholangitis: Time for Personalized Medicine.

Primary Biliary Cholangitis is an autoimmune liver disease distinguished by Anti-mitochondrial Antibodies and chronic non-suppurative lymphocytic microcholangitis. UDCA remains the exclusively recommended initial therapy for PBC. However, 40% of patients experience either incomplete biochemical response or intolerance to UDCA, which represents poorer outcomes and increased mortality. Therefore, early identification of high-risk patients and timely intensive treatments are necessary to delay the progression of PBC and better improve the prognosis. Intensive therapeutic strategies based on more …

Ultrasound for the Detection of Gallbladder Malignancy in Primary Sclerosing Cholangitis.

BACKGROUND AND AIMS: In primary sclerosing cholangitis (PSC), the risk for gallbladder malignancy is increased. Surveillance imaging is used for early diagnosis. The study aims to assess the reliability of ultrasound and magnetic resonance imaging (MRI) for the detection of gallbladder polyps in people with PSC and to define a polyp size as a cut-off at which cholecystectomy is indicated due to the high probability of a malignant finding. METHODS: In this retrospective European multicentre study, we included 51 people with PSC who had cholecystectomy for gallbladder polyps detected on imaging…

Primary Sclerosing Cholangitis in a Child: A Rare Case Report.

Primary sclerosing cholangitis (PSC) is a chronic inflammation of the bile ducts that leads to progressive narrowing, ultimately resulting in liver cirrhosis. A 10-year-old boy presented with jaundice, abdominal pain, nausea, weight loss, and yellow papules and nodules on the eyelids, elbows, and axillae. A physical examination revealed jaundice, pallor, and hepatosplenomegaly. The laboratory results revealed anemia, cholestasis, and elevated liver enzymes, along with hypoalbuminemia. The tests for viral hepatitis and autoimmune markers were negative. Abdominal ultrasonography revealed increas…

Frequency of antithyroid antibodies in patients with primary biliary cholangitis.

OBJECTIVE: Primary biliary cholangitis (PBC) is an autoimmune disease of liver that may be associated with other conditions, including autoimmune thyroid diseases. We aimed to investigate the frequency of anti-thyroperoxidase antibodies (TPO-Ab), antithyroglobulin antibodies (TG-Ab), and anti-thyrotropin receptor antibodies (TSHR-Ab) in Tunisian patients with PBC. METHODS: Sera of 80 patients with PBC were collected over a 9-year period. A total of 189 healthy blood donors (HBD) were included in the control group. Measurements of TPO-Ab and TG-Ab were performed using indirect enzyme-linked im…
