Pipkin Type-II fractures of the femoral head.

Ten patients with a Pipkin Type-II fracture of the femoral head associated with dislocation of the same hip were treated and followed for two to five years. Five of these ten fractures, which were reduced anatomically after closed reduction of the dislocation, were treated by traction. All five healed and no evidence of avascular necrosis was identified radiographically or clinically at follow-up. Of the remaining five patients who had an unsatisfactory closed reduction, four were treated by open reduction and internal fixation with two screws and one, by excision of the comminuted fragment. Medial subtalar dislocation associated with fracture of the posterior process of the talus.

A 17-year-old male sustained a medial subtalar fracture dislocation while playing basketball. The dislocation of the subtalar joint was reduced by closed means, but a large, comminuted, displaced posterior process fracture remained. Open reduction and internal fixation through a posteromedial approach was used to restore congruity to the subtalar joint. MRI at 12 months after injury showed no signs of avascular necrosis in the talus.

Rotational radioulnar dissociation: a case report and review of the literature.

We report a rare case of radioulnar dissociation in an 11-year-old boy associated with fracture of the medial humeral epicondyle. Open reduction and Kirschner wire fixation was performed for the epicondylar fracture. The radial head was reduced closed, and the distal radioulnar joint reduced spontaneously. The mechanism of this unusual injury appears to be hyperpronation of the forearm and valgus stress to the elbow.

Intraoperative Ultrasound Control of Zygomatic Arch Fractures: Does Additional Imaging Improve Reduction Quality?

PURPOSE: Intraoperative navigation to assess anatomic reduction is the general trend in maxillofacial trauma surgery. The aim of this study was to evaluate the closed reduction outcome of isolated zygomatic arch fractures using ultrasound compared with palpation control. MATERIALS AND METHODS: In this case-and-control study, the authors identified consecutively treated patients who underwent closed reduction of isolated zygomatic arch fractures using the Volkmann reposition hook with intraoperative ultrasound. Controls were patients with the same diagnosis and surgical procedure without ultra

Fracture blisters: clinical and pathological aspects.

Fracture blisters are tense vesicles or bullae that arise on markedly swollen skin directly overlying a fracture. There is very little objective data in the literature detailing their characteristics and management. All fracture blisters that occurred over a 3 1/2-year period were studied retrospectively at four hospitals, of which three were level I trauma centers. A total of 53 blisters developed in 51 patients. They occurred in characteristic locations along the human musculoskeleton, most commonly overlying the tibia, ankle, and elbow. They arose within 24-48 h of acute injury in most inst

Fractures of the capitellum in adolescents.

Fractures of the capitellum are rare in children. The treatment of these injuries has been controversial. At a major pediatric trauma center, seven capitellar fractures were seen in children between 1988 and 1994. The average age of the children was 14.7 years (range, 11-17). Six of these fractures were type I injuries, with large anterosuperior fragments that required operative reduction and internal fixation in five cases. Internal fixation methods used were K wires in three patients, Herbert screws in one patient, and cannulated screws in one patient. The remaining type I fracture was treat

Dual Joystick Technique for Reduction of Torsional Profile in Pediatric Supracondylar Humerus Fracture With Delayed Presentation.

BACKGROUND: Reduction of some modified Gartland type III and IV supracondylar humerus fractures can pose difficulties, especially if they present late to the hospital. Various techniques of reduction have been tried for reducing the supracondylar humerus fracture for sagittal and coronal plane correction. This retrospective study assesses the dual joystick technique's possible effectiveness in achieving an anatomical reduction of the supracondylar humerus fracture. METHODS: Patients with modified Gartland's type III and IV supracondylar humerus fractures who underwent closed reduction and per

Open reduction and internal fixation of irreducible displaced femoral neck fracture with femoral Neck System: a preliminary study.

BACKGROUND: Most displaced femoral neck fractures can achieve satisfactory anatomical reduction by closed reduction, but there are still some that cannot reset satisfactorily after closed reduction, and open reduction are required. Such fractures that cannot be repositioned successfully by closed reduction are called irreducible displaced femoral neck fractures in this study. The objective of our study was to evaluate the efficacy of direct anterior incision with the Femoral Neck System in the treatment of irreducible displaced femoral fractures. METHODS: A total of 16 young and middle-aged p

Inability to obtain formal informed consent in the face of a standard surgical indication.

A thirty-eight-year-old intoxicated man was admitted to the surgical trauma service following a single motor-vehicle accident. He had a severe closed head injury, bilateral pulmonary contusions, a fracture-dislocation of the right acetabulum, and an open injury of the right knee joint. The acetabular fracture pattern was an associated both-column fracture with the femoral head dislocated into a widely displaced posterior-column fracture line. The treating physicians agreed that it would be in the patient's best interest to take him to the operating room for emergent debridement and irrigation

Retrospective analysis of two hundred thirty-five pediatric mandibular fracture cases.

