Parenteral nutrition-associated cholestasis in preterm neonates: evaluation of ursodeoxycholic acid treatment.

BACKGROUND/OBJECTIVE: Parenteral nutrition is an integral part of the care of premature infants. Cholestatic liver disease is a frequent complication of prolonged parenteral nutrition, especially in premature infants. It has been suggested that ursodeoxycholic acid may alter the course of parenteral nutrition-associated cholestasis in children and adults. We attempted to determine the efficacy of ursodeoxycholic acid in premature infants with parenteral nutrition-associated cholestasis. METHODS: Retrospective chart review of all infants receiving ursodeoxycholic acid for parenteral nutrition-…

Parenteral nutrition-related cholestasis in postsurgical neonates: multivariate analysis of risk factors.

The medical records of 74 neonates dependent on parenteral nutrition for at least 21 days after emergency abdominal surgery (performed between 1988 and 1992) were reviewed respectively. The role of enteral starvation, prematurity, composition and duration of parenteral nutrition, and sepsis in the evolution of parenteral nutrition-related cholestasis was evaluated by multiple regression analysis. The most important factors for cholestasis were low gestational age (median, 34 weeks), early exposure to parenteral nutrition, and sepsis. Episodes of sepsis were associated with a 30% increase in th…

Total parenteral nutrition-associated cholestasis in infants: clinical and liver histologic studies.

To evaluate total parenteral nutrition-associated cholestasis (TPN-C) in infants, a retrospective clinicopathologic study was conducted of 15 infants who had received TPN. The mean gestational age and birth weight were 32.1 weeks (26-40 weeks) and 1807 g (840-5840 g) respectively. Two-thirds of the patients were kept on TPN for more than 60 days. The onset of rising direct bilirubin ranged 2-9 weeks (mean 4.5 +/- 2.4) after TPN therapy. Preterm babies less than 32 weeks of age had an earlier rise of direct bilirubin and AST. Bile sludge of the gallbladder was observed in only one case, and non…

Reversible cholestasis with bile duct injury following azathioprine therapy. A case report.

A 67-year-old patient, with primary polymyositis and without previous evidence of liver disease, developed clinical and biochemical features of severe cholestasis 3 months after initiation of azathioprine therapy. Liver biopsy showed cholestasis with both cytological and architectural alterations of interlobular bile ducts. Azathioprine withdrawal resulted after 7 weeks in the resolution of clinical and biochemical abnormalities. It is believed that this is the first reported case of reversible azathioprine-induced cholestasis associated with histological evidence of bile duct injury.

Methyltestosterone-induced cholestasis. The importance of disproportionately low serum alkaline phosphatase level.

We describe a 64-year-old man who developed cholestatic jaundice after receiving 20 to 40 mg of methyltestosterone daily for 6 months for impotence but failed to mention it as part of his drug history. He underwent endoscopic retrograde and papillotomy before a positive history for methyltestosterone ingestion could be obtained. Since methyltestosterone is most often used for sexual impotence, the patient may be quite reluctant to mention this hormone as part of his medication. A normal or mildly elevated alkaline phosphatase level, disproportionate to the level of hyperbilirubinemia seen in t

Familial intrahepatic cholestasis associated with progressive neuromuscular disease and vitamin E deficiency.

Three Japanese patients with familial progressive intrahepatic cholestasis developed complications involving neurologic abnormalities characterized by ataxia and pigmentary retinopathy. Serum vitamin E concentrations were extremely low in all patients, suggesting a long-term vitamin E deficiency. High dose oral supplementation of alpha-tocopherol produced normal serum vitamin E levels in two patients. Parenteral administration of vitamin E resulted in no clinical improvement in one patient who first received the treatment at 14 years of age. In the other two patients, the progression of neurol…

Metformin Drives Hepatocyte Molecular Reprogramming Through an FGF1/FGFR4-TBX3 Axis in Cholestasis.

BACKGROUND & AIMS: Cholestatic liver fibrosis involves a pathological shift from hepatocyte to cholangiocyte molecular signatures. Metformin has hepatoprotective effects, but its impact on this shift remains unclear. We investigated whether metformin reverses this pathological program and elucidated the underlying mechanisms. METHODS: We employed purified hepatocyte organoids derived from bile duct ligation (BDL) and Mdr2-/- mouse models of cholestasis, as well as patient-derived organoids (PDOs) from patients. Transcriptomic and functional analyses were performed to assess metformin-induced …

Hepatitis A-related acute liver failure and severe acute pancreatitis with prolonged cholestasis.

Acute hepatitis A virus (HAV) infection is typically self-limiting but may rarely lead to severe hepatic and extra-hepatic complications. However, HAV-related acute liver failure may lead to complicated acute pancreatitis, and bilateral pleural effusions requiring drainage, and prolonged cholestatic hepatitis.

Nanocellulose Alleviates Intrahepatic Cholestasis of Pregnancy via Gut Microbiota-Mediated Bile Acid Homeostasis.

