BACKGROUND: The role of B cells in resistance to Cryptococcus neoformans disease (i.e., cryptococcosis) is unknown. Given evidence that IgM(+) memory B cells are required for immunity to other encapsulated pathogens, we hypothesized that these cells might contribute to resistance to cryptococcosis.

An asymptomatic 71-year-old man was admitted for evaluation of a pulmonary tumor. Chest computed tomography revealed a cavitary tumor in the apical segment of the right lung. Histopathological examination of the resected lung revealed pulmonary cryptococcosis within a papillary adenocarcinoma. Macroscopic and histopathological findings speculated that papillary adenocarcinoma preceded pulmonary cryptococcosis.

Cryptococcosis is the third most common invasive fungal infection in solid organ transplantation, which usually occurs more than 6 months after the primary operation. In our series of 180 consecutive adult living-donor liver transplantation recipients, three (1.5%) had cryptococcosis and one of these patients died. The serum cryptococcal antigen examination was positive in all three patients who suffered from cryptococcosis. The serum cryptococcal antigen test might contribute to the early detection and treatment of cryptococcosis.

OBJECTIVE: The aim of our study was to review the CT findings of pulmonary cryptococcosis in 12 immunocompetent patients.
CONCLUSION: The CT manifestations of pulmonary cryptococcosis consist of pulmonary nodules or masses measuring 5-52 mm in diameter and focal areas of consolidation. The nodules and masses have a predominantly peripheral distribution in 80% of the cases. Cavitation of nodules or consolidation is seen in approximately 40% of the cases. The infection can be due to Cryptococcus neoformans var gattii or var neoformans.

A study was done to investigate the occurrence of Cryptococcus neoformans infection in patients admitted to Tikur Anbessa Hospital. Primary pulmonary cryptococcosis is thought to be relatively less common than other lung mycoses, but recently there has been an increase in reports of patients with this disease. Our report covers 12 cases of primary pulmonary cryptococcosis in which the diagnosis was based on medical examinations. The patients consisted of 11 men and 1 woman, aged 27 to 58 years. In an 11-year-old immunocompetent girl with protracted cryptococcal infection of the central nervous system, CT showed multiple areas of parenchymal calcification. MR imaging showed large gelatinous pseudocysts around the brain stem. These imaging features and the child's age are unusual for intracranial cryptococcosis.

The efficacy of the combination therapy of fluconazole plus flucytosine against pulmonary cryptococcosis was investigated. The minimal inhibitory concentrations of antifungal agents were measured by growth inhibition on Yeast-morphology agar plates, and the fractional inhibitory concentration index of each antifungal combination were calculated by checker-board titration. Cryptococcosis presenting as an intrabronchial mass is not a recognized cause of complete lung collapse. This case report illustrates this extremely rare manifestation of pulmonary cryptococcosis, which mimicked primary pulmonary carcinoma clinically, radiologically and bronchoscopically.

A case of adrenal cryptococcosis diagnosed by fine-needle aspiration biopsy cytology in a 58-yr-old man is presented. The organisms were easily seen with routine modified Wright stain (Diff-Quik) as variably sized yeasts, some with a brightly eosinophilic capsule. The diagnosis was confirmed with mucicarmine and silver stains. The identification of fungi with routine cytologic stains allows a rapid presumptive diagnosis of the infectious agent, collection of material for confirmatory special stains, and prompt initiation of therapy.

The AIDS epidemic has profoundly influenced the expression of deep-seated fungal disease in this country over the past 10 years. Previously an uncommon etiology of life threatening disease, deep-seated fungal infections with Cryptococcus neoformans, Histoplasma capsulatum, and Coccidioides immitis accounted for nearly 3000 AIDS index diagnoses in 1990. In connection with a case of pulmonary cryptococcosis, the clinical manifestations, radiological findings and therapeutic possibilities are discussed. Although pulmonary cryptococcosis is relatively uncommon, the diagnosis should be considered in any immunologically compromised patient with an abnormal chest roentgenogram.
