Epithelioid angiomyolipoma of the kidney: a report of five cases with a prominent and diagnostically confusing epithelioid smooth muscle component.

We report five angiomyolipomas with a prominent component of epithelioid smooth muscle cells that occurred in patients from 20 to 48 (mean, 36) years of age. The tumors often posed problems in diagnosis, particularly with regard to distinction from renal cell carcinoma. Two patients had tuberous sclerosis. Two patients with more than 5 years' follow-up are alive and well. The epithelioid smooth muscle cells typically formed sheets that in two tumors were traversed by hyaline cords. The epithelioid cells ranged from medium sized and polygonal with slightly pleomorphic nuclei and eosinophilic cy

The relationship between well-being and job satisfaction for staff working on long stay wards for the elderly confused.

The psychological well-being and job satisfaction of nursing staff working on long stay wards for the elderly confused were examined in one health district. Feelings of well-being were not related to job satisfaction. However, feelings of unhappiness were found to be related to job dissatisfaction, this relationship was found in the ward managers (predominantly males) and not other qualified or unqualified staff. The implications of this finding are discussed.

Head shaking by visually impaired children: a voluntary neurovisual adaptation which can be confused with spasmus nutans.

Rapid, horizontal, pendular head oscillations were observed in 18 visually impaired children with nystagmus during intense visual fixation, and the characteristics of this behaviour were analysed. Head tilting and eye deviations also occurred in 14 of the children. Their symptoms and signs resembled spasmus nutans. Head shaking appeared to be a voluntary, learned, neurovisual adaptation to improve visual acuity. Accurate simultaneous recordings of eye and head movements are required to understand the pathophysiological significance of these head oscillations.

BEAR I Genetics Panel: An unexpected and troubling historical twist: The untold story of Hermann Muller's significant scientific confusion.

Hermann J. Muller, Nobel Prize recipient for producing gene mutations, was the de facto leader of the radiation genetics community from the mid-1920s until he died in 1967. Muller had a major impact on US hereditary/cancer risk assessment policies/practices, and the course of the secondary school biological sciences curriculum development in the US and worldwide. Despite these accomplishments, Muller was at the center of provocative controversies. Within this context, this paper reports the discovery of a letter (April 4, 1956) from Muller to Warren Weaver, Chair of the BEAR I Genetics Panel,

Untangling the Confusion: A Scoping Review of Terminology and Assessment Methods for Acute Confusional States Following Traumatic Brain Injury.

INTRODUCTION: Hospitalized adults with traumatic brain injury (TBI) often experience acute confusional states. However, conceptual clarity remains limited because of overlapping terminology, which hinders assessment. OBJECTIVES: This study aimed to map the terminology and assessment of acute confusional states in hospitalized TBI adults and examine their alignment with established definitions. METHODS: We conducted a scoping review (2013-2025) in 4 databases (MEDLINE, PsycInfo, CINAHL, and Google Scholar) following the Arksey and O'Malley framework. Peer-reviewed studies and clinical guideli…

Showing the Bonds-A Subtle but Important Difference in Figure Design that May Alleviate Student Confusion about ATP Hydrolysis.

A misconception among biology students is that breaking bonds in adenosine triphosphate (ATP) releases energy. This misconception may be related to imprecise representations of chemical bonding in common diagrams of ATP hydrolysis. We interviewed 33 undergraduate students and randomly assigned them to interpret a figure of ATP hydrolysis that either emphasized bond breaking in the reactants or the formation of new bonds in the products. Students who saw the figure emphasizing bond breaking were more likely to incorrectly classify ATP hydrolysis as endergonic, while students who saw the figure

Acute renal insufficiency, supraventricular tachycardia, and confusion after recombinant human bone morphogenetic protein-2 implantation for lumbosacral spine fusion.

The authors report on a case of a patient who received recombinant human bone morphogenetic protein-2 (rhBMP-2) to augment spinal fusion for the first and third of 3 lumbosacral fusion surgeries. After receiving rhBMP-2 the first time, the patient became febrile and developed mild acute renal insufficiency and transient supraventricular tachycardia (SVT). The second operation was complicated only by perioperative fever. When the patient received rhBMP-2 again during the third operation, he developed fever, acute oliguric renal insufficiency, symptomatic SVT with hypoxemia, confusion, and joint…

Avoid Confusion! Does Survival Processing Shape the Spontaneous Use of Learning Strategies for Distinguishing Edible and Poisonous Mushroom Twins?

