Glycosaminoglycan-based hydrogels capture inflammatory chemokines and rescue defective wound healing in mice.

Excessive production of inflammatory chemokines can cause chronic inflammation and thus impair cutaneous wound healing. Capturing chemokine signals using wound dressing materials may offer powerful new treatment modalities for chronic wounds. Here, a modular hydrogel based on end-functionalized star-shaped polyethylene glycol (starPEG) and derivatives of the glycosaminoglycan (GAG) heparin was customized for maximal chemokine sequestration. The material is shown to effectively scavenge the inflammatory chemokines MCP-1 (monocyte chemoattractant protein-1), IL-8 (interleukin-8), and MIP-1α (mac

A shared care program for the management of patients with chronic hepatitis B in a Canadian primary care refugee clinic.

OBJECTIVES: Refugees in Canada are disproportionately affected by chronic hepatitis B (CHB) and its complications due to healthcare barriers and resettlement challenges. In 2011 the Mosaic Refugee Health Clinic (MRHC) in Calgary, Canada began a shared care model for CHB. Family physicians identify and monitor chronically infected patients, reviewing cases with hepatology. STUDY DESIGN: A cohort of all adult patients (≥18 years old) with a positive hepatitis B surface antigen result (HBsAg) between January 1, 2011 and December 31, 2020 was assessed through retrospective chart review. METHODS:

The role of socio-economic factors on prevalence and health outcomes of persons with diabetes in Rome, Italy.

BACKGROUND: We investigated the role of socio-economic status on diabetes prevalence, on mortality and hospitalization in a large population-based cohort enrolled in Rome, Italy. METHODS: Diabetic residents aged ≥ 35 years in 2007 were identified using multiple data sources. The effect of the deprivation of the area of residence on diabetes prevalence and on mortality and hospitalization (years 2008-10) was investigated by multilevel regression models, both among diabetic and non-diabetic populations. RESULTS: Prevalence of diabetes (8.3%) was directly related to the deprivation of the area

Link between the albumin-corrected anion gap and 28 day all‑cause mortality among patients with sepsis complicated with chronic heart failure: A retrospective analysis using the eICU Collaborative Research Database.

BACKGROUND: These two conditions, namely metabolic acidosis and hypoproteinemia, are prevalently observed in patients within intensive care units (ICU), particularly those with sepsis complicated with chronic heart failure. Nevertheless, the impact of the Albumin-Corrected Anion Gap (ACAG), an indicator reflecting the above conditions, on such patient mortality requires further investigation. This retrospective cohort study analyzed the significance of ACAG levels in forecasting 28-day all-cause mortality among these patients admitted to ICU. METHODS: This was observational cohort study on eI

Satisfaction Resilience in Virtual Home Health Care for Chronic Disease Management: Lifestyle-Integration Motivation and Functional Equivalence Beliefs in a Cross-Sectional Study.

Background/Objectives: Virtual home health care (VHHC) is increasingly used to support chronic disease management, yet patient satisfaction is often examined through a linear barrier-focused lens, in which more barriers are assumed to produce lower satisfaction. This study introduced a satisfaction resilience framework to identify factors associated with preserved satisfaction among barrier-exposed VHHC users. Methods: This cross-sectional analysis included 517 adults with chronic diseases who had used VHHC services and were recruited across five regions of Saudi Arabia. Patients were classifi

Conceptualizing and treating the polytrauma clinical triad as a complex chronic pain syndrome.

The polytrauma clinical triad (PCT) - comprised of posttraumatic stress disorder (PTSD), chronic pain syndromes (CPS), and traumatic brain injury (TBI) - is a significant problem among US Service Members and Veterans (SMV). Many SMV treated for PCT improve within 5 years of initial presentation, but a significant fraction show declining health despite extensive care, underscoring the necessity for enhanced management strategies in this population. Since PCT commonly presents as a distressing and debilitating polysymptomatic experience centered around pain, the proposed Functional Recovery Enab

Joint modelling of potentially avoidable hospitalisation for five diseases accounting for spatiotemporal effects: A case study in New South Wales, Australia.

BACKGROUND: Three variant formulations of a spatiotemporal shared component model are proposed that allow examination of changes in shared underlying factors over time. METHODS: Models are evaluated within the context of a case study examining hospitalisation rates for five chronic diseases for residents of a regional area in New South Wales: type II diabetes mellitus (DMII), chronic obstructive pulmonary disease (COPD), coronary arterial disease (CAD), hypertension (HT) and congestive heart failure (CHF) between 2001-2006. These represent ambulatory care sensitive (ACS) conditions, often use

How many people will need palliative care in 2040? Past trends, future projections and implications for services.

BackgroundCurrent estimates suggest that approximately 75% of people approaching the end-of-life may benefit from palliative care. The growing numbers of older people and increasing prevalence of chronic illness in many countries mean that more people may benefit from palliative care in the future, but this has not been quantified. The present study aims to estimate future population palliative care need in two high-income countries.MethodsWe used mortality statistics for England and Wales from 2006 to 2014. Building on previous diagnosis-based approaches, we calculated age- and sex-specific p

Cost of Disease Progression in Patients with Chronic Lymphocytic Leukemia, Acute Myeloid Leukemia, and Non-Hodgkin's Lymphoma.

INTRODUCTION: To reduce health care costs and improve care, payers and physician groups are switching to quality-based and episodic or bundled-care models. Disease progression and associated costs may affect these models, particularly if such programs do not account for differences in disease severity and progression risk within the cohort. This study estimated the incremental cost of disease progression in patients diagnosed with chronic lymphoid leukemia (CLL), acute myeloid leukemia (AML), and non-Hodgkin's lymphoma (NHL) and compared costs among patients with and without progression. It's Virtually Here: Telemedicine, Virtual Care, and Neuromuscular Disease.