Maxillofacial fractures are encountered less commonly during childhood period due to anatomic, social, cultural, and environmental factors. Although the incidence of all maxillofacial fractures is 1% to 15% among pediatric and adolescent patients, this rate drops to less than 1% in children below 5 years age. Two hundred thirty-five cases (<or=16 age) with mandibular fracture were evaluated retrospectively. Patients records were examined in terms of age, gender, cause of fracture, fracture localization, number of fractures, fracture pattern, accompanying injuries, applied treatment methods, an

Irreducible Traumatic Fracture-Dislocation of the Hip with Impalement onto the Ischial Spine: A Case Report.

CASE: A 45-year-old man appeared to have a central (protrusio) hip dislocation but actually had a transverse posterior wall acetabulum fracture with irreducible posterior dislocation due to impalement of the femoral head on the ischial spine. He underwent urgent open reduction on presentation and subsequent internal fixation in a staged manner. He developed avascular necrosis at 18 months postoperatively. CONCLUSION: The nondisplaced ilioischial and iliopectineal acetabular radiographic lines were alerts that the dislocation was actually posterior. This led to further imaging before any close

The value of manipulation of displaced distal radius fractures in the emergency department.

The aim of this study was to report the success of maintaining reduced distal radius fractures with cast immobilisation and analyse risk factors for redisplacement. A retrospective analysis of distal radius fracture manipulated between April 2011 and 2013 was conducted. Age, gender, fracture classification, ulna fracture, dorsal comminution and volar alignment were recorded. Reduction and redisplacement were measured using Sarmiento's modification of Lidstrom's system. 110 patients were included ; mean age 62.8 years and 83.4% female. The AO classification was used to grade initial fractures A

Outcome of diaphyseal pediatric forearm fractures following non-surgical treatment in a Level I Trauma Center.

OBJECTIVES: Pediatric diaphyseal forearm fractures are common injuries of childhood. Conservative modality of treatments is usually preferred when they are possible. We identified factors that may affect closed reduction success or lead to redisplacement in forearm diaphyseal fractures in children. METHODS: This was a retrospective study from a level I trauma center on patients up to 18 years of age who presented with forearm diaphyseal fractures from January 1, 2007, to December 31, 2015. Cases were obtained from medical records. Data were collected and confirmed by plain films and medical f

Humeral Supracondylar Fractures in Children: A Novel Technique of Lateral External Fixation and Kirschner Wiring.

Introduction: Supracondylar fracture of the humerus is the most common fracture around the elbow in children. Pinning with Kirschner wires (K-wires) after open or closed reduction is generally accepted as the primary treatment modality. However, it comes with the risk of persistent instability and if the K-wire is not inserted properly, it may cause displacement and varus deformity. We present our two-year experience with a new technique of lateral external fixation and K-wiring of the humeral supracondylar fracture. Materials and Methods: A total of seven children with irreducible Gartland Ty

Central acetabular fracture with dislocation treated by minimally invasive plate osteosynthesis.

Central acetabular fractures with dislocation are usually the result of high-energy trauma, resulting in joint incongruity, and are frequently associated with other injuries. Open reduction and internal fixation has been the standard treatment for acetabular fractures, but it is associated with extensive surgical trauma, and complications such as haematoma formation, iatrogenic nerve injury, and heterotopic ossification. We present the case of a 63-year-old female who sustained a central acetabular fracture of the hip with dislocation as a result of an automobile collision. Closed reduction of

[Does the degree of dislocation correlate with therapy procedure in supracondylar humerus fractures in childhood?].

Hundred and twenty-eight supracondylar fractures of the humerus were studied retrospectively after an follow-up time of 4.3 years (1 to 17.8 years). In 87 cases (68%) the operative procedure was the closed reduction and percutaneous crossed-pin fixation for 19 fractures type III (56%). 22 fractures type II (76%) and 46 fractures type I (85%), whereas 41 fractures were treated by open reduction and crossed-pin fixation. The findings were evaluated according to "Flynn's criteria" leading to the following results: "excellent" 77 times (60.2%), "good" 44 times (34.4%) "fair" 3 times (2.3%) and "po

Osteosynthesis in sacral fracture and lumbosacral dislocation.

Sacral fracture and lumbosacral hinge trauma are rare but serious lesions. Neurologic disorder is frequently associated, and nerve release may be required, with reduction and stabilization of the fracture. Management requires knowing the fracture lines and reduction maneuvers and the fixation techniques that may need to be associated. Three classifications allow these fractures to be well understood: the Roy-Camille classification identifies high transverse fractures and their displacement; the Denis classification identifies vertical fracture line location within the sacrum, which correlates

Open Reduction of a Fracture-Dislocation of the Shoulder in an 11-Month-Old Child: A Case Report.

CASE: An 11-month-old boy was brought to the emergency department because he was refusing to use his right arm. X-rays demonstrated an anteroinferior Salter-Harris I fracture-dislocation of the proximal humeral epiphysis. Closed reduction with sedation was unsuccessful, so open reduction under general anesthesia was pursued. At 2 years, the patient has made a full recovery. CONCLUSION: Pediatric shoulder fracture-dislocations are rare. Clinicians must have a high index of suspicion for nonaccidental trauma when evaluating such injuries. When this injury is encountered in patients younger than