Intrahepatic cholestasis of pregnancy (ICP) is a pregnancy-specific disorder characterized by elevated maternal serum total bile acid (TBA) and increased risk of adverse fetal outcomes, yet current therapies and preventive strategies remain suboptimal. Here, we present a preclinical investigation of nanocellulose as a gut microbiota-targeted dietary intervention to restore bile acid homeostasis. We demonstrate that nanocellulose, particularly cellulose nanofibers (CNF), administered prophylactically, attenuated maternal TBA and improved offspring survival in a rat ICP model by enriching probio…

Neonatal Cholestasis Presenting With Seizures Associated With Vitamin K Deficiency: A Case Report.

Neonatal Cholestasis is one of the rare yet important causes of Vitamin K deficiency bleeding (VKDB) in infants. VKDB may present as Gastro Intestinal (GI) bleeding and in severe cases as Intracranial hemorrhage. We report a case of a 42 days old, previously healthy, male infant presenting with seizures and GI bleeding. Laboratory investigations revealed direct hyperbilirubinemia and markedly elevated PT/INR. These findings were consistent with neonatal cholestasis and Vitamin K deficiency. Cranial and Abdominal Ultrasonography were normal. The seizure was managed with Vit K and Fresh Frozen P…

Living Donor Liver Transplantation for Progressive Familial Intrahepatic Cholestasis Type 13 (PFIC 13).

Cholestasis secondary to protein serine kinase H1 (PSKH1) is a recently described entity, which leads to chronic liver disease in childhood or early teenage years with or without renal insufficiency. Mutations in the PSKH1 gene reduce PSKH1's kinase activity, leading to abnormally long and convoluted cilia in renal tubules and bile ducts, disrupting normal function. There are very few reports highlighting the outcomes of liver transplantation (LT) for PSKH1-related liver disease. There have been no reports of PSKH1-induced cholestasis reported from India till date. We describe the first report…

Double Filtration Plasmapheresis for Refractory Hypercholesterolemia in Drug-Induced Cholestasis: A Case Report.

While lipoprotein apheresis (LA) is established for lipid management in primary biliary cholangitis, the role of apheresis techniques in drug-induced cholestatic liver disease remains unexplored. We report the novel application of double filtration plasmapheresis (DFPP) for rapid reversal of severe hypercholesterolemia in iatrogenic cholestasis, where conventional therapies failed or were contraindicated. A 56-year-old male developed severe cholestatic hepatitis following the use of herbal agents. Laboratory investigation revealed extreme hypercholesterolemia, with a total cholesterol (TC) of …

Tumor necrosis factor alpha-induced activation of SREBP2 promotes cholesterol biosynthesis in cholestasis.

BACKGROUND AND AIMS: Cholestasis is frequently associated with lipid metabolism disorders, elevated cholesterol levels and disruptions in bile acid homeostasis. Nevertheless, the mechanisms underlying cholesterol elevation in cholestasis remain inadequately understood. This study aims to investigate alterations in cholesterol levels and potential mechanisms in mouse models of cholestasis. Additionally, we evaluate the therapeutic potential of Sterol Regulatory Element Binding Protein 2 (SREBP2), a key transcription factor regulating cholesterol synthesis, in treating cholestasis. APPROACHES A…

Multi-omics characterization of autophagy-related molecular features in intrahepatic cholestasis of pregnancy†.

INTRODUCTION: Despite the increasing body of evidence that autophagy implicate in intrahepatic cholestasis of pregnancy (ICP), we are still far from a mechanistic understanding of the autophagy in ICP pathogenesis. METHODS: In this study, we performed untargeted lipidomics, scRNA-seq, and RNA-seq data to systematically demonstrate the lipid alterations, cells and gene expression closely related to autophagy in placenta. These results were also verified by tissue, cell, and animal experiments. RESULTS: We confirmed that autophagy played a pivotal role in ICP, autophagy-related lipids includin…

A novel mechanism involving USP53-regulated BSEP trafficking underlies low-GGT intrahepatic cholestasis.

BACKGROUND AND AIMS: USP53 variants cause low-GGT progressive familial intrahepatic cholestasis (PFIC). The mechanism is not well understood. USP53, which encodes a ubiquitin-specific protease, has been proposed to be involved in blood-bile barrier impairment. However, Usp53 knockout mice did not show blood-bile barrier impairment. The aim of this study was to investigate the molecular mechanism underlying USP53 -PFIC. APPROACH AND RESULTS: Immunohistochemistry of patient tissue, confocal immunofluorescence microscopy and surface protein biotinylation were performed to investigate the localiz…

Immunohistochemistry in Progressive Familial Intrahepatic Cholestasis (PFIC): Bridging Gap Between Morphology and Genetics.

BACKGROUND/AIMS: A heterogeneous group of disorders caused by bile secretion and transport defects is progressive familial intrahepatic cholestasis (PFIC). PFIC has various subtypes with different presentations, laboratory findings, treatments, progression, and prognosis. Genetic analysis is the gold standard for diagnosis but is costly, time-consuming, and not readily available. In this study, immunohistochemistry (IHC) was evaluated as a tool for identifying subtypes of PFIC and differentiating them from other causes of pediatric cholestasis. METHODS: The study included genetically confirme…