Hunter-gatherers faced the survival threat of confusing edible mushrooms with their poisonous twins, imposing selection pressure on those who failed to detect subtle visual differences. Grounded in the ancestral priorities framework, which posits that the human mind is adapted to handle prehistoric challenges, we investigated whether humans spontaneously engage in learning strategies that allow a reliable discrimination to cope with the confusion threat-such as contrasting. We manipulated the labels of two mushroom types between subjects-poisonous and edible (survival threat) versus growing on…

Two regions, two lineages: Untangling two centuries of confusion between Machilus glaucescens and M. macranthus (Lauraceae) through integrative taxonomy.

The Asian genus Machilus (Lauraceae) of the Persea group is widely distributed across South and Southeast Asia. Like many other genera of Lauraceae, Machilus has received limited systematic attention, particularly in India. As a result, the nomenclature and systematic positions of Machilus glaucescens (Nees) Wight and M. macranthus Nees have remained unclear. In this study, we employed an integrative taxonomic approach to disentangle the taxonomy, phylogenetic relationships and distributions of M. glaucescens (restricted to Indo-Burma biodiversity hotspot) and M. macranthus (restricted to East

[Ethical Issues in Collaborative Decision-Making for Intractable Neurological Diseases: Focusing on Misunderstandings and Confusion regarding Advance Care Planning].

Medical professionals should play the role of a guide runner in collaborative decision-making. If the guide runner is too fast compared to the athlete, the athlete is disqualified. If he is too slow, he is nothing but a nuisance. The guide runner provides firm guidance to the athlete so that he does not go off course; however, as the athlete is ultimately the main character, the guide runner must respect the athlete's autonomy. Doctors and other medical professionals should guide patients so that they do not "go off course," but at the same time, support the patient, who is the main character,

COVID-19-related stress exacerbates the effect of child maltreatment on negative affect via increased identity confusion during adolescence.

INTRODUCTION: Exposure to childhood maltreatment may undermine the crucial developmental task of identity formation in adolescence, placing them at risk for developing negative affect. The current study investigated whether COVID-19-related stress intensified the indirect link between child maltreatment and adolescents' negative affect through identity confusion. METHOD: Using multidimensional assessments of child maltreatment (threat vs. deprivation), the study included a sample of 124 adolescents (Mage  = 12.89, SD = 0.79; 52% female) assessed before (January 2018 to March 2020) and during …

Head computed tomography findings in geriatric emergency department patients with delirium, altered mental status, and confusion: A systematic review.

BACKGROUND: Delirium, altered mental status (AMS), or confusion among older adults are common presentations to the emergency department (ED). We aimed to report the proportion of older ED patients presenting with delirium who have acute abnormal findings on head imaging. We also assessed whether anticoagulation, neurological deficits, trauma, or headache were associated with head imaging abnormalities in these patients. METHODS: A systematic review was performed using Ovid Medline, Embase, Clinicaltrials.gov, Web of Science, and Cochrane Central from conception to April 8, 2021. Citations wer…

Clinical evaluation and management of a 45-year-old man with confusion, psychosis, agitation, stereotyped behavior, and impaired speech.

Our patient Mr. A is a mentally and physically disabled gentleman. He was first diagnosed with bipolar disorder as a teenager. He incurred a lumbar spinal injury due to a motor vehicle incident in his 20s which led to weakness, numbness, and frequent infection over both of his lower extremities. He also developed alcohol addiction over the course of his life. Mr. A presented to our facility with complicated neuropsychiatric symptoms. By adopting various clinical strategies, we were able to control his symptoms of agitation, self-harm, mood swings, and stereotyped behavior. However, we were not

Apraxia of speech and the study of speech production impairments: Can we avoid further confusion? Reply to Romani (2021).

We agree with Cristina Romani (CR) about reducing confusion and agree that the issues raised in her commentary are central to the study of apraxia of speech (AOS). However, CR critiques our approach from the perspective of basic cognitive neuropsychology. This is confusing and misleading because, contrary to CR's claim, we did not attempt to inform models of typical speech production. Instead, we relied on such models to study the impairment in the clinical category of AOS (translational cognitive neuropsychology). Thus, the approach along with the underlying assumptions is different. This res…

Childhood emotional abuse and adolescent nonsuicidal self-injury: The mediating role of identity confusion and moderating role of rumination.

OBJECTIVE: According to Nock's (2009) integrated model, nonsuicidal self-injury (NSSI) is caused by the interplay of distal environmental and proximal intrapersonal risk factors. Based on this theory, the current study tested a moderated mediation model of family and individual characteristics to investigate how they impact adolescent NSSI. Specifically, we examined whether identity confusion mediated the association between childhood emotional abuse and NSSI, and whether this indirect effect was moderated by rumination. METHODS: 949 Chinese adolescents anonymously filled out questionnaires r…