The COVID-19 pandemic forced a partial shift in neuromuscular clinical practice away from in-person care to remote means. However, this shift was unexpected, abrupt, reactive to the emergency of a pandemic, and disruptive to the usual manner of care, without adequate time to adapt or study the optimal manner to provide care remotely. While traditionally associated with hub-and-spoke telemedicine models, the pandemic popularized direct-to-home virtual care, mostly unsupported by on-site examiners or standardized platforms. In chronic neuromuscular disorders such as amyotrophic lateral sclerosis

The association between care modality and hospitalizations and emergency department visits for ambulatory care-sensitive conditions during and after the pandemic in Ontario, Canada.

The COVID-19 pandemic required a rapid transition to virtual care as a key strategy to maintain healthcare access while minimizing virus transmission risks. However, the impact of this shift on hospitalizations and emergency department (ED) visits for ambulatory care-sensitive conditions (ACSCs) remains unclear. This study aims to assess the relationship between the modality of outpatient care for ACSCs and their outcomes in Ontario, Canada. In this population-based retrospective cohort study, we analyzed hospitalization and ED visit data for ACSCs, including diabetes, epilepsy, congestive hea

Integrating Primary Care into the Management of Cystic Fibrosis.

Over the last 50 years, cystic fibrosis has radically transformed from a fatal disease of infancy to a chronic disease of adulthood. By 2025 it is estimated that 70% of individuals with cystic fibrosis (iwCF) will be cared for in adult clinics. We believe the role of a dedicated primary care provider (PCP) for preventative care will be crucial for the longevity of iwCF. There are various models for incorporating primary care medicine into CF management, but no universally accepted standard exists. Ideally, the PCP and pulmonologist practice in a patientcentered medical home, given the growing

Signs and symptoms indicating the transition to the palliative phase in patients with COPD and heart failure in primary healthcare: a mixed-methods study.

BACKGROUND: Early identification of the palliative phase in patients with chronic obstructive pulmonary disease (COPD) and heart failure (HF) remains challenging. Timely identification enables recognition of palliative care needs and the initiation of comprehensive palliative care, yet it often occurs too late. Because most patients with COPD and HF spend the majority of their disease trajectory at home, recognition within community care is essential to anticipate needs and maintain quality of life. Globally, only 14% of people needing palliative care receive it. In primary care, existing scre

The roles of diabetes-specific self-compassion, affect balance, and self-care self-efficacy in psychological distress among Hong Kong patients with Type 2 diabetes.

OBJECTIVE: Patients with Type 2 diabetes (T2D) often blame themselves for their conditions and not reaching their self-management goals. Guided by the self-regulation resource model, self-compassion facilitates adjustments to chronic diseases through increasing affective and cognitive resources like positive affect and self-care self-efficacy. However, the contributions of positive and negative self-compassion (PSC and NSC) in psychological adjustment among patients with T2D have not been examined. Studies also suggest that patients with T2D experience positive changes from their illness journ

Integration of contraceptive services into anticoagulation management services improves access to long-acting reversible contraception.

OBJECTIVE: Integration of services is a promising way to improve access to contraception in sub-Saharan Africa, but few studies have evaluated this strategy to increase access to contraception among women requiring anticoagulation. Our objective was to evaluate a model of care integrating contraceptive counseling and provision within an anticoagulation management clinic in Eldoret, Kenya, to determine the impact on long-acting reversible contraception (LARC) use. STUDY DESIGN: We performed a prospective observational study of reproductive-age women referred for integrated services from the an

A modified world café approach to identify the core components of palliative care models for underserved populations.

BACKGROUND: Palliative care is becoming increasingly important for many health systems worldwide, driven by ageing populations and the increasing prevalence of chronic conditions. Access to and use of palliative care are not uniform across populations and are particularly limited among people of culturally and linguistically diverse communities, people of low socioeconomic status, and people residing in rural areas. To address these disparties, models of palliative care are required to meet the needs of these populations. The aim of this study was to identify stakeholder perspectives on the co

Did changing primary care delivery models change performance? A population based study using health administrative data.

BACKGROUND: Primary care reform in Ontario, Canada started with the introduction of new enrollment models, the two largest of which are Family Health Networks (FHNs), a capitation-based model, and Family Health Groups (FHGs), a blended fee-for-service model. The purpose of this study was to evaluate differences in performance between FHNs and FHGs and to compare performance before and after physicians joined these new primary care groups. METHODS: This study used Ontario administrative claims data to compare performance measures in FHGs and FHNs. The study population included physicians who b

Decentralization and Integration of Advanced Cardiac Care for the World's Poorest Billion Through the PEN-Plus Strategy for Severe Chronic Non-Communicable Disease.

Rheumatic and congenital heart disease, cardiomyopathies, and hypertensive heart disease are major causes of suffering and death in low- and lower middle-income countries (LLMICs), where the world's poorest billion people reside. Advanced cardiac care in these counties is still predominantly provided by specialists at urban tertiary centers, and is largely inaccessible to the rural poor. This situation is due to critical shortages in diagnostics, medications, and trained healthcare workers. The Package of Essential NCD Interventions - Plus (PEN-Plus) is an integrated care model for severe chro

Models of care for women living with HIV in Europe: A scoping review.

INTRODUCTION: With the widespread availability of antiretroviral therapy, human immunodeficiency virus (HIV) has become a chronic condition requiring long-term, integrated care. Women with HIV experience distinct clinical, reproductive, and psychosocial needs shaped by gender and social determinants. However, care models addressing these needs for women in Europe have not been comprehensively documented. In this scoping review, we aim to map existing literature on models of care for women with HIV in Europe and examine their key components and regional variation. METHODS: We searched the Ovid
