Preoperative imaging of sensorineural hearing loss in pediatric candidates for cochlear implantation.

Cochlear implantation is the only U.S. Food and Drug Administration-approved treatment for children with marked bilateral sensorineural hearing loss. It provides auditory benefits that range from simple sound detection to substantial word understanding. Improved hearing through cochlear implantation has been demonstrated to enhance the rate of language acquisition, enable development of spoken language, and advance literacy in deaf children. Magnetic resonance imaging and computed tomography both have roles in the preoperative assessment of inner-ear abnormalities, cochlear nerve deficiency, a…

Performance after timely cochlear implantation in prelingually deaf children with cerebral palsy.

OBJECTIVES: To investigate auditory perception, speech production, and language ability of prelingually deaf toddlers with cerebral palsy (CP) who were implanted within a sensitive period and who received proper speech therapy. Comparison of their outcomes with age- and sex-matched CI recipients without additional disabilities was also performed. METHODS: We retrospectively reviewed a cohort of pediatric CI in Samsung Medical Center. Eight CP subjects who received CI before 3 years of age and age-sex matched control recipients who had no additional disabilities except idiopathic sensorineural

Necrotizing meningoencephalitis mimicking cerebellopontine angle tumor as late complication following cochlear implantation.

We describe for the first time localized necrotizing meningoencephalitis as the cause of functional hearing loss, facial nerve palsy, and vertigo after cochlear implant (CI) surgery. Magnet resonance imaging (MRI) and computed tomography scans before CI surgery and after 3 years showed no abnormalities, especially no evidence of a tumor in the cerebellopontine angle (CPA). Due to recurrent facial nerve palsy the CI was explanted after 5 years in order to be able to visualize the CPA without artifacts caused by the CI in MRI scans. The MRI scans now showed a tumor in the CPA. Following removal

Partial deafness cochlear implantation at the University of Kansas: techniques and outcomes.

BACKGROUND: One of the most significant recent advances in cochlear implantation is the implantation of patients with residual hearing. These patients have a downsloping sensorineural hearing loss with poor speech discrimination and perform poorly with standard amplification. Studies using a variety of different electrode designs have demonstrated that it is possible to implant an inner ear and preserve residual hearing. Initial studies have demonstrated that a combination of residual acoustic hearing in the low frequencies with electrical stimulation in the mid- to high frequencies resulted i…

A case of bilateral cochlear implantation in single-sided untreated acoustic neurinoma.

Acoustic neurinoma affects the acousticofacial nerve and therefore in many cases is a contraindication for cochlear implantation and an indication for brainstem implant. Nevertheless benefit in these patients has been shown after tumour removal and cochlear implantation. The first case of bilateral cochlear implantation in a patient with single-sided untreated acoustic neurinoma is described here. In a 49-year-old woman with progressive hearing loss during the last 12 years we preoperatively diagnosed an acoustic neurinoma of the left side. After cochlear implantation of the right side she was…

Impaired fixation suppression is a risk factor for vertigo after cochlear implantation.

OBJECTIVES: To analyse the correlation between visual fixation suppression test results and the occurrence of post-operative vertigo in patients receiving a cochlear implant, and to compare this with other possible risk factors. METHODS: In a prospective study setting, caloric vestibular responses, visual fixation suppression and subjective vertigo symptoms were assessed in 59 adult patients undergoing cochlear implantation. These parameters were compared in patients with post-operative vertigo versus vertigo-free patients. RESULTS: Vertigo symptoms were reported by 49 per cent of patients. …

Cochlear implantation in postlingually hearing-impaired adults: choosing the most appropriate ear.

Choice of the most appropriate ear for CI in postlingually hearing-impaired adults is becoming more relevant as more patients are considered eligible for intervention. The aim of this study is to review factors that influence this choice and to formulate a flowchart. An extensive Medline search was performed. Factors can be divided into surgical, audiological, and patient factors. Surgical factors are anatomic variation and otological medical history. Both are divided in absolute and relative contraindications. Duration of deafness and residual hearing are combined in the audiological factor.

Extradural haematoma presenting as a contralateral sixth nerve palsy after cochlear implantation.

Intracranial complications after cochlear implantation are rare. The authors present the case of a 13 month old boy with a contralateral abducens nerve palsy following cochlear implantation that led to the diagnosis of an extradural haematoma on computerised tomography scanning. The abducens palsy resolved after evacuation of the haematoma and the patient made an excellent recovery. The literature is reviewed and the mechanism of injury discussed.

[Neuroradiological imaging in patients with sensorineural hearing loss prior to cochlear implantation].

Cochlear implantation (CI) is an established technique for enabling speech perception in patients suffering from severe bilateral sensorineural hearing loss (SNHL). Thorough preoperative radiological assessment is essential for correctly evaluating the indication for surgery and safely performing cochlear implantation. CT and conventional and functional MRI are available for radiological assessment. Therefore, knowledge of the most frequent congenital syndromal, non-syndromal, and acquired malformations of inner ear structures is fundamental. This article provides information about imaging tec…

Cochlear implantation and management of chronic suppurative otitis media: single stage procedure?

In a series of 360 patients who underwent cochlear implantation at our center, four patients (five procedures) had cochlear implantation with obliteration of the mastoid cavity and management of cholesteatoma as a single-staged procedure. Three patients were bilaterally deaf secondary to CSOM and had bilateral mastoid cavities, and in one patient congenital cholesteatoma was identified during cochlear implantation. A mastoidectomy or revision mastoidectomy with obliteration of the mastoid cavity and cochlear implantation was performed as a single stage procedure. Cholesteatoma reoccurred in on…
